1. A meta-analysis of transjugular intrahepatic portosystemic shunt with or without embolization for esophageal and gastric variceal bleeding in decompensated liver cirrhosis.
PMID:日期:2026-09-30This study aimed to evaluate whether transjugular intrahepatic portosystemic shunt (TIPS) combined with embolization could further improve efficacy for patients with decompensated liver cirrhosis complicated with esophageal and gastric variceal bleeding. We searched PubMed, Web of Science, Embase, and Cochrane Library, for all relevant randomized controlled trials and cohort studies up to March 2025. Statistical results were evaluated using risk ratios and 95% confidence intervals (CIs). A total of 14 studies involving 3079 patients were included. The statistical results showed that the rebleeding rate in the TIPS combined with embolization group was lower than that in the TIPS alone group (risk ratios = 0.67; 95% CI = 0.57-0.79; P < 0.001), and the same result was observed only in the subgroup of patients with gastric variceal bleeding (risk ratios = 0.50; 95% CI = 0.34-0.74; P < 0.001). There was no significant difference between the groups in patients with isolated esophageal variceal bleeding (risk ratios = 0.83; 95% CI = 0.51-1.35; P = 0.46). In addition, the incidence of hepatic encephalopathy in the TIPS combined with embolization group was also lower than that in the TIPS alone group (risk ratios = 0.82; 95% CI = 0.73-0.91; P < 0.001), but there was no significant difference in survival rate (risk ratios = 1.00; 95% CI = 0.97-1.03; P = 0.94). Compared with TIPS alone, the combined treatment showed a trend toward lower rebleeding and hepatic encephalopathy, yet survival appeared similar between the two groups.

2. Percutaneous catheter drainage versus needle aspiration for liver abscess: a systematic review and meta-analysis of efficacy and safety.
PMID:日期:2026-09-30Percutaneous catheter drainage (PCD) and percutaneous needle aspiration (PNA) are the two primary image-guided interventions for liver abscesses. However, their comparative efficacy and safety profiles remain debated. We systematically searched PubMed, Embase, Web of Science, and the Cochrane Library for randomized controlled trials (RCTs) directly comparing PCD and PNA in adults with liver abscesses. The primary outcomes were clinical treatment success rate and procedure-related complication rate. Data were pooled using random-effects models, and heterogeneity was assessed with the I2 statistic. Twelve RCTs involving 1425 patients were included. Meta-analysis showed that PCD was associated with a significantly higher treatment success rate compared to PNA [odds ratio (OR) = 4.12, 95% confidence interval (CI) = 2.01-8.45; P < 0.001; I2 = 56%]. Subgroup analyses by abscess size and etiology confirmed the consistency of this benefit. There was no statistically significant difference in the overall complication rates between the two procedures (OR = 1.02, 95% CI = 0.30-3.50; P = 0.97; I2 = 46%). For the treatment of liver abscesses, PCD is superior to needle aspiration in achieving clinical success without increasing the risk of procedure-related complications. PCD should be considered the preferred percutaneous drainage technique.

3. Allostatic load, unhealthy behaviors, and mortality risk among participants with and without metabolic dysfunction-associated steatotic liver disease.
PMID:日期:2026-09-23Chronic stress and unhealthy behaviors are associated with metabolic dysfunction-associated steatotic liver disease (MASLD). However, whether and to what extent they are associated with mortality across different stages of MASLD remains unclear. This study aimed to examine the associations of allostatic load and unhealthy behaviors with mortality according to MASLD stage. The study included 9761 participants from the National Health and Nutrition Examination Survey (2005-2018). MASLD was defined as hepatic steatosis with at least one cardiometabolic criterion. Allostatic load, an indicator of chronic stress, was assessed using 10 biomarkers. Health behaviors included diet, nicotine exposure, physical activity, and sleep duration. Cox proportional hazards models estimated the associations of allostatic load and health behavior score (HBS) with mortality. Among 9761 participants, 3158 (32.4%) were classified as having MASLD. During a median follow-up of 7.42 years, 922 participants died, including 268 cardiovascular deaths. Allostatic load and HBS were associated with all-cause and cardiovascular mortality among participants with and without MASLD. The combination of high allostatic load and poor HBS was associated with higher risks of all-cause and cardiovascular mortality, particularly among participants with MASLD [hazard ratio = 3.33, 95% confidence interval (CI) = 1.97-5.65 for all-cause mortality; hazard ratio = 3.66, 95% CI = 1.31-10.23 for cardiovascular mortality]. Higher allostatic load and poorer health behaviors were independently and jointly associated with higher risks of all-cause and cardiovascular mortality among participants with and without MASLD. These findings highlight the importance of chronic stress and poor health behaviors in MASLD management and prognosis.

