2. Improving early detection of critical congenital heart disease in a high-performing prenatal screening system: the OXYNAT interventional cohort study.
PMID:日期:2026-09-29Early detection of critical congenital heart disease (CHD) is essential to prevent cardiovascular collapse and improve neonatal outcomes. While prenatal screening has advanced significantly in high-income countries, postnatal detection remains a safety net, particularly for left-sided lesions. Although pulse oximetry (PO) screening has been evaluated in several settings with high prenatal detection rates, the added value of combining it with systematic paediatric examination, and its impact on care pathways remains incompletely characterised in such contexts. To evaluate the diagnostic performance and care pathway impact of PO screening-alone and combined with routine paediatric examination-for detecting critical CHD in a high-performing regional prenatal screening system. The OXYNAT study was a prospective, population-based interventional cohort in 28 maternity hospitals in Nouvelle-Aquitaine, France. 19 639 newborns were enrolled. PO was performed between 6 and 24 hours of life, followed by routine paediatric examination. Critical cases with CHD were identified through active multisource surveillance and followed for 1 year (supplemental file 1, visual abstra). Among the 14 infants diagnosed postnatally with a critical CHD, one death occurred in relation to delayed diagnosis of truncus arteriosus. PO alone yielded a sensitivity of 53.8% and specificity of 99.2% for detecting critical CHD. The combined strategy with paediatric examination improved sensitivity to 92.9% while maintaining high specificity (97.7%). However, only 4 of 14 infants postnatally followed an optimal care trajectory. Delays were mainly due to misinterpretation of early signs, incomplete echocardiographic assessment and coordination gaps. In a healthcare setting with high antenatal CHD detection, PO alone is insufficient. Integrating systematic paediatric examination significantly improves diagnostic performance. NCT03078218.

3. Clinical performance and practical implications of continuous glucose monitoring in very preterm infants: a cohort study.
PMID:日期:2026-09-29To explore the clinical performance of continuous glucose monitoring (CGM) in very preterm infants during the first week of life, using data from the multicentre Real time continuous glucose monitoring (REACT) randomised controlled trial (RCT) and to consider how CGM can best support neonatal glucose management. Prespecified secondary analysis of paired sensor glucose (SG) and blood glucose (BG) measurements from the REACT RCT. 13 neonatal intensive care units (NICUs) in the UK, Spain and the Netherlands. 155 very preterm infants (≤1200 g birth weight or <34 weeks' gestation) with CGM data collected during the first week of life. Mean absolute relative difference (MARD), ISO 15197:2013 system agreement, Bland-Altman bias, error grid analysis and clinical contextualisation of CGM performance. Across 2223 SG-BG pairs, overall MARD was 11.9%, and 98%-99% of values fell within clinically acceptable error grid zones. SG tended to be less accurate at BG extremes, although error grid analyses show that clinical risk from discrepancies was low. CGM detected episodes of dysglycaemia not captured by intermittent sampling. Glucose variability did not materially influence accuracy. CGM can provide clinically meaningful information in very preterm infants despite modest analytical inaccuracy when compared with single point-of-care values. Its greatest utility lies in continuous trend monitoring, early detection of silent dysglycaemia and reduced invasive sampling, rather than replacement of diagnostic blood testing. Clinicians should interpret SG values in context, confirm unexpected extremes with BG and focus on patterns rather than single values.

4. Association between cerebral proton magnetic resonance spectroscopy and school-age neurodevelopment after perinatal asphyxia and therapeutic hypothermia: a cohort study.
PMID:日期:2026-09-24To assess the association of early proton magnetic resonance spectroscopy (¹H-MRS) metabolite ratios with adverse outcomes at 5-8 years in infants with hypoxic-ischaemic encephalopathy (HIE) undergoing therapeutic hypothermia (TH) and to compare the association with outcome at 5-8 years versus outcome at 2 years. Single-centre, retrospective cohort study of 91 infants with moderate to severe HIE undergoing TH between 2011 and 2020, ¹H-MRS performed between day 4 and 6 after birth and outcome at 5-8 years. The subgroup with both 2-year and 5-year to 8-year outcomes (n=81) was used for comparative analyses. Adverse outcome at 5-8 years was defined as death or moderate-severe disability (total intelligence quotient score <85, Movement Assessment Battery for Children II <15th percentile, cerebral palsy and severe sensory impairment). Among 91 infants, 49 (54%) had adverse outcomes at 5-8 years, including 20 who died. The lactate/N-acetylaspartate (Lac/NAA) peak-area ratio was the strongest predictor of adverse outcome at school age (area under the curve (AUC) of 0.90 and OR 1.11 per 0.01 increase (95% CI 1.05 to 1.16)).The predictive performance of neonatal ¹H-MRS was slightly higher at 2 years, with Lac/NAA AUC 0.93 (OR 1.11, 95% CI 1.05 to 1.16) versus 0.89 (OR 1.09, 95% CI 1.04 to 1.15) at 5-8 years, with comparable effect sizes indicating preserved long-term association. Neonatal ¹H-MRS biomarkers, especially Lac/NAA peak-area ratio, were associated with adverse outcomes at school age. Including ¹H-MRS in routine neonatal imaging could improve prognostication and guide early interventions.

