1. Evaluating gender differences in depression screening: diagnostic accuracy and measurement invariance of the DESY-PC questionnaire in primary care.
PMID:日期:2026-09-26Recent advances in depression diagnostics emphasize gender-specific evaluations to avoid measurement bias, given gender disparities in prevalence and symptom presentation. This study evaluated the psychometric performance of the newly developed "Questionnaire for the assessment of DEpression SYmptoms in Primary Care" (DESY-PC) in women and men. We conducted a secondary data analysis of a diagnostic cross-sectional study involving 431 primary care patients. Current depression diagnosis was assessed using the Structured Clinical Interview for DSM-V disorders (SCID-V-CV) as the reference standard. Gender-specific diagnostic accuracy was evaluated using Receiver Operating Characteristic (ROC) analyses, including Area Under the Curve (AUC), sensitivity, and specificity. AUC values were compared to those of the commonly used PHQ-9 (Patient Health Questionnaire-9). Measurement invariance across genders was assessed by Multigroup Confirmatory Factor Analysis (Multigroup-CFA). Among 431 participants (61% women, mean age 47.6 years), the DESY-PC demonstrated comparable diagnostic accuracy, with an AUC of 0.85 (95%CI: 0.80-0.91) for women and 0.90 (95%CI: 0.83-0.97) for men (p = 0.340). At the cut-off score of 16, sensitivity (women: 74%, men: 83%; p = 0.422) and specificity (women: 82%, men: 87%; p = 0.220) were comparable across genders. The PHQ-9 showed a significantly higher AUC for men (0.90) than for women (0.78; p = 0.048). Multigroup-CFA confirmed measurement invariance of the DESY-PC across genders. The DESY-PC demonstrated high and comparable diagnostic accuracy across genders and assessed the same constructs in women and men. These findings support its suitability as a gender-sensitive tool for depression screening in primary care.

2. Theta burst stimulation with 80 and 120% resting motor threshold do not differ in their antidepressant efficacy: a retrospective analysis.
PMID:日期:2026-09-26rTMS, particularly intermittent theta burst stimulation (iTBS), is an effective treatment for treatment-resistant depression; however, the optimal stimulation intensity remains uncertain. Clinical protocols typically use intensities between 80 and 120% of resting motor threshold (RMT), but direct comparative evidence is limited. We conducted a retrospective analysis of 215 patients with unipolar or bipolar depression who received iTBS at either 120% RMT (n = 108) or 80% RMT (n = 107). Depression severity was assessed using the Hamilton Depression Rating Scale (HAMD-21) and Major Depression Inventory (MDI) before and after treatment. Response (≥ 50% symptom reduction) and remission (HAMD-21 ≤ 7; MDI ≤ 14) rates were compared between groups using mixed ANOVAs, with Bonferroni correction for multiple comparisons (p < .025). In addition, non-inferiority analyses were performed. Both groups showed large, significant symptom reductions (HAMD-21: F(1,213) = 250.98, p < .001, partial η² = 0.541; MDI: F(1,213) = 247.45, p < .001, partial η² = 0.537). No significant group effects or group × time interactions emerged for either scale. In a formal non-inferiority analysis (ANCOVA adjusting for baseline severity), 80% RMT was non-inferior to 120% RMT on the HAMD-21 (adjusted difference - 0.90, 95% CI [- 2.61, 0.82]; margin 2.25 points), a result robust across sensitivity analyses. Response rates ranged from 30.84 to 38.32% and remission rates from 23.36 to 31.48%, with no significant differences between intensity groups. Side effects occurred in 20.56% (80% RMT) versus 27.77% (120% RMT) of patients. Notably, treatment-site pain necessitating intensity reduction occurred exclusively in the 120% RMT group (n = 5, 4.6%). iTBS at 80% RMT produced antidepressant outcomes non-inferior to iTBS at 120% RMT on the HAMD-21, with supportive MDI findings and better tolerability. These findings suggest a broader intensity range for antidepressant effects of iTBS and support the systematic investigation of lower-intensity protocols.

