1. Most common diagnoses requiring physical restriction of psychiatric patients: humanity at the test.
PMID:日期:2011-12-01Attitude about physical restriction has been changing through history. It has always been multidimensional approach, including ethic, medical and judicial aspect. The main aim was establishing distribution of physical restrictions of patients for the following years: 2006/07, 2007/08, 2008/09, 2009/10 and 2010/11. The research included patients that were hospitalized in the Special Hospital for Psychiatric Disorders "Dr. Laza Lazarević" in Belgrade from June 1, 2006 to June 1, 2011. Retrospective review of illness history of hospitalized female patients was done (350 in total) and records were formed containing data on physically restrained patients. The largest number of referral diagnoses belonged to groups F20, F23 and F29; it was established that the number of physical restrictions was associated with referral diagnoses on the level of highly statistically significant difference (p < 0.001). The average number of restrainees per hospitalized patient which was growing in the studied period, which was also shown by the trend line (y = 0.5x + 1.06; R2 = 0.7242). Physical restrictions of psychiatric patients must backed up by benevolence, and it is not by any means the doctor's arbitrariness, which is strongly criticized and represents breach of ethical norms, human rights and the rights of the patient as guaranteed by law. This topic, which, indeed, refers to modern psychiatry, deserves more attention by public discussions, as well as by legislative regulations.

2. Aggression in adolescents: characteristics and treatment.
PMID:日期:2011-12-01Vulnerability of young people and frustration of their basic biological, emotional, cognitive and social needs can induce a series of psycho-pathological manifestations, including aggression. Aim of this study is to examine the manifestations of aggressiveness in young people and to establish the difference between aggressive responses of two age groups; adolescents aged 16-19 years and older adolescents aged 20-26 years. The sample consists of 100 young people aged 16-19 years (46 adolescents) and 20-26 years (54 adolescents). For the purposes of this study, we have constructed a questionnaire in which we entered the data obtained on the basis of a standard psychiatric examination, auto- and hetero-anamnesis data, and data obtained using the standard battery of psychological tests. Statistically significant association was found between verbal aggression and physical aggression (p = 0.002), verbal aggression and suicide attempts (p = 0.02), verbal aggression and substance abuse (p = 0.009), verbal aggression and low frustration tolerance (LFT) (p = 0.007), suicide attempt and LFT (p = 0.052). The younger group was significantly more verbally aggressive compared to the older group (p = 0.01). Verbal aggression, which was significantly associated with physical aggression, suicide attempts, substance abuse and LFT, indicates the need for timely interventions for the prevention of more serious and malignant forms of aggression.

3. Hetero-aggressive outbursts in patients' behaviour in emergency psychiatry.
PMID:日期:2011-12-01Hetero-aggressive behaviour in patients with mental disorders is frequent and requires special skills of doctors and other medical staff in order to urgently protect the patient and indicate hospitalization. The objective of this paper was to examine the frequency of the most common diagnostic categories in psychiatric patients with hetero-aggressive behaviour. The secondary objective was to examine aspects of realized hospitalization (voluntary, involuntary) and socio-demographic characteristics of these patients. The sample was formed of 100 patients who expressed hetero-aggressive behaviour just before and during admission for hospital treatment. The data was collected from auto-anamnesis and hetero-anamnesis, and they were based on general somatic, neurological and psychiatric examination. The statistical analysis used descriptive analysis and Pearson's chi2-test. The frequency of the most common diagnostic categories was: F20-F29 73%; F10-F19 19%; F60-F69 11%; F00-F09 8%; F30-F39 and F70-F79 6%. Involuntary hospitalization was implemented in 56%. Socio-demographic characteristics of examinees revealed that the most common were: male gender (63%) and younger age, dominant secondary school qualification (74%), unmarried (76%), unemployed (76%), frequently repeated hospitalizations (66%). The most common diagnostic categories to which the patients with hetero-aggressive behavioural outbursts belonged were from the schizophrenic circuit, followed by substance abuse, personality disorders, etc. Involuntary hospitalization was the most common form of hospitalization in patients with violent behaviour. Socio-demographic characteristics of examinees revealed this profile as the most common: male gender, younger age, secondary education, unmarried and unemployed, repeatedly hospitalized.

