To reduce reliance on labor-intensive and operator-dependent explicit plaque segmentation, we evaluated whether a texture-change score (ΔTexture) derived from semi-automated 3-D ultrasound (3-DUS) vessel wall segmentation predicts vascular events better than baseline vessel wall texture (BL-Texture), vessel wall volume change (ΔVWV) and Framingham risk score. 3-DUS images were acquired from 272 patients with carotid atherosclerosis at baseline and 1 y later. Vessel walls were delineated using a validated, minimally user-initialized segmentation workflow. Longitudinal texture change was summarized as vessel wall ΔTexture and compared with BL-Texture, ΔVWV and Framingham risk score. Patients were followed for up to 5 y (median, 3.16 y) for myocardial infarction, transient ischemic attack and stroke. ΔTexture and BL-Texture predicted vascular events (log-rank, p < 0.001 for both), whereas ΔVWV did not. No event occurred in the lowest-risk tertile of ΔTexture. The area under the receiver operating characteristic curve of ΔTexture was 0.89, compared with 0.82 for BL-Texture, 0.53 for ΔVWV and 0.53 for Framingham risk score. In Cox regression, ΔTexture (median hazard ratio, 4.05; p < 0.001) and BL-Texture (median hazard ratio, 3.63; p < 0.001) were significant predictors, whereas ΔVWV and Framingham risk score were not. Vessel wall ΔTexture derived from minimally user-initialized 3-DUS vessel wall segmentation predicted vascular events better than ΔVWV and Framingham risk score. This approach provides a scalable imaging biomarker for vascular event risk assessment without requiring explicit plaque segmentation.

To develop a Doppler-based radiomics model that utilizes tumor characteristics related to neovascularization, with or without integrating intraoperative frozen section analysis (FSA), to enhance borderline ovarian tumors (BOT) prediction. In this retrospective multicenter study, patients diagnosed with BOT or epithelial ovarian cancer (EOC) between January 2010 and October 2022 across five centers were included. Data from Center I were randomly divided into training (80%) and validation (20%) sets, while Centers II-V provided the test set. Three ultrasound-radiomics models were developed: color Doppler features (CD), grayscale features (GS), and combining both (GSCD). An Integration Model was developed by incorporating clinical variables, FSA and the best-performing radiomics model. Color Doppler signals were acquired through three channels with RGB information. Model performance was evaluated using the area under the receiver operating characteristic curve (AUC). A total of 1244 patients with a median age of 49 years (IQR, 37-58 years) were included, consisting of 520 patients with BOT and 724 EOC. The GSCD exhibited the best predictive performance for BOT among the three ultrasound-radiomics models, showing an AUC of 0.975 (95%CI, 0.962-0.985) in the training set, with a sensitivity of 92.9% and a specificity of 88.9% in the validation set. The Integration Model further improved the performance, achieving an AUC of 0.997 (95%CI, 0.993-0.998) in the training set, with a sensitivity of 87.2% and a specificity of 98.7% in the validation set. Additionally, both the GSCD and Integration Model effectively distinguished BOT from Stage I EOC, with AUCs of 0.919 (95% CI: 0.849-0.963) and 0.936 (95% CI: 0.871-0.974) in the validation set. We developed a Doppler-based radiomics model utilizing red-green-blue information. This model, irrespective of integration with FSA, demonstrated improved accuracy in predicting BOT and stratified the FIGO Stage EOC.