3. The role of social support in transdiagnostic symptom networks in affective and psychotic disorders: a cross-sectional and longitudinal network analysis.
PMID:日期:2026-09-26Symptoms and psychosocial factors interact dynamically across affective and psychotic disorders, yet longitudinal evidence remains limited. Building on the network theory of psychopathology, this study conceptualizes mental disorders as systems shaped by symptom-environment interactions. Using data from a two-site German cohort (N = 2743), we examined 36 psychopathological symptoms and 11 psychosocial factors using cross-sectional and longitudinal analyses. At baseline, transdiagnostic symptom networks were estimated and compared between patients with affective or psychotic disorders (N = 1569) and healthy controls (N = 1174). In addition, a separate baseline network including both symptom and psychosocial nodes (N = 1462) was estimated to identify psychosocial bridge variables, and a temporal symptom-psychosocial cross-lagged panel network (N = 642) was used to examine prospective associations over a two-year follow-up. The baseline transdiagnostic symptom network was denser in patients than in healthy controls. In the baseline symptom-psychosocial network, perceived stress and social support emerged as key psychosocial bridge variables. In the longitudinal cross-lagged panel network, social support showed the strongest overall predictive associations with follow-up nodes among psychosocial variables. These findings suggest that social support may be closely linked to symptom-environment associations across affective and psychotic disorders and warrant further investigation as a potentially modifiable psychosocial factor in transdiagnostic intervention research.

4. Clinical Global Impression-Stimulation Outcome Monitoring Tool (CGI-STIM): development and preliminary evaluation of a pragmatic clinical rating tool for electroconvulsive therapy.
PMID:日期:2026-09-26Electroconvulsive therapy (ECT) is highly effective for severe and treatment-resistant mental disorders, but concerns about cognitive side effects and enduring stigma continue to hinder its broader acceptance. Although treatment response and cognitive tolerability are both clinically important, their systematic assessment and documentation remain challenging in routine clinical practice. To address this gap, we developed the Clinical Global Impression-Stimulation Outcome Monitoring Tool (CGI-STIM), a pragmatic clinician-rated tool that integrates both domains into a single clinically interpretable outcome classification. We retrospectively analyzed 21 ECT series in 15 patients. Two independent clinicians applied the CGI-STIM using routine clinical documentation to classify treatment outcomes based on clinical response and cognitive tolerability. Feasibility and interrater agreement were evaluated. The CGI-STIM was successfully applied to all 21 ECT series. Outcome classification was feasible in all cases using routinely available clinical information. Average completion time was approximately 5 min. Interrater agreement was excellent (quadratically weighted Cohen's κ = 0.84). The resulting classifications reflected varying combinations of clinical improvement and cognitive tolerability, illustrating the instrument's ability to capture multidimensional treatment outcomes. In this preliminary retrospective evaluation, the CGI-STIM was feasible to apply to routine clinical documentation and showed strong interrater reliability. These findings do not establish validity, prospective reliability, sensitivity to change, or clinical utility. Future prospective studies in larger ECT samples are required to evaluate these measurement properties and the potential utility of the CGI-STIM for routine outcome monitoring.

