1. Comparative Analysis of Antidepressant Mechanisms and Side Effects: Network Pharmacology and Molecular Docking Study of Amitriptyline, Mirtazapine, and Escitalopram for Treating Major Depressive Disorder.
PMID:日期:2026-10-02Amitriptyline, mirtazapine, and escitalopram are effective antidepressants for treating major depressive disorder (MDD). Our study aimed to use computational biology to understand the mechanisms for the similarities and differences in efficacies and side effects of these drugs. Potential targets for MDD and the three drugs were obtained using public databases. Venn diagrams and protein-protein interaction (PPI) networks were constructed. Drug-target-pathway (D-T-P) networks were built using Cytoscape, and core targets were filtered using CytoNCA and CytoHubba plugins. Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway analyses were conducted using the Database for Annotation, Visualization, and Integrated Discovery. Ligand compounds and receptor proteins were processed with PyMOL and AutoDock. The binding of amitriptyline, mirtazapine, and escitalopram with core targets was predicted by molecular docking using AutoDock Vina. The results were visualized using PyMOL and protein-ligand interaction profiler (PLIP). Forty-eight overlapping targets and 66, 50, and 30 individual targets for MDD treatment were identified for amitriptyline, mirtazapine, and escitalopram, respectively. Critical pathways included "Neuroactive ligand-receptor interaction," "Calcium signaling pathway," and "Serotonergic synapse." Key targets, indicating a wide range of effects on cellular activities and signaling mechanisms, included D(2) dopamine receptor (DRD2), CYP2D6, monoamine oxidase type B (MAOB), solute carrier family 6 member 3 (SLC6A3), solute carrier family 6 member 4 (SLC6A4), 5-hydroxytryptamine receptor 2C (HTR2C), and CHRNA4. MAOB had the highest binding affinity with amitriptyline, followed by SLC6A4 and DRD2. Binding stability was enhanced by salt bridges and π-stacking interactions. Mirtazapine formed strong binding affinities with HTR2C, SLC6A4, and CHRNA4 through hydrogen bonds and π-stacking interactions. Escitalopram exhibited the highest affinity for SLC6A3, followed by HTR2C. All three drugs regulated serotonin, norepinephrine (NE), and dopamine, but target specificity, pathways, and binding patterns differed. Amitriptyline exhibited stronger antidepressant effects and broader mechanisms but a higher risk of side effects. Mirtazapine's dual mechanism regulates broad targets with reliable efficacy and fewer side effects. Escitalopram selectively acts on serotonin-related targets, with narrow actions and potentially fewer side effects. These findings align with the clinical profiles of the antidepressant efficacy and side effects.

2. A Randomized Controlled Trial of a Digital Stepped-Care Program to Reduce Anxiety and Depressive Symptoms Among Long-Term Care Workers With Psychological Distress: The REPICAL Study.