Cerebral perfusion disturbances contribute to neonatal brain injury, and ultrafast power Doppler can quantify regional cerebral blood volume but relies on manual segmentation that limits clinical use. We developed and validated a U-Net model for automated regional brain segmentation on B-mode images derived from neonatal transfontanellar ultrafast ultrasound to enable rapid arterial and venous cerebral blood volume quantification. Time-integrated, log-compressed ultrafast B-mode images from 80 neonates were manually segmented by an expert into the cortico-sub-cortical areas, the cingulate gyrus and the basal ganglia, and then used to train a U-Net with class-weighted loss (a 70/15/15 train/validation/test split). The trained model was then applied to segment B-mode images, with the resulting regions of interest used to quantify arterial and venous cerebral blood volume from the corresponding power Doppler data. Segmentation performance was quantified using the Dice similarity coefficient, with agreement between artificial intelligence- and manually derived cerebral blood volume assessed by Bland-Altman analysis. The median Dice similarity coefficients were 83%, 84% and 88% for the three regions. Mean Bland-Altman differences were ≤0.103 dB (arterial) and ≤0.096 dB (venous). Perturbation of the predicted masks by ±35% surface area change showed the narrowest limits of agreement at the unperturbed prediction, changing by no more than 0.1 dB between -5% and +5%, demonstrating automated segmentation as a rapid, reproducible alternative to manual delineation.

Central retinal artery occlusion (CRAO) is an acute that can result in irreversible vision loss and blindness. However, the efficacy of conventional treatments for retinal artery recanalization and vision restoration remains uncertain. The objective of our study was to evaluate effectiveness and potential side effects of conventional multimodal treatment (CMT) combined with ultrasound-stimulated microbubbles (USMB) in the patients with CRAO. In this nonconcurrent controlled trial, 34 CRAO patients were consecutively enrolled in the USMB group (CMT+ USMB). USMB was performed using a commercially available diagnostic ultrasound (US) device with a clinical US contrast agent (microbubbles, MBs). Twenty-three CRAO patients treated only with CMT were assigned to the CMT group (historical control). The primary endpoint was best-corrected visual acuity (BCVA; converted to logMAR) at the initial and follow-up visits, and the secondary endpoint was safety. Fundus photography and optical coherence tomography (OCT) were performed in the USMB group upon admission and within 24 h of USMB treatment. Of 64.7% (22/34) of patients showed clinically significant visual improvement (≥+0.3 logMAR) in the USMB group at 3 mo follow-up, compared to only 21.7.0% (5/23) in the CMT group (p < 0.001). Fundus photography and OCT demonstrated the safety performance of USMB for patients with CRAO. The combined therapeutic regimen did not induce any additional toxicity or increased frequency of side effects when compared to CMT alone (p > 0.05). USMB-augmented CMT may improve the clinical efficacy of CRAO patients, with sustained stability of visual recovery at 3-mo follow-up. This innovative non-invasive approach offers excellent clinical safety and feasibility for acute CRAO management.

Single-cycle histotripsy was investigated for the rapid ablation of porcine liver using high pulse repetition frequency (PRF) pulsing methods. Recent evidence from our group has suggested that single-cycle histotripsy does not experience a decrease in ablation efficiency across a wide range of PRFs. Histotripsy ablation efficiency was quantified in a 1% agarose gel phantom containing a layer of red blood cells using a 500 kHz transducer with an F# of 0.61. Lesions were generated using PRFs of 500-2500 Hz at peak negative pressures of 15, 24 and 42 MPa with an 800 pulse exposure, with results compared to a low PRF control (1 Hz). Second, high PRFs were used for volumetric ablations of porcine liver using doses of 400-800 pulses per point. Tissue damage was assessed via ultrasound imaging and histologically using Masson's trichrome staining. Ablation experiments in red blood cell gel phantoms showed no significant differences in ablation efficacy as PRF increased from 500 to 2500 Hz, supporting the hypothesis that single-cycle histotripsy can achieve efficient tissue ablation at high PRFs. Volumetric tissue ablation experiments showed that single-cycle histotripsy resulted in effective tissue ablation for all tested PRFs, with near complete or complete ablation observed after 600-800 applied pulses. Partial ablation in some samples at lower doses (400 pulses for 500-1500 Hz PRF, 600 pulses for 2000-2500 Hz PRF and 800 pulses for 2500 Hz PRF) suggested a slight decrease in ablation effectiveness with increasing PRF. This study supports the hypothesis that high PRF single-cycle histotripsy can achieve faster tissue ablation while maintaining precise volumetric lesion generation.