6. Bilateral theta burst stimulation as an add-on to schema therapy in major depression: methods and design of a randomized, sham-controlled study.
PMID:日期:2026-09-25Schema therapy (ST) is an integrative psychotherapeutic treatment that may be as effective as cognitive behavioural therapy in treating depression. Novel approaches are focusing on augmenting psychiatric treatments with brain stimulation techniques, like repetitive transcranial magnetic stimulation (rTMS), to enhance efficacy. Therefore, this randomized, sham-controlled, prospective and double-blind study (for two of the three treatment groups) will aim to evaluate the effects of augmenting ST with bilateral theta burst stimulation (TBS), a specialized rTMS protocol, in patients with major depression receiving routine ST inpatient or day-hospital care. We hypothesize that active TBS will show a greater reduction in depressive symptoms than sham and no TBS (control). Approximately 120 patients with major depression who are undergoing 8- to 10-week ST will be randomly assigned to additionally receive 4-weeks of bilateral active TBS, bilateral sham TBS, or no TBS. TBS will be administered as intermittent TBS (iTBS) to the left side of the dorsolateral prefrontal cortex to increase and continuous TBS (cTBS) to the right to decrease excitability. Depressive symptoms will be assessed with the Montgomery-Åsberg Depression Rating Scale (MADRS) and the Beck Depression Inventory-II (BDI-II). Functional levels will be assessed with the Global Assessment of Functioning (GAF) scale, and therapy-specific changes with the Young Schema Questionnaire (YSQ-S3R). Differences in efficacy between groups will be analyzed by linear mixed model repeated measures. This forthcoming study is expected to provide important insights into potential benefits of combining ST with bilateral TBS in patients with major depression and may lead to more effective therapeutic strategies. 7. Associations between smooth pursuit, predictive pursuit, anomalous self-experiences, and psychotic-like experiences.
PMID:日期:2026-09-21Oculomotor dysfunction is well-documented in the psychosis spectrum, particularly deficits in smooth pursuit eye movements. These disruptions are thought to serve as endophenotypic markers of psychosis vulnerability and more recent work has linked oculomotor dysfunction with alterations in agency, a core feature of the psychosis spectrum. Understanding this link between oculomotor impairment, agency, and psychosis can provide insight into key mechanisms of psychosis vulnerability. To address this gap, we utilized an eye tracking paradigm to examine smooth and predictive pursuit in 100 young-adults, recruited with an enriched sampling strategy to represent a range of severity of distress associated with psychotic-like experiences. Relationships with positive psychotic-like experiences, agentic symptoms, depression, anxiety, and cognitive symptoms were investigated. There was a significant relationship between distress associated with overall positive psychotic-like experiences and bizarre experiences with frequency of catch-up saccades on both smooth pursuit (r = 0.25, p = 0.012; r = 0.27, p=0.036) and predictive pursuit (r = 0.29, p = 0.004; r = 0.36, p=0.002) trials, and distress from positive psychotic-like experiences with phase difference on predictive pursuit trials (r=-0.23, p = 0.023). There were no significant relationships between anomalous self-experiences and performance on either task. Performance on the predictive pursuit task was related to cognitive performance (r = 0.22, p = 0.039), but relationships between psychotic-like experiences and predictive pursuit remained significant after controlling for cognitive performance. No other secondary clinical symptoms were related to performance on either task. The current study supports theory suggesting smooth pursuit as an endophenotypic marker of psychosis risk and presents predictive pursuit as a promising paradigm for assessment in psychosis vulnerability populations.

8. Results of the validated German Perceived Stress Questionnaire (PSQ) from a large German cohort of healthy adults and different disease groups compared to historical reference values.
PMID:日期:2026-09-21Reference values of the validated German Perceived Stress Questionnaire (PSQ) were published in 2005 for healthy adults and individuals with different clinical conditions. We compared historical values with PSQ from the large German cohort participating in "Risk Factors for COVID-19 Vaccine Failure study (RisCoin). In late 2021, we enrolled 4115 adults: hospital employees or patients with inflammatory bowel disease (IBD) or psychiatric disorders with ≥ 2 COVID-19 vaccinations. Participants completed extensive questionnaires including PSQ. Healthcare workers (HCW) were categorized as HCW-healthy or HCW-plus (any chronic medical condition). PSQ total and domain scores (worries, tension, joy, demands) were compared to reference values. Multivariable logistic regression identified factors associated with cohort-specific high scores (mean + 1SD). HCW-healthy (N = 1998, mean age ± SD: 38 ± 12 years) reached higher mean total PSQ scores than historical healthy individuals (36 ± 18 vs. 33 ± 17, respectively, p<0.001), but lower scores (40 ± 19, p < 0.001) than HCW-plus (N = 1717, 43 ± 13 years). Female HCW-healthy perceived higher stress than males, especially at younger age. Participants with IBD (N = 161, 45 ± 15 years) exhibited lower scores than historical IBD patients (38 ± 19 vs. 43 ± 17, p < 0.001); psychiatric patients (N = 100, 41 ± 15 years) showed the highest scores (60 ± 19). Among HCW, female gender, frequent consumption of ready-to-eat meals/foods, and living with children were associated with higher, whereas regular physical activity and healthy dietary habits with lower PSQ scores. Assuming representativeness of our cohort, these PSQ data could serve as updated benchmark for future research.