2. 一项数字阶梯式护理程序减少长期护理工作者心理困扰中焦虑和抑郁症状的随机对照试验:REPICAL研究PMID:日期:2026-10-02Long-term care workers (LTCWs), encompassing those in long-term care facilities and home-care settings, constitute a particularly vulnerable population to psychological distress. This study aimed to evaluate the effectiveness of a digital stepped-care program, adapted from two World Health Organization (WHO) psychological interventions-Doing What Matters in Times of Stress (DWM) and Problem Management Plus (PM+)-in reducing anxiety and depressive symptoms among LTCWs. A parallel-group randomized controlled trial was conducted. Participants were LTCWs from Catalonia (Spain) experiencing psychological distress (Kessler Psychological Distress Scale, K10 ≥ 16). All participants received Psychological First Aid (PFA), and those in the intervention group subsequently received the digital stepped-care program: DWM as step 1, and if psychological distress persisted (K10 ≥ 16), PM + as step 2. The primary outcome was the between-group difference in self-reported depressive and anxiety symptoms at week 21 (2-month post-program follow-up), measured using the Patient Health Questionnaire-Anxiety and Depression Scale (PHQ-ADS). Secondary outcomes included depressive and anxiety symptoms measured separately, post-traumatic stress disorder (PTSD) symptoms, and health-related quality of life (HR-QoL; measured using PHQ-9, 7-item Generalized Anxiety Disorder [GAD-7] scale, PCL-5, and EuroQol 5-dimensional descriptive system-5-level [EQ-5D-5L], respectively). Between October 2022 and May 2024, 165 participants were randomized (intervention: = 83; control: = 82). At week 21, depressive and anxiety symptoms were significantly lower in the intervention group, with moderate to large effect sizes (mean difference = 4.2, 95% CI: 1.1-7.3; Standardized effect size [SES] = 0.8, 95% CI: 0.1-1.4). Secondary outcomes also showed significantly greater improvement in the intervention group, except for HR-QoL, for which no differences were found. This study provides evidence supporting the effectiveness of a digital and scalable stepped-care program to support LTCWs' mental health. These robust findings may inform occupational and public health strategies for supporting LTCWs' experiencing psychological distress. ClinicalTrials.gov identifier: NCT05526235.

3. Dual Sensory Impairment and Depressive Symptoms: Association and Pathways in Three Cohorts and a Meta-Analysis.
PMID:日期:2026-09-29The overall impact of dual sensory impairment (DSI) on depressive symptoms and its underlying mechanisms remains unclear, particularly across different culture contexts. This study aimed to examine the association between DSI and depressive symptoms in three distinct populations while exploring the mediating roles of social engagement and cognitive function. Participants were recruited from three aging cohorts: the China Health and Retirement Longitudinal Study (CHARLS), the Health and Retirement Study (HRS), and the English Longitudinal Study of Ageing (ELSA). Harmonized survey wave data (2011/2012, 2015/2016, and 2018) were analyzed. Depressive symptoms were assessed using the Center for Epidemiologic Studies Depression Scale (CES-D), and DSI was defined based on self-reported hearing and vision impairments. To address potential confounding, logistic regression models were adjusted for demographic, lifestyle, and health factors. Mediation analyses were conducted using the PROCESS Macro (model 4) to examine the roles of social engagement and cognitive function. Furthermore, a meta-analysis was performed by integrating the results of the present study with those from previous cohort studies. The study included 19,310 participants from the three cohorts. Separate analyses showed that DSI was significantly associated with higher odds of depressive symptoms across these cohorts (odds ratio [OR] = 1.24, 95% confidence interval [CI] = 1.15-1.33 in CHARLS; OR = 2.04, 95% CI = 1.78-2.31 in HRS; OR = 2.40, 95% CI = 1.79-3.00 in ELSA). A meta-analysis confirmed this association (OR = 1.73 and HR = 1.30). This relationship was partially mediated by reduced social engagement (2.31%-6.35%) and cognitive decline (5.72%-14.00%). DSI is consistently associated with depressive symptoms in middle-aged and older adults across China, the United States, and England. Both reduced social engagement and cognitive decline significantly mediated this association, with comparable effect sizes. The mediation effects are largely robust, with instability observed only in one sensitivity analysis.

4. Cross-Sectional and Longitudinal Associations Between Dietary Intake and Depressive Symptoms in Young South African Adults: The African-PREDICT Study.
