1. Impact of an Educational Intervention on Job Satisfaction Among Home Health Aides: Findings from a Mixed-Methods Analysis of a Pilot Randomized Controlled Trial.
PMID:日期:2026-04-21Home health aides (HHAs) provide critical support for patients with chronic conditions, such as heart failure (HF), by assisting with activities of daily living, symptom monitoring, and medication management. Despite their essential role, HHAs are often undervalued and deal with challenging work conditions which contribute to high turnover rates. In contradiction, job satisfaction has consistently been reported as high, suggesting a disconnect that warrants investigation. While prior research identified factors contributing to job satisfaction and retention, few have evaluated interventions and their subsequent effects on employment outcomes. This study assessed the impact of an educational intervention on job satisfaction and turnover intention among HHAs caring for patients with HF. We conducted a mixed-method analysis of a pilot randomized clinical trial involving 102 HHAs who received virtual HF training. Job satisfaction and turnover intention were measured at baseline and 90-day follow-up using validated scales. Semi-structured interviews were conducted with 66 HHAs. Baseline job satisfaction was high (77-86%), with modest improvements observed at follow-up. Turnover intention decreased from 21% to 14%. Qualitative findings revealed HHAs valued the training, reported enhanced motivation for learning, increased self-efficacy, and perceived improvements in patient care. Educational interventions addressing intrinsic motivators may increase job satisfaction and reduce turnover intention among HHAs, suggesting that investing in professional development could strengthen workforce retention in homecare.

2. Beyond Keywords: A Qualitative Study of Context-Dependent Stigmatizing Language in Clinical Documentation from a Large, Multi-Payer Home Healthcare Agency.
PMID:日期:2025-09-22Stigmatizing language in clinical documentation is increasingly recognized as a contributor to healthcare disparities, with evidence that such language disproportionately affects marginalized populations. This study aimed to explore home healthcare (HHC) clinicians' perceptions of stigmatizing language in documentation and to identify strategies for reducing bias. We conducted semi-structured interviews with seven nurses between may and October 2024 from a large nonprofit HHC agency in New York City, which serves patients across diverse payer types, such as Medicare, Medicaid, and private insurance. We used maximum variation sampling to ensure diversity in race, experience, and geography. Interviews were transcribed and analyzed using a thematic analysis approach to identify clinicians' views on stigmatizing terms, contextual influences, and strategies for bias reduction. Five themes emerged. First, clinicians demonstrated varied interpretations of potentially stigmatizing language, emphasizing the role of context in how terms are perceived. Second, clinical and system-level factors, such as complex patient needs, time pressures, and regulatory demands, shape documentation practices. Third, stigmatizing language can influence provider attitudes, erode patient trust, and hinder patient-clinician care collaboration. Fourth, clinicians identified both individual and organizational strategies to reduce bias, including reflective documentation practices and formal education and training to increase language awareness. Fifth, participants highlighted the potential role of technology in reducing biased documentation. This study highlights the complexity of identifying stigmatizing language in HHC and the importance of contextual understanding. Findings support the development of clinician-informed, context-sensitive NLP tools and training programs to promote equitable, patient-centered documentation practices.

3. The Impact of the COVID-19 Pandemic on Homecare Use for Individuals with Physical Disabilities Stratified by Sex, Age, and Mental Health Condition: A Cohort Study Using Administrative Health Data.
[英文摘要] This study investigated the impact of the COVID-19 pandemic on homecare service use among individuals with physical disabilities, stratified by age, sex, and mental health conditions. Monthly utilization of personal support and nursing services was assessed using linked health administrative databases from ICES in Ontario, Canada, over two periods: pre-pandemic (March 2015 to February 2020) and during the pandemic (March 2020 to June 2022). Predictive Autoregressive Integrated Moving Average (ARIMA) models were used to estimate changes in service use. During the pandemic, personal support service use declined significantly across multiple subgroups with some groups experiencing greater impacts. Significant decreases were observed in 78.5% of months for males, 14.3% for females, 78.5% for individuals aged 65 years and younger, 17.9% for those older than 65 years, and 78.5% for individuals with mental health conditions. In contrast, nursing service use increased significantly, with significant increases observed in 85.7% of months for males, 60.7% for females, 60.7% for those aged 65 years and younger, 17.9% for those older than 65 years, 85.7% for individuals with mental health conditions, and 28.6% for those without mental health conditions. The findings highlight substantial variation in the pandemic's impact across subpopulations, with certain groups disproportionately affected. Targeted strategies are needed to mitigate these disparities and ensure equitable access to homecare services. Further research is warranted to explore the long-term implications and the underlying factors contributing to these differences.