To evaluate the applicability and diagnostic performance of multimodal ultrasound in assessing intramuscular venous disorders of the lower extremity. This retrospective study included 142 individuals who underwent lower-limb venous ultrasonography: 50 with intramuscular venous dilatation, 42 with intramuscular venous thrombosis (IMVT), and 50 healthy controls. Intramuscular vein diameter, muscle thickness, and muscle stiffness of the triceps surae were evaluated using multimodal ultrasonography. Ultrasound parameters and biochemical markers were compared among the three groups. Compared with the dilatation (13.74 ± 3.00 mm, 18.23 ± 3.04 mm) and control groups (13.69 ± 2.71 mm, 16.58 ± 2.93 mm), the IMVT group exhibited reduced medial gastrocnemius (MG) and soleus muscle thicknesses (12.36 ± 2.95 mm and 15.35 ± 3.04 mm, respectively; p < 0.05). The dilatation group had greater soleus thickness than the control group (18.23 ± 3.04 vs. 16.58 ± 2.93 mm, p < 0.05). MG shear wave velocity (SWV) was lower in the IMVT group than in the control group (1.80 [1.69, 1.96] vs. 1.99 [1.74, 2.26] m/s, p < 0.05), whereas soleus SWV was lower in the IMVT group but higher in the dilatation group than in the control group (1.95 [1.75, 2.59] vs. 3.10 [2.19, 3.73] vs. 2.67 [2.14, 3.19] m/s; p < 0.05). MG vein diameter, soleus vein diameter, soleus SWV, and D-dimer levels were key factors influencing IMVT (p < 0.05). A logistic regression model combining these indicators achieved an area under the curve of 0.934 (95% CI: 0.888-0.972) for differentiating IMVT. Multimodal ultrasound represents a useful adjunctive tool for screening individuals at high risk of IMVT.

Bone exhibits piezoelectricity originating from the alignment of collagen fibers, enabling detection of acoustically induced electric polarization through the acoustically stimulated electromagnetic (ASEM) method. In this technique, piezoelectric polarization is modulated at the radio frequency of ultrasound irradiation and captured by a resonant antenna precisely tuned to the frequency of the ultrasound waves. Goto-Kakizaki rats, a model of type 2 diabetes mellitus (DM), were bred for 40 wk. Their femoral bones were harvested for micro-computed tomography (micro-CT) imaging and mechanical evaluation using a three-point bending test. Additionally, osteoporosis was induced in Sprague-Dawley rats by immobilizing the right knee in a cast for 12 wk, confirmed via micro-CT. ASEM measurements revealed significantly lower signal intensities in both the metaphysis and diaphysis of femoral bones from the DM rats compared to controls. Among the mechanical parameters examined, breaking displacement showed the strongest association with ASEM signal intensity (r = 0.499, p = 0.058). Although this association did not reach statistical significance, it suggests that ASEM may be sensitive to deformation-related aspects of bone quality and warrants further investigation using direct measures of post-yield behavior. Following decalcification, ASEM signal intensity in metaphysis remained lower in the DM group than in controls. Similarly, immobilized femora exhibited a significant reduction in ASEM signal intensity in metaphysis. These findings demonstrate that ASEM signal intensity is reduced in osteoporotic bone across two models with distinct pathological backgrounds. The persistence of the between-group reduction after decalcification supports the involvement of collagen-related electromechanical alterations, although the specific collagen properties responsible remain to be identified. The present findings support the potential of the ASEM method to detect osteoporosis-associated changes in the collagen-related electromechanical response of bone and to image their spatial distribution.