4. Quality controlling in capsule gastroduodenoscopy with less annotation via self-supervised learning.
PMID:日期:2026-09-22It is possible to control the quality of capsule endoscopic images using artificial intelligence, but it requires a great deal of time for labeling. Verifying the performance of self-supervised learning for quality control for capsule endoscopic images. A simple framework for contrastive learning of visual representations (SimCLR), is capable of acquiring the inherent image representation with minimal annotation, but the feasibility is not studied. A total of 62 840 images were collected to train models in internal cross-validation (more training data and less testing data) and reversed cross-validation (less training data and more testing data). Random forest and eXtreme Gradient Boosting (XGBoost) were used to complete the quality control after SimCLR extracted the features from images. Random forest and XGBoost reported that the mean area under the receiver operating characteristic (AUROC) curve exceeded 0.98 and 0.97 using SimCLR-derived features. Moreover, XGBoost surpassed supervised convolutional neural network (CNN). Extra 12 032 images were gathered for prospective validation and the AUROC of SimCLR surpassed 0.93 (95% confidence interval = 0.9271-0.9548), which is close to supervised CNN (0.9645) in cross-validation. Moreover, the AUROC of random forest and XGBoost (trained with SimCLR-derived features) surpasses 0.96, which is better than supervised CNN (0.8374) in reversed cross validation. Our results demonstrate that the SimCLR framework enables high-performance quality control in capsule endoscopy, achieving accuracy comparable to - or even surpassing - supervised learning methods, while requiring substantially fewer annotated images.

5. Underwater versus conventional polypectomy for colorectal polyps 10 mm or smaller: a systematic review and meta-analysis of randomized controlled trials.
PMID:日期:2026-09-22Underwater polypectomy has emerged as an alternative to conventional polypectomy for the removal of colorectal polyps. We conducted a systematic review and meta-analysis of randomized controlled trials (RCTs) to evaluate the comparative efficacy and safety of underwater polypectomy and conventional polypectomy. We systematically searched PubMed, Embase, Cochrane Library, and Web of Science for RCTs comparing underwater polypectomy and conventional polypectomy for colorectal polyps. We restricted the analysis to polyps 10 mm or smaller. The primary outcome was en bloc resection rate. Secondary outcomes included incomplete resection, piecemeal resection, R0 resection, intraprocedural bleeding, and resection time. Risk ratios with 95% confidence intervals (CI) were calculated for binary outcomes, and standardized mean differences for continuous outcomes using random-effects models. Four RCTs contributed 1166 polyps 10 mm or smaller (598 underwater polypectomy, 568 conventional polypectomy). Underwater polypectomy and conventional polypectomy showed comparable en-bloc resection rates (risk ratio: 1.01, 95% CI: 0.97-1.05; I2 = 68.9%). Underwater polypectomy was associated with significantly lower incomplete resection rates (risk ratio: 0.38, 95% CI: 0.25-0.56; I2 = 0%). R0 resection favored underwater polypectomy (risk ratio: 1.16, 95% CI: 1.00-1.34; I2 = 85.8), although substantial heterogeneity limits interpretation. Intraprocedural bleeding and resection time did not differ between techniques. Underwater polypectomy demonstrates a significant advantage over conventional polypectomy in reducing incomplete resection rates while maintaining comparable en bloc resection rates and safety profiles. These findings support underwater polypectomy as a reasonable and, in specific settings, preferable alternative to conventional cold snare polypectomy.