4. Differentiation between opiate addicts in relation to judicial problems.
PMID:日期:2011-12-01Some research results point to significant benefit in the use of methadone substitution treatment in reduction of criminogenic activities in opiate addicts, as in positive affect on their somatic and mental state. The objective of the study was to indicate factors which lead to criminogenic activities in addicts with judicial problems before entering substitute, methadone program. Addicts were divided into two groups: addicts who had judicial problems before they entered substitution methadone program (group A-46 addicts) and addicts withoutjudicial problems (group B-20 addicts). A questionnaire containing basic data about the addicts in the treatment program (Pompidou questionnaire) was for questioning. A statistically significant difference was recorded related to the way of taking PAS. The largest number of examinees from the group A took primal PAS intravenously (41; 89.1%), while from the group B 11 took it intravenously (55.0%). The majority of examinees in the group A committed the first criminal act before taking PAS (psychoactive substances) (19; 41.3%), then after taking so-called harder PAS (16; 34.8%), and finally after taking the so-called lighter PAS (11; 23.9%). In somewhat over half of the examinees in the group A (24; 52.2%) the measure of juvenile court was imposed. A suspended sentence was passed upon 19 (41.3%) examinee, then prison sentence in 16 (34.8%), multiple prison sentences in 6 (13.0%) and misdemeanour in 4 (8.7%). Future research at our centre should show the efficiency of methadone program in a decrease of risky behaviour, degree of criminogenic activity and judicial problems, improvement of life quality, as well as show the ways for preventive acting.

5. Forensic psychiatric expertise: posttraumatic stress disorder.
PMID:日期:2011-12-01This article presents our experiences in the field of forensic post-traumatic stress disorder (PTSD). The study examined parameters of 30 patients with PTSD who were the subject of forensic expertise (PTSDF) and in 30 patients with PTSD who were not (PTSDN). Clinical research and the battery of tests (Impact of Event Scale - IES, Mississippi Scale, and list of symptoms of PCL-M) covered a total of 60 male subjects with a verified diagnosis of PTSD. The study involved socio-demographic variables, catastrophic experience, enduring personality change after catastrophic experience (EPCACE), comorbidity disorders and non-material damage. In terms of respondents' average age, years of education, marital status, time of military engagement, there were no statistically significant differences between PTSDF and PTSDN groups. In terms of EPCACE statistically significant differences were found in both PTSDF and PTSDN groups. Among PTSDF respondents (N = 30) EPCACE was verified in 83.33% (N = 25), and among PTSDN in 23.33% (N = 7) (p < 0.05). In terms of comorbidity disorders and the parameter of non-material damage no statistically significant differences were found either in PTSDF or PTSDN group. In terms of EPCACE there were statistically significant differences both in PTSDF and PTSDN group. Forensic and psychiatric meaning of PTSD encompasses a number of complex elements on which forensic expert opinion depends, while the existence of PTSD diagnosis itself does not affect creation of opinions. The study should serve to identify methodological and conceptual problems in the field of forensic aspects of PTSD.

6. Lamotrigine augmentation in delirium tremens.
PMID:日期:2011-12-01Delirium tremens (DT) is most severe neurological complication of alcohol withdrawal with high mortality rate. DT is related to an altered balance of excitatory and inhibitory amino-acid neurotransmitters, which is basically due to upregulation of glutaminergic neurotransmission induced by chronic ethanol exposure. Lamotrigine (LTG) is believed to act by reducing excitatory glutamate release due to inhibition of Na (+) channels. The aim of this study was to investigate efficiency of the LTG therapy in the treatment of delirium tremens. This prospective clinical study included 240 patients with ICD-10 criteria for DT, who were randomly divided into control and experimental group. The patients were observed within 28 days at the Intensive Care Unit of the Centre for Urgent Psychiatric disorders, according to a specific protocol, which included CIWA-Ar and MDAS clinical scales. Control and experimental group were treated according to the NIAAA protocol for 2004, and experimental group with adding of LTG according to a specific program. CIWA and MDAS scores in the experimental and control group has statistical significant differences after the third day (p > 0.1), and especially after the fifth day (ECIWA5/KCIWA5 = 8.36 +/- 6.782/32 +/- 5.562; EMDAS5/KMDAS5 = 4.89 +/- 3.408/26.33 +/- 1.497) (p > 0.5). LTG is significantly efficient in the treatment of delirium tremens, but it does not decrease mortality rate.

7. Quality of life of schizophrenic patients treated with haloperidol depot and injection preparation of long-lasting risperidone.
PMID:日期:2011-12-01In the last decades psychiatric patients' quality of life attracts great attention of researchers. Improving the quality of life of schizophrenic patients is increasingly becoming an imperative in pharmacological therapy. Analysis of certain aspects of quality of life in patients with schizophrenia treated with depot formulations of a typical antipsychotic (haloperidol) and injection preparation of a long-acting atypical antipsychotic (risperidone). Research was conducted as a cross-sectional study that included 60 patients of both genders. Examinees diagnosed with schizophrenia (ICD-10, F20.0-F20.9) were divided into two groups: the group of patients that received haloperidol depot (n = 30) and the group of patients that received injection preparation of long-acting risperidone (n = 30). In order to assess the quality of life, social functioning scale (SFS), satisfaction with life scale (SWLS), and short version of World Health Organization quality of life scale (WHO-QoL-Brief) were applied. Results showed statistically significant differences when it comes to social activity and satisfaction with life in favour of patients treated with injection preparation of long-acting risperidone. Examinees from this group were much more satisfied with themselves, their health and sleep compared to those on haloperidol depot. There was no statistically significant difference found on the quality of life scale. Applying the scales for the assessment of the quality of life of schizophrenic patients in terms of psychosocial functioning, statistically significant difference between groups was found. Results showed higher scores in the group of patients treated with injection preparation of long-acting risperidone concerning social activities and life satisfaction.