9. Adverse event reporting patterns of selective serotonin reuptake inhibitors (SSRIs): disproportionality analysis using the European spontaneous reporting system.
PMID:日期:2026-09-19We analysed adverse event reporting patterns associated with SSRIs using data from the European pharmacovigilance database, EudraVigilance. A descriptive analysis of individual case safety reports (ICSRs) was conducted between January 2020, 1st and December 2024, 31st. Disproportionality analysis were conducted for the two most frequently reported system organ classes (SOCs) and the three most frequently reported preferred terms (PTs). During the study period, 21,665 ICSRs reporting at least one SSRI as suspected drug were retrieved. A total of 78,132 adverse events were identified, and sertraline (N = 26,161; 33.5%) was the most reported SSRI. Most events were not serious, and the most reported criterion was Other Medically Important Condition (N = 19,603; 25.1%). The most frequent outcome was "Unknown" (N = 27,555). In the disproportionality analysis, for the SOC "Psychiatric Disorders", escitalopram and fluvoxamine showed a higher disproportionate reporting (ROR 1.08; 95% CI 1.03-1.13; ROR 1.15; 95% CI 1.01-1.31, respectively). For the SOC "Nervous System Disorders", paroxetine showed a higher disproportionate reporting (ROR 1.10; 95% CI 1.04-1.16). Headache higher disproportionate reporting with sertraline (ROR 1.17; 95% CI 1.04-1.30), while dizziness with escitalopram (ROR 1.42; 95% CI 1.25-1.61), and somnolence with fluvoxamine and paroxetine (ROR 1.86; 95% CI 1.27-2.72; ROR 1.58; 95% CI 1.33-1.89, respectively). SSRIs showed homogeneous adverse event reporting patterns, with relatively few differences in disproportional reporting between individual drugs. These results reinforce the importance of continuous pharmacovigilance monitoring.

10. Effect of ECT-anesthetics on reorientation time.
PMID:日期:2026-09-19Electroconvulsive therapy (ECT) remains one of the most effective treatments in psychiatry, but cognitive side effects still pose a clinical concern. Delayed postictal reorientation is linked to worse cognitive outcomes, adversely affecting patient-perceived treatment tolerability and acceptability, while also increasing post-ECT and post-anesthesia care demands. Despite its clear clinical and operational relevance, comparative evidence on anesthetic effects remains scarce. This study therefore investigates the impact of different anesthetic agents (S-ketamine, propofol, etomidate, thiopental, and the combination of S-ketamine and propofol) on reorientation time following ECT. Retrospective data from 5148 ECT sessions were analyzed using a linear mixed-effects model controlling for age, sex, diagnosis, stimulation dose, electrode placement, seizure quality, ECT session number, need for restimulation, ECT duration, and postictal agitation. Compared with S-ketamine, propofol reduced reorientation time by 6.2 min, and the combination of S-ketamine and propofol reduced reorientation time by 1.5 min. Neither thiopental nor etomidate differed significantly from S-ketamine. The differences may be attributed to the distinct pharmacodynamic profiles of the anesthetic agents. The findings demonstrate that anesthetic choice is a significant and independent determinant of reorientation time following ECT, with propofol and the combination of S-ketamine and propofol associated with markedly faster cognitive recovery compared to S-ketamine.