PMID:日期:2026-09-29Depression is highly prevalent among young adults worldwide. While research links health behaviours, such as dietary intake, to depression, few studies have examined these associations in low- and middle-income countries (LMICs), including South Africa. This study investigated whether dietary intake was associated with risk of depression in a cohort of young South African adults, aged 20-30 years, as part of the Global burden of disease Lifestyle And mental Disorder (GLAD) project. This 5-year prospective cohort study was conducted in the North West Province of South Africa following the GLAD project protocol (DERR1-10.2196/65576). Dietary exposures were evaluated using three non-consecutive 24-h dietary recalls, measuring daily intake of food groups and nutrients defined by the GBD study. Depression outcomes were assessed at baseline ( = 1039) and follow-up ( = 551) using the Patient Health Questionnaire (cut-off ≥10). Logistic and Poisson regression analyses were performed, reporting odds ratios (ORs) and relative risks (RRs). Four models were tested: unadjusted, sociodemographic-adjusted, total energy (TE) intake-adjusted and fully adjusted (both sociodemographic and TE-adjusted). For longitudinal analyses, baseline depression cases were additionally excluded ( = 403). Participants (mean age 24.55 years; 51.4% female; 48.6% Black) had a 29.45% baseline depression prevalence. Higher milk intake was associated with a lower risk of incident depression (RR = 0.94, 95% CI 0.91-0.98) in the TE-adjusted model. Cross-sectionally, higher sugar-sweetened beverage consumption was associated with higher odds of depression, whereas higher calcium (OR = 0.48, 95% CI: 0.31, 0.76) and vegetable intake (OR = 0.74, 95% CI: 0.61, 0.91) were associated with lower odds after TE adjustment. Higher fibre intake was associated with lower odds in the unadjusted model. Higher milk intake was associated with lower incident depression, while higher calcium, vegetable and fibre intake were associated with lower depression prevalence, but not when adjusting for sociodemographic information. These findings suggest that sociodemographic context should be considered alongside dietary strategies to support prevention efforts for common mental disorders in young adults. 5. Perceived Disinformation, Anxiety, and Depressive Symptoms in the Czech Population: Age-Specific Associations.
PMID:日期:2026-09-29The main goal of the study was to examine the relationship between anxiety and depressive symptoms and the perception of disinformation in the Czech population. The research was performed on a sample of 2867 respondents. Analytical processing was carried out through quasi-Poisson regression analysis separately for the three age groups (aged 16-39, 40-64, and 65-90). The dependent variables were the GAD-7 (Generalised Anxiety Disorder) and PHQ-9 (Patient Health Questionnaire) total scores, measuring anxiety and depressive symptoms, respectively. The independent variables were represented by the subjective perception of exposure, severity of disinformation, and credibility of specific disinformation in the Czech Republic. The 11 narratives, verified by a fact-checking organisation, were examined, of which eight were disinformation claims and three were factual reports. The outcomes demonstrated that 69.3% of the population experienced no or minimal anxiety, 19.0% mild, 7.2% moderate, and 4.5% severe anxiety. In the case of depression, 43.0% of the population demonstrated minimal symptoms, 25.4% mild, 9.9% moderate, and 14.3% no symptoms. After Benjamini-Hochberg false-discovery-rate (FDR) adjustment, perceived exposure to disinformation remained positively associated with GAD-7 and PHQ-9 scores across all three age groups. Among the 66 narrative-specific tests, 20 of 33 GAD-7 associations and 17 of 33 PHQ-9 associations remained statistically significant after FDR correction. The retained associations varied by narrative and age group rather than indicating a uniform age gradient. Because the data are cross-sectional, these associations do not establish causal direction.

6. Longitudinal Trajectories of Grief Before and After Bereavement and Their Association With Mental Health Outcomes: A Pilot Prospective Cohort Study From the China Grief Cohort 2025.