4. Using Interpersonal Continuity of Care in Home Health Physical Therapy to Reduce Hospital Readmissions.
PMID:日期:2024-07-28This paper is an examination of the relationship between continuity of care with home health physical therapists following hospitalization and the likelihood of readmission. We conducted a retrospective cohort study. Using rehospitalization as the dependent variable, a continuity of care indicator variable was analyzed with a multivariable logistic regression. The indicator variable was created using the Bice-Boxerman Index to measure physical therapist continuity of care. The mean of the index (0.81) was used to separate between high continuity (0.81 or greater) of care and low continuity of care (lower than 0.81). The sample included 90,220 patients, with data coming from the linking of the Outcome Assessment and Information Set (OASIS) and an administrative dataset. All subjects lived in the NYC metro area. Inclusion criteria was a patient's admission to their first home health care site following discharge occurring between 2010 and 2015, and individuals who identified as Male or Female. In comparison to low continuity of physical therapy, high continuity of physical therapy significantly decreased hospital readmissions (OR = 0.74, 95% CI 0.71-0.76, ≤ .001, AME = -4.28%). Interpersonal continuity of physical therapy care has been identified as a key factor in decreasing readmissions from the home care setting. The research suggests an increased emphasis in preserving physical therapist continuity following hospitalization should be explored, with the potential to reduce hospital readmissions.

5. The Paradox of Choice in Palliative Care Decision-Making in Managed Long-Term Care: A Qualitative Study.
PMID:日期:2024-04-15Palliative care offers undeniable advantages to elderly patients with multiple chronic illnesses in managed long-term care. However, only approximately 14% of those who require palliative care actually receive it. To investigate factors influencing decision-making regarding palliative care acceptance, and healthcare providers' communication strategies, which patients or family caregivers perceive during managed long-term care. Qualitative thematic content analysis study. Two data sources from home and community-based health care organization in a metropolitical city in the Northeastern United States were used: (a) retrospectively collected 79 phone call audio recordings between patients and/or family caregivers with healthcare providers when initiate palliative care option, and (b) prospectively collected 10 exploratory qualitative interviews with patients and/or family caregivers for this study purpose. From a total of 89 conversations, 7 themes emerged during palliative care decision-making: (a) capability for self-management; (b) symptom severity; (c) perceptions of chronic disease; (d) satisfaction with current health services; (e) code status; (f) caregiver burden and (g) other reasons, including financial considerations and service affiliations. In addition, from 10 qualitative interviews, 4 key themes in healthcare providers' communication that optimize palliative care conversations were indicated: (i) trust and relationship dynamics; (ii) positive reinforcement and personal connection; (iii) conversation setting and initiator; and (iv) collaborative approach to care coordination. Enhancing decision-making around palliative care and improving acceptance of palliative care services may be improved through the integration of personalized care plans, empathetic communication, use of symptom-inclusive assessments for timely care, and proactively addressing caregiver burden.

6. A Leave of Absence Might Not Be a Bad Thing: Registered Practical Nurses Working in Home Care During the COVID-19 Pandemic.
PMID:日期:2024-03-16To describe the resilience and emotional intelligence of Registered Practical Nurses working in Home and Community Care during the COVID-19 pandemic. Specifically, to determine if there was a relationship between resilience and emotional intelligence based on whether a nurse: (1) left the sector, (2) considered leaving, or (3) took a leave of absence during the pandemic. An online cross-sectional survey was used to capture respondents' demographic information and scores on the Connor-Davidson Resilience Scale, Resilience at Work Scale, and Wong and Law Emotional Intelligence Scale. Registered Practical Nurses working, or who had worked, in Home and Community Care January 2020 to September 2022 were eligible to participate. The Checklist for Reporting Results of Internet E-Surveys was used. The survey was available June to September 2022 and advertised by the Registered Practical Nurses Association of Ontario to approximately 2105 members. Descriptive statistics and independent samples -tests were used to analyze results at a level of < .05 was used for all analyses. A total of 672 respondents participated (completion rate = 92.8%). There were no differences on resilience or emotional intelligence scores based on whether a nurse left, or considered leaving, the Home and Community Care sector during the pandemic. However, nurses who took a leave of absence scored significantly higher on resilience and emotional intelligence measures when compared to those who did not. Results suggest that a leave of absence for these nurses during the pandemic may have been a supportive coping strategy.