6. Rapid ammonia normalization and long-term efficacy of rifaximin in patients with cirrhosis: a multicenter retrospective study.
PMID:日期:2026-09-17Hyperammonemia drives the pathogenesis of hepatic encephalopathy and independently predicts poor outcomes in cirrhosis. Despite the widespread use of rifaximin, data on ammonia normalization kinetics and long-term effects on hepatic functional reserve remain limited. We aimed to evaluate the efficacy and safety of long-term rifaximin therapy for hyperammonemia in patients with cirrhosis. This multicenter retrospective study analyzed 247 cirrhotic patients with hyperammonemia treated with rifaximin at six institutions in Japan (2016-2022). Ammonia normalization kinetics and predictors, hepatic functional reserve, and safety were assessed over a median follow-up of 377 days. Blood ammonia concentrations (BACs) decreased by 34.7% at 1 week, and 74.5% of patients achieved normalization at a median of 63 days. Lower serum albumin was independently associated with delayed normalization. Serum albumin, prothrombin time, and the Child-Pugh score showed small but statistically significant improvements at 12 months, whereas the MELD and ALBI scores remained stable. Hepatic encephalopathy-related hospitalizations decreased by 60.2% compared with the preceding 2 years. Adverse events occurred in 2.0% of patients, all grade 1. Rifaximin rapidly normalized BACs, and this effect was sustained during long-term therapy. Long-term rifaximin administration was associated with a mild improvement in some indicators of hepatic functional reserve, suggesting that it may contribute to the maintenance or improvement of hepatic functional reserve. Lower baseline albumin is associated with delayed normalization, highlighting the importance of hepatic functional reserve in treatment response.

7. The effect of tenofovir alafenamide fumarate on lipid profiles in pregnant women with hepatitis B virus.
PMID:日期:2026-09-09Tenofovir alafenamide fumarate (TAF) and tenofovir disoproxil fumarate (TDF) are usually used in pregnant women with hepatitis B virus (HBV), yet TAF has been associated with unfavorable lipid profiles in other HBV-infected groups. Data regarding the effects on the lipid profiles of pregnant women are scarce. This study compared the effects of TAF and TDF on lipid profiles in this population. This retrospective study included 650 HBV-infected pregnant women, grouped into TAF and TDF cohorts based on the antiviral regimen. Generalized linear models were used to compare the effects of TAF and TDF on lipid levels, and causal mediation analysis was performed to assess whether the treatment difference in lipids was mediated by gestational weight gain. At delivery, the geometric mean ratios for total cholesterol (10.8%), low-density lipoprotein cholesterol (14.2%), and high-density lipoprotein cholesterol (10.0%) were elevated in the TAF group compared to the TDF group (all P < 0.001). An upward trend was also observed for triglycerides (6.3%; P = 0.062). The causal mediation analysis indicated that gestational weight change was not a significant mediator, accounting for less than 3.3%. Maternal and infant safety profiles were similar for most outcomes. TAF is effective in chronic HBV-infected pregnant women, but its lipid-elevating effect relative to TDF warrants clinical attention.

8. Clinical trends in hepatocellular carcinoma from 2005 to 2019 with a focus on metabolic diseases: a German nationwide study.
PMID:日期:2026-09-09Hepatocellular carcinoma (HCC) is the leading primary liver cancer and is associated with a high mortality rate. Liver cirrhosis represents the main risk factor for HCC; however, its underlying etiologies have shifted over the last decade. This German population-based study investigated the effects of the changing trends on etiologies of HCC hospital admissions. We analyzed 369 369 HCC-admissions in German hospitals from 2005 to 2019 based on the diagnosis-related groups via German International Classification of Diseases and Operation and Procedure key System codes. Temporal trends and associations were assessed using linear and multivariate logistic regression. HCC admissions increased significantly by 21.8%, from 19 594 in 2005 to 25 899 in 2019, while the in-hospital mortality rate decreased from 13.6 to 10.8%. The most common underlying etiology was alcohol-related liver disease (ALD), accounting for 55.5% of all HCC etiologies, followed by viral hepatitis (32.3%). While admissions of patients with ALD increased by 34.5% from 4008 to 5392, viral hepatitis declined significantly from 3532 to 1580. Metabolic dysfunction-associated steatotic liver disease (MASLD) underlying for HCC represented 4.5%, but exhibited the strongest increase of 3.7-fold among all etiologies. The in-hospital mortality rate of ALD-associated HCC decreased from 15.3 to 11.6%, whereas in viral hepatitis-associated HCC, it remained more similar (11.5 vs. 11.0%). Cardiometabolic risk factors significantly increased in HCC admissions over time ( P < 0.001). Furthermore, HCC admissions exhibited a greater burden of metabolic comorbidities than other malignancies. This study highlights the increase in underlying ALD, MASLD, and metabolic risk factors in HCC admissions. Especially, cardiometabolic risk factors appear to play a significant role as drivers of hospital admissions in patients with HCC.

