Inflammatory bowel disease (IBD), including Crohn's disease (CD) and ulcerative colitis (UC), is increasingly recognized as being associated with obstructive sleep apnea (OSA). Recent cohort studies and meta-analyses identify IBD as an independent risk factor for OSA, supporting the need for OSA screening in patients with IBD and management of modifiable risk factors. However, these associations come primarily from observational studies with virtually no confirmatory mechanistic studies. Further, the available clinical and experimental evidence of an association between OSA and IBD is highly heterogeneous and comes from multidisciplinary sources, suggesting that the association may reflect multiple shared biological mechanisms rather than simple disease coexistence. IBD-associated inflammation, epithelial barrier dysfunction, immune dysregulation, gut dysbiosis, and OSA-associated intermittent hypoxia, oxidative stress, immune activation, gut barrier dysfunction, and gut dysbiosis appear to be the key reciprocal processes establishing a self-reinforcing cycle that links intestinal and respiratory pathophysiology. This theoretical review synthesizes multidisciplinary heterogeneous evidence across clinical, experimental, and mechanistic studies to develop the hypothesis-generating Gut Barrier-Microbiota-Immune-Hypoxia (GBMIH) framework. This hypothesis-generating systems theory highlights intestinal barrier disruption, dysbiosis, immune activation, and intermittent hypoxia as interconnected biological nodes that may mediate this bidirectional relationship and define future biomarker and therapeutic development.

While the link between sleepiness and impaired driving performance is well-established, identifying high-risk sleepiness episodes is crucial for public safety. We assessed the predictive ability of sleepiness assessment tools in relation to impaired driving performance. A systematic review and meta-analysis searched four major databases, Medline (PubMed), Embase, Web of Science and the Cochrane Central Register of Controlled Trials (CENTRAL). Studies reporting associations between driving performance metrics (off-road events, Standard Deviation of Lateral Position, and reaction time) and at least one sleepiness measure (subjective: Epworth Sleepiness Score and Karolinska Sleepiness Scales [KSS]; objective: PERcentage of eye CLOsure [PERCLOS], Johns Drowsiness Scale, Karolinska Drowsiness Scale [KDS], EEG power alpha/theta ratio, and behavioural/physiological microsleep episodes) were included. Sixty-one studies were included (1441 participants, 353 observations). Both subjective (KSS [slope = 0.1, 95%CI 0.08; 0.12, p = 0.00]) and objective sleepiness measures (PERCLOS-80 [slope = 0.02, 95%CI 0.01; 0.04, p = 0.01]; KDS [slope = 0.00, 95%CI 0.00; 0.01, p = 0.02]; EEG power [slope = 0.01, 95%CI 0.07; 0.26, p = 0.00]; microsleep episodes [slope = 0.03, 95%CI 0.02; 0.03, p = 0.00]) were robustly associated with impaired driving performance. Certainty of evidence was low for KSS, PERCLOS-80, microsleep episodes, and ESS, and very low for KDS, EEG power, and JDS, owing to serious risk of publication bias and methodological limitations. These findings support associations between subjective and objective sleepiness measures and impaired driving performance but their predictive utility requires further investigation. Future research should assess combined tools under standardized real-world driving conditions in larger studies. PROSPERO: CRD42024427166.

Elevated ambient temperatures harm global sleep health, yet research into their effects on clinical sleep disorders is sparse and poorly synthesised. We review relevant literature and introduce the term "thermosomnia"-a portmanteau of thermo (heat/temperature) and somnia (sleep)-to characterise the multifaceted impact of high and rising ambient temperature on clinical sleep disorder outcomes. We address five key areas: (1) effects of elevated ambient temperatures on sleep disorder prevalence and symptom severity; (2) implications of thermosomnia for the safe and effective treatment of clinical sleep disorders; (3) potential mechanisms of thermosomnia; (4) social determinants of thermosomnia; and (5) the worsening of thermosomnia due to anthropogenic climate change. We highlight literature showing that elevated ambient temperatures are associated with exacerbated obstructive sleep apnoea and greater nighttime insomnia symptoms, and threaten treatment effectiveness via reduced adherence. Consequently, elevated temperatures likely compound existing risks associated with clinical sleep disorders. Select demographic groups are disproportionately susceptible to temperature-related sleep disorder effects, and climate change will likely exacerbate thermosomnia and increase global disparities in clinical sleep health. This review identifies an urgent need for research to inform equitable clinical and public health responses to rising ambient temperatures and to mitigate the growing burden of thermosomnia.

