1. Fifty years of elder abuse research: what have we learned about risk, assessment, prevention, and treatment?
PMID:日期:2026-10-03Elder abuse has been a focus of research and policy for over half a century. This article discusses this history, Focusing on elder abuse in the community, we trace seminal studies and innovative research and policy efforts that have sought to address key elder abuse questions, including how prevalent elder abuse is, why it occurs, risk and protective factors, and what is known about how it can be prevented/addressed. Although reviews indicate that effective interventions are lacking, we discuss several promising approaches at the program and organizational level. We recommend building on approaches from child maltreatment and intimate partner violence, including applying risk assessments that offer important lessons for elder abuse. We conclude by recommending refinement of assessments to identify different types of risks, along with interventions that recognize different etiologies and treatment approaches.

2. Elder abuse education: past, present, and future.
PMID:日期:2026-09-27Elder abuse awareness, intervention, and prevention efforts have inevitably included calls for increased education for health care, legal, protective services, and financial services professionals and pre-professionals, in part due to low levels of awareness across disciplines. Education is needed to increase capabilities and capacities to detect and effectively respond to abuse, increase confidence among professionals, improve identification and substantiation, meet legal mandates, and prevent and reduce harm. This article will highlight key historical events in elder abuse education, how these disciplines have addressed professional and pre-professional education, modalities of training, and current and future challenges facing elder abuse education.

3. Development and psychometric evaluation of the nurses' elder abuse assessment scale.
PMID:日期:2026-09-15This study aimed to develop and evaluate the psychometric properties of a nurse-administered instrument for the structured assessment of indicators potentially associated with elder abuse. Scale items were generated through literature review and expert evaluation, yielding a content validity index of 0.878. Each nurse assessed one unique older adult. Exploratory factor analysis included 373 nurse-older adult dyads, and confirmatory factor analysis included 334 independent dyads, totaling 707 assessments. Older adults were required to communicate effectively and be clinically suitable for assessment; cognitive status was not formally measured. Exploratory analysis showed adequate sampling adequacy (KMO = 0.861) and a significant Bartlett's test ( < .001), identifying a four-factor structure explaining 61.81% of variance. Confirmatory analysis supported the proposed structure, and Cronbach's alpha ranged from 0.890 to 0.944. Diagnostic accuracy was not evaluated against an independent reference standard; therefore, sensitivity, specificity, predictive values, and clinically meaningful cutoff scores remain to be established.

4. Elder abuse prevention through enhanced caregiver support: a qualitative case study of the COACH intervention.
PMID:日期:2026-09-13The Comprehensive Older Adult and Caregiver Help (COACH) intervention, evaluated in a randomized controlled trial (RCT) by Gassoumis et al. (2024), achieved an unexpected outcome: during the three months after the intervention, no caregivers in the treatment group self-reported elder mistreatment (EM), compared to 23.1% in controls, despite no significant reduction in measured caregiver risk factors such as burden, depression, or anxiety. To explore factors that may have contributed to this outcome, we conducted a case study with three COACH care coaches and their supervisor, supplemented by 100 post-visit notes and six pages of supervisory meeting notes. Thematic analysis identified three elements perceived as central to COACH's impact: tailored, person-centered support; care navigation and resource provision; and culturally responsive, iterative caregiver education. Findings suggest that relational support may mitigate EM risk even when measured risk factors do not change, underscoring the need for theory-informed frameworks to guide caregiver-focused prevention strategies.

5. Intersectional harms: discriminatory abuse of older LGBTQ+ adults in long-term care.
PMID:日期:2026-09-11Discriminatory abuse (harm motivated by a person's identity characteristics, including sexual orientation, gender identity, race and sex) remains under-addressed in mainstream "elder abuse" scholarship. This commentary argues that silence in scholarship and absence in data, concepts and regulatory frameworks produce a form of structural invisibility that leaves older LGBTQ+ people in long-term care vulnerable to harm that is neither recognized nor addressed. Drawing on the case of Noel Glynn, a gay man with dementia subjected to homophobic abuse in an English care home, it calls for discriminatory abuse to be more fully recognized conceptually, empirically and within adult protection practice.

6. Stigma and systemic distrust: a multidimensional sentiment analysis of social media discourse on elder abuse in Chinese long-term care institutions.
PMID:日期:2026-09-08This study analyzed Weibo and Douyin comments posted during a five-day period surrounding a suspected elder abuse incident in a long-term care institution in mainland China. Of 51,776 comments collected, 41,362 remained after preprocessing. We used lexicon-based emotion classification, Baidu AI Cloud sentiment scoring, and TF-IDF and Latent Dirichlet Allocation to examine emotional, temporal, regional, and thematic patterns. Sentiment was predominantly negative (mean = -35.941). Among 4,305 classified comments, resentment was most frequent (n= 1,433, 33.29%), followed by confidence (n= 1,297, 30.13%), vengefulness (n= 659, 15.31%), fear (n= 538, 12.50%), and shame (n= 378, 8.78%). Higher provincial resident-to-direct care worker ratios were associated with more negative sentiment, although this ecological correlation does not establish causality. Recurring themes concerned prison-like images of institutional care, direct care worker stigmatization, regulatory oversight, vulnerability among people aging without children, and tensions surrounding family responsibility.