PMID:日期:2026-09-28Prolonged grief disorder (PGD) impairs psychophysiological well-being and social functioning. Although post-death grief trajectories have been well-documented, pre-death grief trajectories remain understudied, particularly within the same cohort. This exploratory pilot study aimed to identify candidate longitudinal trajectories of grief before and after bereavement and to examine their associations with subsequent mental health outcomes, with the goal of generating hypotheses to inform a larger confirmatory cohort. This prospective cohort study was conducted at Peking Union Medical College Hospital. Participants were adult immediate family members of terminally ill patients with a life expectancy of 6 months or less enrolled between October and December 2023. Grief, depression, anxiety and stress were measured using the Anticipatory Grief Scale (AGS) and the PGD-13 Revised (PG-13-R), the 17-item Hamilton Rating Scale for Depression (HAMD-17), the Hamilton Anxiety Scale (HAMA) and the Impact of Event Scale-Revised (IES-R), at baseline, monthly before bereavement and quarterly for 6 months after bereavement, respectively. A total of 60 participants (mean [SD] age, 52.82 [10.18%] years; 32 [53.3%] women) were enrolled, most were offspring of terminally ill patients (44 [73.3%]). Two pre-death grief trajectories (Low, = 51 [85%]; and increasing, = 9 [15%]) and four post-death grief trajectories (high/decreasing, = 9 [15%]; moderate/decreasing, = 26 [43.33%]; low/decreasing, = 15 [25%]; and high/prolonged, = 10 [16.67%]) were identified. At the final visit, the incidences of PGD, depressive disorders, anxiety disorders and stress-related disorders were 16.7%, 16.7%, 30.0% and 0%, respectively. Participants in the Increasing pre-death grief trajectory and the persistently high/prolonged post-death grief trajectory had higher incidences of PGD, depressive disorders, and anxiety disorders compared to other groups. Individuals experiencing increasing pre-death grief or persistently high-level post-death grief are more likely to develop PGD and other mental disorders. Findings are exploratory and require replication in a larger, theoretically informed cohort. Chinese Clinical Trial Registry Identifier: ChiCTR2300071236.

7. The Moderating Role of Trait Anxiety in the Relationship Between Acute Aerobic Exercise and Cognitive Control in College Students.
PMID:日期:2026-09-28This study aimed to introduce trait anxiety as an individual-difference factor and further examine the complex relationship between acute aerobic exercise and cognitive control in college students. A mixed experimental design was adopted. Trait anxiety was assessed using the State-Trait Anxiety Inventory (STAI) and used to classify participants into high- and low-trait-anxiety groups. Cognitive control was measured with the AX continuous performance task (AX-CPT). The intervention conditions consisted of 20 min of seated rest and moderate-intensity acute aerobic exercise. A 2 (intervention: acute aerobic exercise vs. rest) × 2 (time: pretest vs. posttest) × 2 (trait anxiety: high vs. low) repeated-measures analysis of variance (ANOVA) was conducted. Only participants with high trait anxiety showed a significant reduction in posttest reaction time on BX trials, an index of proactive control, following acute aerobic exercise ( < 0.001), with no significant change in accuracy ( > 0.050). In addition, the proactive behavioral index (PBI) for error rate [PBI(error)] significantly increased in this group after exercise ( = 0.031). No significant changes were observed in '(Proactive), A-cue bias, PBI(RT), PBI(composite), or indices of reactive control under any intervention condition. Among participants with low trait anxiety, neither intervention produced significant changes in proactive control, as reflected by BX trials, or reactive control, as reflected by AY trials and '(Reactive) (all > 0.050). Trait anxiety exerted a significant influence on the relationship between acute aerobic exercise and cognitive control, with this effect being particularly evident in reaction time during proactive control. These findings suggest that future exercise interventions should incorporate individuals' baseline cognitive control characteristics to develop more precise and individualized exercise prescriptions.

8. Physical Activity Volume, Exercise Guidance, and Depressive Symptoms in Young-Old Adults: The Role of Internet Use Frequency and Self-Perceived Digital Proficiency.