5. Child's daily life at 10 years following moderate, late preterm, or early term birth: results from EPIPAGE-2 and ELFE cohorts.
PMID:日期:2026-09-23Preterm birth includes four subgroups: extremely (<28 weeks' gestation), very (28-31), moderate (32-33, MPT) and late (34-36, LPT). While school attainment is well-studied across subgroups, other aspects of daily life are rarely reported for MPT, LPT and early term (37-38, ET) or are explored only qualitatively. Assess daily life, including school and quality of life at age 10 years for MPT, LPT and ET compared with full-term (39-40, FT) peers. Children born in 2011 from two birth cohorts (ELFE: ≥33 weeks; EPIPAGE-2: ≤34 weeks) completed a 10-year follow-up via telephone interview (n=8373) and home visit (n=6418). Weighting addressed selection bias. Outcome-wide regressions (modified Poisson, linear) were used to calculate adjusted relative risks (aRR) and beta-coefficients (β). MPT had a higher risk of receiving any school support (aRR 1.67, 95% CI 1.10 to 2.54). Their risk for difficulties in French and mathematics was, respectively, 1.32 (95% CI 1.01 to 1.74) and 1.50 (95% CI 1.13 to 1.99) times higher, while school-related quality of life was 0.30 (95% CI -0.58 to -0.02) SD lower relative to FT. They were 2.02 times more likely to be non-swimmers (95% CI 1.24 to 3.29). Both MPT/LPT were less likely to eat school meals but reported higher overall well-being scores (+0.16 SD and +0.13 SD, respectively). The study sample may under-represent socioeconomically deprived groups. MPT/LPT children's daily lives differ from those of FT children in some respects. Those discrepancies should be explored further to better understand needs and accommodate them. Findings may not generalise to disadvantaged groups.

6. Neonatal automated oxygen titration algorithms: response characteristics and performance.
PMID:日期:2026-09-23Neonatal respiratory support devices now have algorithms for automated adjustment of oxygen concentration (fraction of inspired oxygen (FiO)) in response to oxygen saturation (SpO) disturbances, but their performance has not been systematically examined. We investigated the response of six available devices and algorithms to standardised and simulated SpO disturbances. Bench-top study in neonatal centres. Devices under study received SpO input from a vital signs simulator at 1 Hz; device response was recorded as measured FiO in the respiratory circuit. Response to protracted hypoxaemia (SpO=80%) and hyperoxaemia (SpO=98%) was examined in several test conditions. In closed-loop testing, measured FiO was combined with abstracted patient data in an oxygenation simulator, generating novel SpO values input to the device. Device capability in keeping simulated SpO in target range (91-95%) was examined, as was FiO adjustment frequency and propensity for FiO oscillation. The FiO trajectories in response to SpO disturbances differed considerably between devices. Maximum rate of sustained FiO escalation (ΔFiO/min) during hypoxaemia was Avea: 0.30; Fabian: 0.20; Leoni: 0.20; SLE: 0.10; Sophie: 0.062; Vapotherm: 0.30. Corresponding values for FiO reduction during hyperoxaemia were 1.20, 0.20, 0.20, 0.13, 0.53 and 1.32. In closed-loop testing, average SpO target range time proportion was 51%, 52%, 55%, 77%, 64% and 53% for the six devices, respectively, with 1.9-5.1 automated FiO adjustments/min, in some cases with a prominent oscillatory pattern. Automated oxygen control devices exhibit considerable variation in their FiO response to SpO disturbances, with important consequences for SpO targeting potential and the pattern of FiO adjustment.