7. The effects of occupational and mental stress among home care rehabilitation professionals working during the COVID-19 pandemic: An exploratory qualitative study.
PMID:日期:2024-01-30Home care rehabilitation professionals (hcRPs) provide health services for clients with a broad range of medical conditions. During the COVID-19 pandemic, home care rehabilitation professionals experienced exacerbations of pre-existing work-related stressors, increased risk of transmission of the COVID-19 virus, reduced resource availability, greater workloads, and staffing shortages. The primary aim of this study was to examine the experience and impact of occupational and mental stress on hcRPs working during the COVID-19 pandemic. Semi-structured interviews were conducted with 24 hcRPs working in Ontario, Canada during the COVID-19 pandemic. Inductive thematic analysis was used to interpret and organize the data into conceptualized themes. Interview data was organized into three themes: (a) unique challenges of a home care rehabilitation professional, (b) COVID-19 exacerbations of home care occupational and mental stress, and (c) personal and workplace coping strategies. Many participants reported reducing their hours or taking on adjunctive roles in different clinical settings outside of home care due to work-related stress exacerbated by the COVID-19 pandemic. With a focus on the effects of COVID-19 on the practice of home care, this study provides a unique perspective on the challenges experienced by hcRPs during an emergent and evolving global public health concern. The exploratory nature of this research works towards providing a framework of factors to be addressed when creating sustainable healthcare interventions, as well as recommendations to support hcRPs to benefit both the community and health-care providers.

PMID:日期:2023-03-13Infant's health care management at home was an important challenge for the families. Therefore, the policymakers provided a program as an extension to the care plan to implement at home and examine families' attitudes toward the program. Examination of the parents' attitude was a coincidence with COVID-19 pandemic in this study, a descriptive-analytical cross-sectional study that was conducted on 385 parents with infants less than 2 months old in the health centers of Isfahan-Iran. The samples were selected by stratified random sampling method. Data gathering was done through a researcher-made Likert questionnaire, which had 30 items and 3 domains including individual-cultural, educational-supportive, and environmental domains. The data were analyzed through the descriptive and analytical statistic. The results of the study revealed a mean score of 104.12 ± 11.69 on family attitude and their positive view toward the infant home-based care program. Also, the highest score was related to the individual-cultural domain 36.95 ± 4.44, educational-supportive 34.88 ± 5.04 and environmental domains 32.29 ± 3.98, respectively. In addition, a significant relationship was found between the mean score of the family attitude and age, the number of children, education, and place of care ( < .05). Based on the results of the study, the attitude of family toward infant home-based care is positive, and the challenges of parents in caring their infants at home can be reduced as the program is implemented.

9. Efficacy, feasibility, and acceptability of in-home stroke education for stroke survivors.
PMID:日期:2023-01-12Barriers to learning after a stroke may prevent stroke survivors from acquiring helpful information regarding stroke prevention and preparedness. The objective of this study was to evaluate the efficacy, feasibility, and acceptability of a novel in-home stroke education program for survivors in the acute phase following a stroke. Study participants completed four in-home education sessions about stroke prevention and preparedness following their discharge home from inpatient rehabilitation. Sessions were designed to be completed within an 8-week period. Sessions were presented with evidence-based teaching methods and could be tailored to individual needs. Participants completed short quizzes before and after each education session to measure knowledge attainment. Forty-nine participants were included in this study. On average, the program was completed in 10 weeks, or 69.5 days (SD 29.6), and visits lasted 66.26 minutes; 81.5% of participants completed Visit 1, 77.5% completed Visit 2, and 73.5% completed Visits 3 and 4. Statistically significant changes from pretest-to-posttest scores were found for all races and genders and for ages 50-79. There was no significant change in pretest-to-posttest scores for participants over age 80 (n = 3). Results show that delivering a stroke education program can be accomplished, on an expanded timeline. The program was effective in increasing stroke knowledge for participants recently discharged from inpatient rehabilitation following a stroke.

10. Health Information Technology Adoption at U.S. Home Health Care Agencies: Results from a Multi-Methods Study.
PMID:日期:2022-12-24Health information technology (HIT) holds potential to transform Home Health Care (HHC), yet, little is known about its adoption in this setting. In the context of infection prevention and control, we aimed to: (1) describe challenges associated with the adoption of HIT, for example, electronic health records (EHR) and telehealth and (2) examine HHC agency characteristics associated with HIT adoption. We conducted in-depth interviews with 41 staff from 13 U.S. HHC agencies (May-October 2018), then surveyed a stratified random sample of 1506 agencies (November 2018-December 2019), of which 35.6% participated (N = 536 HHC agencies). We applied analytic weights, generating nationally-representative estimates, and computed descriptive statistics, bivariate and multivariable analyses. Four themes were identified: (1) Reflections on providing HHC without EHR; (2) Benefits of EHR; (3) Benefits of other HIT; (4) Challenges with HIT and EHR. Overall, 10% of the agencies did not have an EHR; an additional 2% were in the process of acquiring one. Sixteen percent offered telehealth, and another 4% were in the process of acquiring telehealth services. In multivariable analysis, EHR use varied significantly by geographic location and ownership, and telehealth use varied by geographic location, ownership, and size. Although HIT use has increased, our results indicate that many HHC agencies still lack the HIT needed to implement technological solutions to improve workflow and quality of care. Future research should examine the impact of HIT on patient outcomes and the impact of the COVID-19 pandemic on HIT use in HHC.