9. Significance of serum osmolarity in patients with acute pancreatitis.
PMID:日期:2026-09-09Approximately 15-25% of cases of acute pancreatitis follow a severe clinical course. Early identification of severe acute pancreatitis is crucial for predicting potential morbidity and mortality. This study aimed to evaluate the association between serum osmolarity and in-hospital mortality in acute pancreatitis patients. Patients hospitalized with a diagnosis of acute pancreatitis between 1 January 2015 and 1 August 2019 in the Gastroenterology Service and Internal Medicine ICU in a tertiary center were analyzed retrospectively. A total of 311 patients were enrolled in the study. Demographic, clinical, laboratory, and radiological data, including calculated serum osmolarity values and their components, were analyzed based on acute pancreatitis morphological type, disease severity, in-hospital mortality, and ICU admission. Admission serum osmolarity was significantly higher in patients with severe acute pancreatitis and those who required intensive care (P < 0.001 and P = 0.003, respectively). Admission osmolarity showed positive correlations with the Ranson score, length of stay, and high-sensitive C-reactive protein. The area under the curve (AUC) for serum osmolarity at 24 h was [AUC: 0.822, 95% confidence interval (CI): 0.728-0.921, P = 0.001], with a cut-off value of 295.69 mOsm/L (sensitivity: 60% and specificity: 90.4%). The AUC for serum osmolarity at 48 h (OSM48) was (AUC: 0.932, 95% CI: 0.815-0.976, P < 0.001), with a cut-off value of 296.67 mOsm/L (sensitivity: 70% and specificity: 94%). Each 1 mOsm/L increase in (OSM48) was associated with a 15.1% increase in the odds of in-hospital mortality. Elevated serum osmolarity, measured at 48 h after admission, is a valuable and easily obtainable parameter and is related to in-hospital mortality in patients with acute pancreatitis.

10. Early but not serial ustekinumab concentration measurements are associated with clinical outcomes in Crohn's disease and ulcerative colitis: the Stockholm Ustekinumab study (STOCUSTE).
PMID:日期:2026-09-01Ustekinumab (UST), an anti-interleukin-12/23 antibody, is used to treat moderate-to-severe inflammatory bowel disease (IBD). The STOCUSTE study includes IBD patients treated with UST at four teaching hospitals in Stockholm to provide long-term follow-up data. The utility of therapeutic drug monitoring to optimize treatment and correlate the serum concentration of UST with clinical outcomes was investigated in a real-world setting. This retrospective study includes patients diagnosed with Crohn's disease and ulcerative colitis treated with UST and followed until withdrawal of treatment for any reason or until the end of the study. Concentration data and dosing interval were collected at each follow-up (3, 6, 9, 12, 24, 36, and 48 months) in relation to the presence of remission defined as Physician Global Assessment = 0. A total of 418 patients were included, 322 with Crohn's disease (48% female) and 96 with ulcerative colitis (46% female). UST concentrations were obtained in 229 (55%) patients, while 189 had no measurements. The mean serum concentrations for all were above 1 μg/ml. At 3 months, patients in remission had significantly higher concentrations than nonremitters (P < 0.001). No significant differences were observed at 6 months or later. Within 12 months of concentration result, approximately one-third of patients underwent treatment adjustments, including interval shortening, intravenous reinduction, or discontinuation/switch. In this study, an early (≤3 months) UST concentration was associated with remission, whereas concentrations measured at later time points did not differ between remission and nonremission. In this real-world cohort serial monitoring beyond 3 months showed poor correlation with clinical outcomes.
