Digital health information is an important source of preventive health advice during pregnancy. Its public health value depends on whether advice is guideline-concordant, safe, transparent and understandable. This study aimed to assess the guideline concordance, content coverage, safety information, transparency and readability of Australian consumer-facing webpages providing advice about physical activity during pregnancy. We conducted a systematic, cross-sectional audit of English-language, freely accessible webpages affiliated with Australian organisations that provided substantive advice about physical activity or exercise during pregnancy. Searches were undertaken between June and July 2025, with data collection continuing until September 2025. Data were extracted using a structured checklist mapped to Australian pregnancy physical activity guidelines. Transparency was assessed using Journal of the American Medical Association (JAMA) benchmarks and readability using Flesch Reading Ease (FRE), Flesch-Kincaid Grade Level (FKGL) and Simplified Measure of Gobbledygook (SMOG). Sixty webpages were included. Most encouraged physical activity during pregnancy, but practical guidance and essential safety information were often incomplete. Fourteen webpages (23.3%) reported weekly exercise volume consistent with current guidelines. Fourteen (23.3%) included no warning signs, relative contraindications or absolute contraindications, and only four (6.7%) covered all three safety domains. Transparency was limited: authorship was reported by 12 webpages (20.0%), attribution by 16 (26.7%), disclosure by three (5.0%), and currency by none. Readability exceeded recommended levels for consumer materials; no webpage met the SMOG threshold of 8 or below. Australian online information about physical activity during pregnancy is often not fit for purpose as public-facing preventive health guidance. Minimum standards for safety content, transparency and readability may improve the quality and accessibility of digital antenatal health information.

Improving household decision-making during the perinatal period is a vital strategy for reducing low birth weight. In Sri Lanka, the prevalence of low birth weight has remained close to 17% for nearly two decades, and evidence on community-based participatory approaches to improving collective decision-making among pregnant women remains scarce. This quasi-experimental study, conducted between January and October 2022, evaluated a community-based health promotion intervention to improve collective household decision-making among 403 pregnant mothers in the North Central Province of Sri Lanka (intervention group: Anuradhapura district; comparison group: Polonnaruwa district). Grounded in an established, community-centred health promotion process model, the intervention was delivered through 71 Neighbourhood Action Committees using six community-developed participatory tools addressing community-identified determinants of poor household decision-making. Collective decision-making during pregnancy was significantly higher in the intervention group than the comparison group for five of six indicators (all < .05), with the largest differences for "items to buy for delivery" (58% vs. 36%) and "when to seek medical care" (64% vs. 50%). Postpartum collective decision-making on newborn expenditure and food and clothing spending was also significantly higher in the intervention group; decision-making on social activities and investments did not differ significantly between groups. Qualitative framework analysis identified five themes, including transformed spousal roles, enhanced financial dialogue, and increased maternal agency. Community-based participatory approaches using locally developed monitoring tools can significantly improve household-level collective decision-making during the perinatal period, contributing to greater equity in maternal health. Integration into routine antenatal services in lower-middle-income countries is recommended.

Civic engagement is an important example of health and health care inequity in the United States, with voting being an active form of civic engagement that allows citizens to be involved in the political processes that directly impact their health. Existing literature indicates that health care workers are willing to assist in voter registration efforts and are a trusted, nonpartisan source for patients. As such, Vot-ER was founded in 2019 to support nonpartisan voter registration efforts through health care sites, initially through emergency departments. Despite improvements in voter representation in recent elections, voter registration rates remain suboptimal in community health settings, where historically marginalized populations, young adults, and people with lower incomes-who may also find voting inaccessible-often go for their health care. We describe the lessons learned from health care workers in our pilot program who were provided Vot-ER badges for use at their community health sites and shared their experiences. The simplified introduction of the Vot-ER badge led to provider and patient use, though limited in number. Our survey of participants revealed that health care workers felt pressed for time and expressed a desire for a dedicated staff member to facilitate this conversation and/or signage and handouts that would act as support and reminders for both the providers and patients. Among this group, resources such as dedicated time, dedicated role, or signage and handouts would be necessary and non-negotiable to increase Vot-ER use. Insights from this pilot will be used to plan subsequent voter registration programs in our community health activities.