Non-rapid eye movement (NREM) parasomnias are sleep disorders characterized by abnormal, involuntary behaviours during partial arousals from slow-wave sleep. NREM parasomnias are common in childhood and usually remit, but a subset persist into or first emerge in adulthood, where they may cause significant morbidity. Evidence to guide treatment is limited, and no formal treatment guidelines exist. This review summarises evidence for behavioural, psychological, and pharmacological interventions. Ovid (Embase), the Cochrane Library, and PubMed were searched for English-language reports on the treatment of NREM parasomnias (confusional arousals, sleepwalking, sleep terrors, sexsomnia, sleep-related eating disorder) from January 2003 to 25 February 2026. The review followed PRISMA and was prospectively registered with PROSPERO (CRD420261327423). Risk of bias was assessed with RoB 2, ROBINS-I, and Joanna Briggs Institute tools, as appropriate; certainty of evidence was rated using GRADE. Heterogeneity precluded meta-analysis; findings were synthesized narratively. Fourteen studies were included, mostly retrospective and uncontrolled. Pharmacotherapy, particularly benzodiazepines, was the most studied modality; cognitive behavioural therapy was the leading non-drug intervention. Interventions also included melatonin, antidepressants, Z-drugs, topiramate for sleep-related eating disorder, serotonergic precursors in children, and hypnosis. Most interventions suggested benefit, but small samples, reliance on subjective outcomes, and the absence of comparators limited interpretation. Certainty was very low for most outcomes. High-quality trials with standardized and ideally objective measurements are needed to guide clinical practice.

Calcitonin gene-related peptide (CGRP)-targeted therapies have changed migraine treatment, but their effects on sleep remain incompletely understood. This review summarizes the emerging evidence on sleep outcomes with CGRP-targeted therapies, including monoclonal antibodies and gepants. Available studies suggest improvement in subjective sleep measures and few reported sleep-related adverse events, but the data are limited. Current findings do not establish whether sleep improvements reflect direct CGRP pathway modulation or improved migraine control. Interpretation is further complicated by patient heterogeneity, including comorbid sleep disorders, psychiatric symptoms, chronic pain, autonomic dysfunction, and concomitant medications. Future studies should use multidimensional sleep assessment and longitudinal objective measures to clarify whether CGRP-targeted therapies influence sleep physiology, sleep perception, or both.

This systematic review summarizes the scope, structure, and quality of the development process of generic sleep patient-reported outcome measures (PROMs). A literature search identified 24 articles reporting on the development of 27 generic sleep PROMs. Using the consensus-based standards for the selection of health measurement instruments (COSMIN) as guiding framework, the overall quality of the development was found doubtful for 4 PROMs and inadequate for 23 PROMs. For 20 PROMs no concept elicitation study and/or cognitive interview or other pilot study was reported. Common concerns were unclear construct definitions, unclear origin of the constructs, unclear target populations, and the observation that patients were not consulted in the development process. These concerns are interpreted in light of evolving standards and situated in ongoing scientific debates concerning: 1) increasing input from patients and the broader public, 2) multitude of guidelines, 3) lack of conceptual clarity, and 4) challenges in applying COSMIN criteria. While appreciating the substantial methodological progress achieved in recent decades, we advocate for retrospective improvements to existing generic sleep PROMs and encourage future developments to adhere to current best practices, thereby further strengthening sleep research and clinical care.