8. Bloodstream infections in young infants: a cohort study.
PMID:日期:2026-09-16The burden of bloodstream infection (BSI) is highest among infants. We aim to describe temporal trends in the incidence and characteristics of BSI in young infants. Birth cohort study. Queensland, Australia. All <90-day-old infants with culture-confirmed BSI detected by Pathology Queensland from 2000 to 2019. The annual incidence of BSI was calculated using live births in public hospitals. Infections were categorised as early onset (0≤3 days) or late onset (4<90 days). Comorbidities were defined using a paediatric complex chronic conditions classification system. Temporal trends were examined using time series models. In total, 2327 incident BSIs occurred in 2173 infants. Late-onset BSI comprised 67% (n=1568). Comorbidities were present in 25% (n=188) of early-onset and 48% (n=716) of late-onset BSI. Common pathogens were (21%, n=501), Group B Streptococcus (GBS) (21%, n=493) and (14%, n=338). Overall incidence was 2.98 per 1000 live births (early-onset 1.01, late-onset 1.96), which increased over time (incidence rate ratio 1.01 (95% CI 1.00 to 1.01)). The incidence of Enterobacterales BSI increased by 1.96% each year (1.02 (1.01 to 1.03)) and GBS by 1.91% (1.02 (1.01, 1.04)). Early-onset GBS increased by 11% per year from 2014 to 2019 (1.11 (1.04 to 1.19)). The overall case fatality rate was 8.8% (early onset 6.4%, late onset 10%). The incidence of GBS and Enterobacterales BSI has risen in young infants in Queensland, especially for early-onset GBS since 2014. These findings highlight the increasing importance of Enterobacterales infections in young infants and the need to reconsider prevention strategies for early-onset GBS.

9. Paediatric outcomes after maternal nipocalimab for haemolytic disease of the fetus and newborn: an open-label, single-arm study.
PMID:日期:2026-09-07To describe haemolytic disease outcomes, growth, neurodevelopment and health-related quality of life (HRQoL) through 24 months of life in infants exposed to nipocalimab in pregnancies at high risk of early-onset severe haemolytic disease of the fetus and newborn (EOS-HDFN). A multicentre, open-label, single-arm trial. Centres with expertise in HDFN management. Pregnant individuals with previous EOS-HDFN and maternal anti-D titres ≥32 or anti-K titres ≥4. Weekly nipocalimab (30 and/or 45 mg/kg) from 14 to 35 gestational weeks. Infants' cord-blood alloantibody titres at birth, HDFN management, growth through 6 months, neurodevelopment (caregiver-reported Ages and Stages Questionnaire, third edition (ASQ-3)) and HRQoL assessments through 24 months of life. Of 13 pregnancies, 12 resulted in live births; one fetal loss occurred due to intrauterine transfusion (IUT) complications. A single simple transfusion was administered in one of seven infants delivered after a maternal nipocalimab course without IUTs, where cord blood alloantibody titres were <8. Multiple transfusions (one to seven simple transfusions/infant; one exchange transfusion) were administered to five infants delivered after early nipocalimab discontinuation and IUTs, with cord-blood alloantibody titres ranging from 512 to 32 768. No unusual growth patterns were observed through 6 months. No neurodevelopmental delays were identified through 24 months of life, with mean ASQ-3 domain scores within normal ranges. HRQoL outcomes were positive across physical, emotional, social and cognitive functioning. Maternal nipocalimab in pregnancies at high risk of EOS-HDFN may reduce adverse neonatal outcomes correlating with low cord-blood alloantibody titres, without affecting growth through 6 months, neurodevelopment or HRQoL through 24 months of life. NCT03842189.

10. {"_":"Effect of brewers' yeast or beta-glucan derived from on breast milk supply following preterm birth: the BLOOM randomised controlled trial.","i":["Saccharomyces cerevisiae"]}
PMID:日期:2026-09-01Breast milk is the optimal source of nutrition for preterm infants; however, low breast milk production is common following a preterm birth. This study aimed to determine if taking brewers' yeast or beta-glucan improves daily expressed breast milk volume. Randomised, blinded, parallel, placebo-controlled trial. Three Australian tertiary-level neonatal units. Mothers with a singleton or twin pregnancy who gave birth at <34 weeks' gestation. Mothers were randomised within 72 hours of birth into three parallel groups in a 1:1:1 ratio to receive either brewers' yeast, beta-glucan or placebo capsules for 7 days. Total expressed breast milk volume over a 24-hour period on day 7 of intervention. A total of 105 mothers underwent randomisation between August 2022 and April 2024 (36 brewers' yeast, 35 beta-glucan and 34 placebo). The adjusted mean difference in daily expressed breast milk volume was 94 mL/day (95% CI -51 mL/day to 239 mL/day) between the brewers' yeast and placebo groups and -25 mL/day (95% CI -173 mL/day to 123 mL/day) between the beta-glucan and placebo groups. Maternal side effects were similar across groups. We found no clear effect of short-term administration of brewers' yeast or beta-glucan on breast-milk production following preterm birth; both interventions were well tolerated. Given the small sample size, these findings do not rule out the possibility of a clinically meaningful benefit of brewers' yeast and suggest further research with a larger sample size may be warranted to clarify the potential clinical impact.