Public transportation has positive impacts for both individual and community health, but funding and scalability are political challenges. Understanding civic processes, such as voting, is key to advancing health equity through large-scale policy change. The purpose of this study is to understand voter support for tax-funded public transportation. This study employed a cross-sectional survey of randomly sampled voters in Kansas City, Missouri, collected from October 2023 to January 2024. Of the 2,577 respondents, 79% participated in the election of interest. Two-sample proportion tests for pairwise comparisons of support for taxes were used to calculate test statistics, -values, and effect sizes. Most participants supported the current (84.43%, = 1,719) and potential future (72.45%, = 1,475) taxes to support public transit. Recent bus use and proximity to a bus stop were significantly associated with current and future tax support. Notably, 48.26% of respondents unaware of the free fares supported the current tax compared to 71.06% of those aware ( < .001, h = 0.47). Free fare public transit may be a motivating factor for funding services among voters. Public health professionals and researchers should work to better understand and leverage the interplay between public policy, institutional legitimacy, and civic engagement to improve the upstream determinants of health.

Youth suicide prevention and crisis response require coordinated action across public health, behavioral health, education, health care, law enforcement, and community systems. However, local jurisdictions often lack practical tools to assess system readiness and prioritize system improvements across fragmented service systems. This article introduces the Youth Crisis Reporting and Response Maturity Model, a practice-based framework developed through a multicounty pilot initiative in California. The model was informed by participatory system mapping, county reporting data, and iterative input from technical assistance providers, state partners, and community stakeholders. It defines five core domains of system infrastructure: planning and collaboration, prevention and outreach, reporting, response, and data and continuous quality improvement. Together, these components describe a staged progression from fragmented processes to coordinated and standardized practices. During the final year of the pilot, the model was applied as a structured dialogue and technical assistance tool to support counties in assessing infrastructure, identifying gaps, and prioritizing improvements. Application of the framework facilitated cross-sector alignment, clarified roles and processes, and supported incremental system strengthening across domains. Rather than serving as a benchmarking instrument, the maturity model functions as a practical tool for operationalizing system readiness and guiding policy, systems, and environmental change. It provides a shared framework for practitioners and policymakers to sequence system improvements, strengthen coordination, and advance more integrated and equitable youth crisis response systems.

Gastric cancer remains a major prevention gap in the United States, disproportionately affecting immigrants from high-incidence regions. Although is the leading modifiable risk factor for non-cardia gastric cancer, systematic and equitable pathways for testing, treatment, and eradication confirmation are lacking, and evidence on downstream implementation outside hospital settings remains limited. These findings suggest that community-based screening paired with sustained navigation is feasible, and that the principal losses in gastric cancer prevention occur downstream rather than at risk identification or therapeutic efficacy. Approaches that explicitly address multistep, time-lagged care delivery are essential for translating eradication into population-level gastric cancer prevention in high-risk immigrant communities. We conducted a pragmatic, descriptive implementation evaluation of a culturally and linguistically tailored, community-anchored screening and navigation program, with outcomes assessed over a 6-month follow-up period in partnership with community and faith-based organizations serving Korean American populations. The workflow included multilingual outreach, a brief survey, on-site urea breath testing, privacy-protective result disclosure, and active navigation to support clinical evaluation, treatment completion, and post-treatment assessment. Among 323 participants screened, 58 (18.0%) had active infection, and all (100%) were successfully contacted for confidential result disclosure. Within 6 months, 31 (53.4%) completed clinical evaluation, 15 (25.9%) completed eradication therapy, and nine (15.5%) completed post-treatment retesting; eradication was confirmed in eight of nine retested participants (88.9%). Attrition occurred across the care cascade and primarily reflected structural and logistical barriers rather than treatment failure.