Delirium is a common neuropsychiatric syndrome among hospitalized patients and has been linked to disturbances in sleep and circadian regulation. We conducted a network meta-analysis of randomized controlled trials evaluating sleep-modulating pharmacological interventions for delirium prevention. PubMed, Embase, and Web of Science were searched through August 2025. Thirty-three trials involving 5045 patients were included. In the random-effects model, melatonin and melatonin receptor agonists were associated with a lower risk of delirium compared with non-active control conditions (RR = 0.77, 95% CI: 0.64-0.92). Evidence from suvorexant trials was also associated with a lower delirium risk (RR = 0.51, 95% CI: 0.27-0.95), whereas midazolam-based benzodiazepine evidence did not demonstrate a statistically significant reduction in delirium risk (RR = 1.64, 95% CI: 0.81-3.31). Subgroup analyses suggested that the association for melatonin-based interventions was more evident in non-ICU and surgical populations. The certainty of evidence ranged from low to moderate. Melatonin-based interventions and suvorexant were associated with lower delirium incidence; however, evidence directly comparing different active interventions remains limited, and findings should be interpreted cautiously given the agent-specific evidence base. Further direct comparative trials are needed to clarify the effectiveness and safety of sleep-modulating pharmacological interventions for delirium prevention.

Obstructive sleep apnoea (OSA) diagnosis and severity classification is typically determined from a single-night sleep study. However, high night-to-night variability (N2NV) in OSA severity is associated with adverse health outcomes and complicates diagnosis and treatment. While the contributions of the four main OSA endotypes (i.e., anatomical compromise, unstable ventilatory control, low arousal threshold, and poor upper-airway muscle function) on OSA severity are relatively well established, the extent to which specific mechanistic contributors to N2NV in OSA severity is poorly understood. This narrative review collates evidence-based and theoretically plausible contributors to high N2NV. We consider a plethora of physiological, behavioural, and environmental factors which are known to, or hypothetically contribute to variable OSA severity. How a single-night evaluation may fail to capture key mechanistic contributors to OSA and its variability is also highlighted. Understanding the complexity and interactive nature of these mechanisms may help inform diagnosis, risk stratification, and treatment selection for OSA via a more holistic, precision-medicine approach. Studies that include multi-night in-laboratory and at-home assessments combined with wearable/nearable sleep sensors are required to better understand the underlying mechanisms that contribute to high N2NV in OSA severity and its associated adverse consequences.

Sleep disorders are increasingly understood as systems-level conditions in which the brain and body reweight sensory evidence from the external world and internal milieu. We systematically reviewed human evidence for sensory and multisensory dysfunction across sleep disorders and experimental sleep loss. Searches identified 25,777 records; after removal of 7235 duplicates, 18,542 records were screened, 4452 full records were assessed for eligibility, and 377 studies met inclusion criteria. Evidence was organised across 31 sensory modalities in seven domains: olfaction, gustation, vision, audition, somatosensation/nociception, vestibular-proprioceptive function, and interoception/autonomic function, with higher-order perceptual outcomes analysed separately. Using a prespecified conservative criterion requiring a non-null direction of effect with at least moderate certainty, 23 of 31 modalities were altered in at least one sleep disorder or experimental sleep-loss category (74.2%). The evidence clustered in specific disorder-domain pairs. Moderate-certainty signals identified clinically meaningful hyposmia in isolated rapid eye movement sleep behaviour disorder and obstructive sleep apnoea, heightened pain sensitivity in chronic insomnia, altered chronoception after experimental sleep loss, and obstructive sleep apnoea-associated hearing-threshold impairment. Central auditory-processing, visual, gustatory, and vestibular-proprioceptive findings were more heterogeneous or lower certainty. Across disorders, the emerging map is consistent with state-dependent disturbances in sensory gain, gating, salience, and interoceptive control. These findings position sensory profiling as a clinically tractable strategy for phenotyping and for prospective evaluation of risk and treatment response in selected sleep disorders. Longitudinal and interventional studies should now test whether sensory measures predict disease course, sleep stability, and clinical outcomes, and whether targeted sensory or interoceptive modulation can improve sleep-related symptoms.