8. Changing the structure of the hospitalized patients at the Psychiatric Clinic Vrapce.
PMID:日期:2011-12-01Psychiatric treatment in developed countries has significantly changed in the last few decades, with a significant shortening of the duration of hospitalization. Numerous West European countries are undertaking health system reforms to improve treatment efficiency. The aim of the study was to present the structure of hospitalized patients according to diagnostic categories of International Classification of Diseases and Related Health Problems - 10th Revision (ICD-10), and average duration of hospitalization from 2001 till 2010. Data was collected from hospital medical charts for the period from 2001 till 2010 on the number of hospitalized patients, diagnosis and duration of hospitalization. Number of hospitalized patients increased by 27% (6309 vs. 8032). Average duration of hospitalization decreased for 45.1% (72.3 vs. 39.7 days), while the number of hospitalized patients decreased from 1284 (20.4%) in 2001 to 995 (12.4%) in 2010, the number of patients hospitalized because of depressive disorder increased from 309 (4.9%) in 2001 to 994 (12.4%) in 2010. The number of patients hospitalized because of enduring personality changes increased from 104 (1.7%) in 2001 to 449 (5.6%) in 2010. In the last decade the number of hospitalized patients suffering from schizophrenia at the Psychiatric Clinic Vrapce decreased, while the number of those suffering from depression and enduring personality change increased. The number of hospitalizations in the observed period increased overall. Observed changes can be explained by new therapeutic approaches, but also by objective social and economic factors and organization of the healthcare system.

9. Epidemiological characteristics of suicidal patients admitted to the Psychiatric Clinic in Kragujevac: a ten-year retrospective study.
PMID:日期:2011-12-01Suicide attempts are a significant public health problem. They are associated with significant morbidity and result in significant use of health care resources. The aim of our study was to analyze epidemiological characteristics attempts of suicide for persons in the period 2001-2010 who were hospitalized at the Psychiatric Clinic in Kragujevac after the attempt of suicide. We analysed 286 clinical records of patients who had attempted suicide (133 males, 153 females). We used the illness history of patients, who were treated at the Psychiatric Clinic in Kragujevac after their suicide attempts. We made the analysis of socio-demographic characteristics of the sample and characteristics of the suicide act itself. Our results showed a higher rate of female population, except for the period 2003-2005:2003 - 14 males (58.3%), 2004 - 15 (62.5%) and 2005 - 60%. The largest number of attempts was in the population aged between 25 and 34 years (23.8%). The percentage of persons from urban areas was higher (66.4%). Depressive disorder was a common pre-existing disorder before a suicide attempt (42.3%). Most attempts of suicide were performed by overdosing on medications (57.7%). In the last three years we observed a decrease of suicide attempts. We noticed that middle aged persons are the most frequent ones to attempt suicide. Depressive disorder is the most frequent pre-existing condition of suicide attempt. In order to prevent suicide attempts we must improve early detection of depressive disorder, as well as initial treatment.

10. Reasons and conditions for involuntary hospitalization with special emphasis on detention without consent of mentally disordered person who voluntarily started hospital treatment.
PMID:日期:2011-12-01The nature of some mental illness is such that persons affected by their conduct endangering life, health and safety, as well as life, health and safety of others from their surroundings. These persons, because of their mental condition, are often unable to properly assess their own interest. Because of the above it is permitted for these persons, under certain circumstances, to be forcibly hospitalized against their will. However, the problem of involuntary hospitalization of persons with mental disorders remains a controversial and complex ethical and legal problem, because it is characterized by a conflict of opposing interests and moral values. The main reason is the fact that involuntary hospitalization is an act of deprivation of liberty and intervention into the personal integrity, which at that the measure is taken against the individual who has not committed any crime. In order to provide restricted approach to the application of compulsory hospitalization, it is necessary to pass a legislation on the protection of persons with mental disorders that would more closely define the undertaken proceedings, reasons and conditions for involuntary detention and involuntary hospitalization in a psychiatric institution, forced detention of voluntarily hospitalized persons and penal policy violation of this law. It is necessary to initiate the procedure for amending the Law on Contested Procedure, which would reform the procedure for compulsory hospitalization, as an important segment of mentally disordered persons' rights, in order to be in accordance with international and European standards within this field.