Intimate partner violence (IPV) impacts one in four individuals during the perinatal period. Doulas are instrumental sources of support; however, there are few trainings created by and for doulas on healthy relationship education and addressing IPV. In this practice note, we describe the development and evaluation of a mixed-media, hybrid, longitudinal training for a cohort of English- and Spanish-speaking community doulas on supporting IPV survivors during pregnancy, birth, and postpartum. A multidisciplinary team of IPV advocates, IPV survivors, doulas, community leaders, artists, and researchers co-created the training. The training comprised videos, podcasts, in-person sessions, and a longitudinal learning collaborative. Eleven doulas completed the training and provided feedback via surveys and semi-structured interviews. Participants found the training to be engaging, interactive, and insightful in learning about IPV. They valued hearing their colleagues and friends facilitate the training and appreciated the opportunity to reflect on their own experiences with IPV. Participants also described the importance of creating a linguistically affirming space and noted that complex topics like IPV require continuous skills-building opportunities, learning, and community support. Next steps include continuous learning and team building for the doulas through the longitudinal learning collaborative and revising and expanding the training based on doulas' experiences working with IPV survivors. We also aim to incorporate training feedback for future doula cohorts and enhance its sustainability and community-building aspects by using a train-the-trainer model.

This study centered the voices of lesbian, gay, bisexual, transgender, queer, and other (LGBTQ+) college students in vaping prevention research focused on electronic nicotine delivery systems (ENDS). We used a participatory approach guided by Centers for Disease Control and Prevention best practices to establish a Youth Council (YC) of six LGBTQ+ students (ages 18-24) from George Mason University and other Virginia colleges from August 2023. The recruitment strategy was purposeful and occurred through institutional emails, social media, and newsletters. Data included YC meeting minutes and a Zoom-based focus group with three members. The FGD transcript was inductively coded, and preliminary themes were validated by YC members Three themes emerged: (a) motivation; members sought research experience and valued contributing to inclusive projects; (b) challenges; scheduling conflicts, inconsistent attendance, and limited faculty interaction; and (c) perceived impact and sustainability; YC members perceived that the council expanded outreach and promoted inclusion but emphasized that continued faculty support, flexible engagement, and digital outreach were needed to ensure sustainability. Findings underscore the value of participatory frameworks for equitable engagement of LGBTQ+ youth in health research.

The increasing rates of breast cancer are often linked to a lack of awareness among women about prevention strategies, limited access to comprehensive, multidisciplinary treatment, and challenges related to resources and infrastructure needed for early detection, diagnosis, and treatment. Despite the availability of research and medical interventions, these advancements have not yet been fully integrated into practice and policy, particularly in the Philippines. This underscores the importance of dissemination and implementation (D&I) science in advancing the use of evidence-based interventions for breast cancer, with the ultimate goal of enhancing patient outcomes. The proposed D&I framework focuses on enhancing public and community engagement, as well as fostering the implementation of clinical practices aimed at breast cancer control. This article proposes a framework for coupling D&I science with breast cancer control and management initiatives in the Philippines.

Adolescent girls and young women in Tanzania experience stigma related to sexual and reproductive health (SRH) and contraceptive use, which can discourage care-seeking and constrain reproductive autonomy. This mixed-methods pilot study assessed the feasibility, acceptability, and preliminary outcomes of participatory stigma-reduction workshops delivered in urban and rural Kilimanjaro, Tanzania. Data were collected between February 7 and March 7, 2025. Forty adolescent girls and young women aged 15 to 20 participated in a 3-day workshop series facilitated by trained local female facilitators. The workshops were adapted from an established stigma-reduction curriculum and used participatory dialogue, critical reflection on gender norms, and peer learning. Participants completed pre- and post-workshop surveys assessing contraceptive use stigma, broader adolescent SRH stigma, and sexual double standards. Focus group discussions with participants and facilitators explored workshop experiences, acceptability, and perceived impacts. All participants completed the workshops and surveys, and qualitative findings indicated that the participatory format was engaging and acceptable. Contraceptive Use Stigma scores decreased from pre- to post-workshop ( < .001; Cohen's = -0.76), whereas changes in broader adolescent SRH stigma and sexual double standards were not statistically significant. Qualitative findings suggested increased comfort discussing SRH, reduced moral judgment toward contraceptive use, and greater confidence communicating with peers and adults, while community- and provider-level barriers persisted. These pilot findings support the feasibility and acceptability of participatory stigma-reduction workshops and justify longer-term evaluation using a controlled design.