PMID:日期:2026-09-28Physical activity is associated with depressive symptoms in later life, but this association may depend on the form of exercise-related support and older adults' digital engagement. This study examined whether physical activity volume (PAV) and exercise guidance (EG) were associated with depressive symptoms among young-old adults in China and whether these associations varied by internet use frequency (IUF) and self-perceived digital proficiency. Data were drawn from the 2020 to 2023 waves of the China Longitudinal Aging Social Survey (CLASS). The analytic sample included 5643 adults aged 60-75 years. Generalized estimating equations (GEE) were used to examine the associations of PAV and EG with depressive symptoms, as well as their interactions with IUF and self-perceived digital proficiency. PAV and EG were associated with lower depressive symptom scores in the full sample, although the association for PAV was less consistent across subgroup and moderation models. More frequent internet use was associated with lower depressive symptom scores and was more consistently related to the EG-depressive symptoms association than to the PAV-depressive symptoms association. The interaction between PAV and IUF was evident mainly among rural respondents. Among internet users, high self-perceived digital proficiency was associated with lower depressive symptom scores and showed a clearer conditional association with EG than with PAV. Digital engagement was more consistently related to the depressive-symptom relevance of EG than to PAV itself. These findings suggest that supporting young-old adults' mental health may require attention not only to physical activity levels but also to accessible EG and age-friendly digital support.

9. The Descriptive Epidemiology of Panic Attacks and Panic Disorder in Australia 2020-22: Prevalence, Correlates, Age of Onset and Changes Over Time.
PMID:日期:2026-09-28Panic disorder is highly debilitating and contributes a significant amount to anxiety-related disability-adjusted life years globally. To understand this burden, the current study provides an up-to-date understanding of the descriptive epidemiology of panic attacks and panic disorder in the Australian population. Data were from the most recent 2020-2022 National Survey of Mental Health and Wellbeing, a large-scale population-based study conducted in Australian households with respondents aged 16-85 years of age ( = 15,893). Panic attacks and panic disorder experienced across the lifetime, within the past 12 months and within the past 30-days were assessed by the Composite International Diagnostic Interview according to the Diagnostic and Statistical Manual of Mental Disorders (DSM-IV) diagnostic criteria to facilitate direct comparisons with the 2007 National Survey of Mental Health and Wellbeing ( = 8841). The lifetime prevalence of panic attacks and panic disorder was ~27% and 4%, respectively. Those with panic disorder indicate a median age of onset in their early 20s and the experience of panic disorder is not trivial, with nearly half of all cases (45%) falling within the 'severe' range in terms of severity and functional impairment. There were significantly elevated odds in select subgroups of the population with evidence that panic attacks and panic disorder remain a pressing concern for the Australian population, and particularly so in 2020-22 for younger cohorts (16-24), females, those who have never been married, and people who reside in inner regional areas. Despite improvements in access to mental health services, there remains a strong need for improved mental health services and targeted prevention programmes.

10. Pathways From Cyber Victimization to Adolescent Depression: A Latent Profile and Bridge Network Analysis.
PMID:日期:2026-09-26Cyber victimization is among the most consistent predictors of depression (DEP) in adolescence, yet research has largely treated both constructs as unidimensional totals, obscuring which specific victimization experiences activate which depressive symptoms and for whom. This study applied a person-centered and network psychometric approach to map these pathways in a cross-sectional community sample of 622 Turkish adolescents (55.0% female; = 13.90 years). Latent profile analysis (LPA) identified three distinct subgroups based on self-esteem (SE), family affluence, and social media (SM) use: a low SE/moderate SM group ( = 395, 63.5%), a high SE/high DEP group ( = 115, 18.5%), and a high SM use group ( = 112, 18.0%). Depressive symptom networks differed significantly in global strength across profiles; the high SM use group showed the densest intersymptom connections. A bipartite bridge network analysis revealed that self-harm/suicidal ideation was the strongest cross-community bridge node (bridge expected influence = 0.143), despite being the most peripheral symptom within the DEP network itself. This dissociation suggests that the symptom is linked mainly to cyber victimization rather than to other depressive symptoms. Embarrassing messages was the strongest victimization-side bridge node (EI = 0.095). These findings challenge the assumption that intranetwork centrality predicts intervention priority: Peripheral symptoms may function as critical entry points when external stressors drive activation.