Right-to-left shunting detected by contrast-enhanced transcranial Doppler (cTCD) is commonly interpreted as evidence of an intra-cardiac communication such as a patent foramen ovale, often prompting cardiac-focused investigations and management decisions in patients with ischemic stroke or transient ischemic attack. However, cTCD positivity alone is insufficient to localize shunt origin; multiple extra-cardiac mechanisms can generate similar microbubble patterns and lead to diagnostic misclassification. To clarify these alternative pathways, we narratively reviewed the literature from 1980 to 2025 across PubMed, Embase and Scopus, identifying 26 studies in which cTCD was used to evaluate non-cardiac causes of right-to-left shunting, including hepatopulmonary syndrome, pulmonary arteriovenous malformations, physiologic intra-pulmonary shunts, intra-pulmonary arteriovenous anastomoses, extra-cardiopulmonary and extra-hepatic venous anomalies and pressure-dependent pulmonary shunting. Across these conditions, cTCD revealed reproducible physiologic signatures-such as delayed microbubble appearance, prolonged signal duration, temporal dispersion, variable bubble load, limited provocation response and distinct reproducibility patterns-that differ meaningfully from the early, Valsalva-dependent signals typical of isolated intra-cardiac shunts. These findings underscore that cTCD should be interpreted as a dynamic physiologic assessment rather than a binary screening test for cardiac defects. Recognizing the characteristic behaviors of non-cardiac shunts can improve diagnostic precision, reduce the misattribution of stroke mechanisms and support more appropriate, individualized patient management.

Idiopathic intracranial hypertension (IIH) is a major cause of elevated intracranial pressure (ICP) typically diagnosed by lumbar puncture (LP) with an opening pressure ≥18 mmHg; however, LPs are invasive and variable. Subharmonic-aided pressure estimation (SHAPE) is an ultrasound-based technique that relies on the subharmonic signal of contrast microbubbles to detect surrounding pressure changes. This study investigated the utility of SHAPE for noninvasive assessment of ICP. This IRB-approved, two-center study enrolled subjects undergoing LP for suspected IIH. SHAPE data were obtained pre- and post-LP from the ophthalmic artery and vein. The SHAPE gradient, defined as the difference between ophthalmic artery (OA) and ophthalmic vein (OV) subharmonic signal intensities, was calculated offline and the change in the SHAPE gradient between pre- and post-LP scans was correlated with the measured reduction in ICP following LP. Intracranial elastance (E), pressure-volume index (PVI), and optic nerve sheath diameter (ONSD) were also compared with LP pressures. A total of 20 patients were enrolled. A significant negative correlation was observed between the SHAPE gradient and the reduction in ICP following drainage (r = -0.61, p = 0.01). Individual SHAPE values pre- and post- LP did not correlate with the individual OP and CP pressures (p = 0.72). Neither ONSD, E, nor PVI correlated significantly with LP pressures (p ≥ 0.05). SHAPE demonstrated responsiveness to intracranial pressure changes and may have potential for noninvasive monitoring of ICP trends.

Chronic wounds are often infected by bacteria, which aggregate into biofilms. Biofilms impede antibiotic efficacy, promote infection relapse, and are the leading cause of chronic wound treatment failure. Ultrasound combined with phase-change contrast agents (PCCA) has shown potential to enhance antimicrobial delivery in animal models, yet quantification of cavitation in animal models remains unexplored. In a murine wound infection model with methicillin-resistant Staphylococcus aureus (MRSA), animals received topical gentamicin and palmitoleic acid twice daily from days 2-4 post infection. One daily treatment was followed by administration of octafluoropropane (OFP) PCCA and ultrasound (1.1 MHz, 9.09% duty cycle) at either 700 or 1700 kPa. Passive cavitation signals were recorded every 15 seconds during five one-minute exposures. Cavitation dose was quantified from harmonic and broadband emissions and bacterial burden was assessed on day 5. A strong inverse correlation was observed between cavitation dose and wound bacterial burden (R = 0.88), indicating that increased cavitation was associated with greater reduction in bacterial burden. Cavitation was sustained throughout treatment with 1700 kPa and significantly higher than at 700 kPa (p = 0.0145), where signal declined over time. Limited wound coverage (∼40%) may have reduced overall efficacy. This study provides the first in vivo evidence correlating cavitation dose to bacterial reduction in MRSA biofilm-infected wounds. These findings support cavitation monitoring as a valuable feedback tool for optimizing ultrasound-enhanced antimicrobial therapies.