7. Perceived authoritarian parenting during childhood and elder abuse tendencies in adulthood: internalized resentment as a mediating mechanism.
PMID:日期:2026-08-27The present study examined the relationships among perceived authoritarian parenting (PAP) during childhood, internalized resentment (IR), psychological abuse toward elderly parents (PAEP), and passive elder neglect (PEN) among adults in Pakistan. It further investigated the mediating role of IR in the associations between early parenting experiences and later caregiving behaviors. A cross-sectional survey design was employed, and data were collected from 847 adults using self-report measures. Structural equation modeling revealed that PAP was positively associated with IR, PAEP, and PEN. IR also showed strong positive associations with both forms of elder mistreatment. Mediation analysis indicated that IR significantly mediated the associations between PAP and PAEP and PEN, supporting the proposed theoretical model. The results indicate that perceived early autonomy-restrictive parenting is associated with long-term emotional and behavioral outcomes in family systems. These findings have important implications for understanding elder abuse within intergenerational family relationships and for informing family counseling interventions.

8. The Kerala State Elderly Commission: preventive governance and the protection of older persons in India.
PMID:日期:2026-08-26Population aging is transforming the institutional requirements of elder protection in India. Kerala's establishment of the Kerala State Elderly Commission under the Kerala State Elderly Commission Act, 2025, creates a specialized statutory mechanism for protecting the welfare, rights, and dignity of older persons. This article examines the Commission as an emerging instrument of preventive governance, focusing on its capacities for inquiry, inspection, coordination, legal-aid facilitation, policy guidance, and institutional oversight. Preventive governance is conceptualized through three dimensions: prevention before harm, early intervention when risks emerge, and prevention of recurrence following abuse or neglect. Drawing on statutory provisions, official government materials, and elder-abuse scholarship, the article examines mechanisms of reporting, inquiry, referral, intervention, follow-up, and institutional learning. It also considers challenges involving professional capacity, financing, accessibility, coordination, and implementation. The article argues that Kerala represents a significant institutional experiment whose effectiveness requires systematic empirical evaluation.

9. The mediation role of depression on the relationship between elder abuse and sleep quality: findings from Ardakan Cohort Study on Aging (ACSA).
PMID:日期:2026-08-25Elder abuse is linked to depression and poor sleep quality, yet depression's mediating role remains underexplored, particularly in Iran. This cross-sectional study analyzed baseline data from the Ardakan Cohort Study on Aging (n = 3,128 adults aged ≥ 60 years). Sleep quality, depression, and elder abuse were measured using the Pittsburgh Sleep Quality Index (PSQI), CESD-10, and Elder Abuse Suspicion Index (EASI). Logistic regression and mediation analysis were performed in Stata 18, adjusting for confounders. Elder abuse prevalence was 16.37%; abused individuals were older (69.61±6.75 vs. 68.20±5.98, p=.001) and more often female (p=.03). Depression (27.61% vs. 14.53%) and poor sleep quality (83.40% vs. 72.57%) were more prevalent among abused elders (p<.001). Elder abuse increased odds of poor sleep quality by 46% (OR=1.46; 95%CI:1.12-1.92; p<.001), rising to 75% (OR=1.75; 95%CI:1.25-2.58; p<.001) with depression as mediator. Elder abuse impairs sleep quality, mediated substantially by depression, underscoring the need for mental health-focused elder abuse interventions.

10. A case of sexual assault resulting in life-threatening injuries to an older woman.
PMID:日期:2026-08-24Sexual violence includes all acts directed against an individual's sexuality through force, coercion, or threats, regardless of the relationship between the perpetrator and the victim. According to the Turkish Penal Code, lack of consent or legally invalid consent constitutes a fundamental element of the offense, while conditions such as physical or mental incapacity, common in the geriatric population, are considered aggravating factors. With the global increase in the elderly population, elder abuse has become a significant human rights issue. It is defined as any act or omission within a relationship of trust that causes harm or distress to an older person, including physical, psychological, sexual, and economic abuse, as well as neglect. We present the case of an 83-year-old woman referred following sexual assault. Examination revealed severe vaginal and rectal lacerations requiring surgical intervention. This case highlights the severity of sexual violence and the vulnerability of the geriatric population.
