1. Impact of Smokeless Tobacco on Semen Quality: A Systematic Review and Meta-Analysis.
PMID:日期:2026-10-04Smokeless tobacco products are widely regarded as a safer alternative to smoking. However, a growing body of evidence points to harmful effects on male reproductive health. The aim of this systematic review and meta-analysis was to assess the association between smokeless tobacco use and the principal indicators of male infertility. This review was conducted in accordance with PRISMA 2020 guidelines and registered with PROSPERO (CRD420251056391). A systematic search of PubMed, Medline, Web of Science, and Google Scholar was performed from January 1993 to December 2025. Observational and interventional studies that examined smokeless tobacco exposure in adult men were eligible. Risk of bias was assessed with ROBINS-I and the certainty of evidence with GRADE. Sixteen studies with a total of 3401 participants met the inclusion criteria. Smokeless tobacco use was associated with lower sperm motility (mean difference, MD: -9.56; 95% confidence interval, CI: -14.85 to -4.28), lower sperm concentration (MD: -14.90; 95% CI: -21.43 to -8.37), lower sperm vitality (MD: -12.86; 95% CI: -19.86 to -5.86), and a higher proportion of abnormally shaped sperm (MD: +3.58; 95% CI: 0.34 to 6.82). Semen pH was slightly but significantly lower in users (MD: -0.03; 95% CI: -0.04 to -0.02). Semen volume was also lower, but the difference did not reach statistical significance (MD: -0.36; 95% CI: -0.76 to 0.04; p = 0.078). Heterogeneity was high for several outcomes (I > 90%), but sensitivity analyses did not change the direction of effect. The GRADE certainty of evidence was moderate for all pooled outcomes except semen volume, for which it was low. Smokeless tobacco use was associated with poorer semen quality, particularly in sperm motility, concentration, vitality, and morphology. These findings are consistent with smokeless tobacco being an important and frequently overlooked risk factor for male infertility, although the evidence is observational and derived largely from South Asian populations. Clinicians should consider asking about smokeless tobacco use during infertility evaluations, and targeted public health measures appear warranted to address its reproductive effects.

2. Hypogonadism in Cisgender Men Living With HIV: Prevalence, Clinical and Metabolic Features.
PMID:日期:2026-10-04Hypogonadism is frequently observed in men living with human immunodeficiency virus (HIV), although its prevalence, etiology, and predictors remain incompletely understood. To evaluate the prevalence of hypogonadism and its association with metabolic factors in men living with HIV (MLWH), on stable antiretroviral therapy. Total testosterone (TT), sex hormone-binding globulin (SHBG), calculated free testosterone (cFT), metabolic parameters, and ADAM questionnaire responses were assessed in 136 MLWH (19-73 years), classified as hypogonadal or eugonadal. Subnormal cFT or TT levels were detected in 35.3% of the participants; 52.1% had isolated low cFT and 78.4% were hypogonadotropic. ADAM showed 75.6% sensitivity and 44.7% specificity. Hypertension, dysglycemia, higher triglycerides levels, higher BMI, increased waist circumference, older age, longer time of infection, and insulin resistance indexes were positively associated with hypogonadism in univariate analysis. Serum estradiol levels were similar between groups. After multivariate analysis, age and dysglycemia remained independently associated with testosterone deficiency. One-third of MLWH had hypogonadism, mostly of central origin. cFT assessment was essential to avoid underdiagnosis. Hypogonadism in MLWH seems to be associated with metabolic impairment, however only age and dysglycemia remained independently associated after adjustment.

3. Androgens Modulate Tumor-Immune Interactions in Testicular Germ Cell Tumors.
PMID:日期:2026-09-29The majority of testicular germ cell tumors (TGCT) are derived from the germ cell neoplasia in situ and arise after puberty in the local presence of high levels of testosterone. The seminoma subtype features a rich inflammatory infiltrate which impacts tumor progression but the role of testosterone in shaping tumor-immune interactions is yet unknown. We characterized androgen receptor and associated gene expression in the TGCT microenvironment and investigated the impact of dihydrotestosterone (DHT) on seminomatous tumor cells as well as stimulated T cells and monocytes. As an in vitro model of the seminoma microenvironment, cocultures of TCam-2 cells with immune cell subtypes were analyzed in the presence of DHT. Gene expression was analyzed in tumorous and nontumorous tissue specimens from TGCT patients and TGCT cell lines. Bulk and single-cell RNA-sequencing data were used for validation. T cells and monocytes were isolated from the blood of healthy individuals and either activated in vitro or cocultured with TCam-2 cells. Proliferation and metabolic assays, quantitative RT-PCR, flow cytometry, and ELISA were employed to assess the impact of DHT on tumor and immune cells. Androgen receptor and related gene expression was demonstrated in tumorous tissue from patients, TGCT cell lines, and activated primary immune cells, which was supported by the analysis of published RNA-seq data sets. DHT exerted only a minor effect on TCam-2 cells, whereas hormone treatment markedly mitigated T cell and monocyte activation in a dose-dependent manner. In addition, DHT modulated immune cell activity and selected features of TCam-2 cells in T cell and monocyte cocultures serving as an in vitro model of the seminoma microenvironment. Our results suggest that androgens present in tumorous testis of TGCT patients could impact tumor-immune interactions and thereby clinical outcome.

4. Cross-Sectional Study of the PREValence of Erectile DysfunctIon in the Arteriopathic Population (PREVEDI).
PMID:日期:2026-09-26Although the association between erectile dysfunction (ED) and cardiovascular disease is well established, its prevalence in patients with atherosclerosis remains poorly defined. To evaluate ED prevalence, severity, and associated risk factors (RFs) in patients with atherosclerotic disease. This single-centre, cross-sectional, non-randomised observational study included male patients referred to a vascular surgery service with documented atherosclerotic disease. Erectile function was assessed using the International Index of Erectile Function-15 questionnaire, and RFs were analysed using univariable and multivariable regression models. A total of 162 patients were enrolled (mean age, 70.1 ± 8.6 years); ED was present in 93.8%, with severe ED observed in 62.3%. Multivariable analysis confirmed age as a significant RF for ED, particularly > 75 years, while age, diabetes mellitus, and subjective ED duration were significantly associated with greater severity. The high prevalence of ED supports its role as a manifestation of systemic vascular disease linked to endothelial dysfunction and diffuse atherosclerosis. ED is highly prevalent among patients with clinically overt atherosclerotic disease requiring vascular surgery referral, and may represent a marker of systemic vascular pathology. Routine assessment of sexual function may improve cardiovascular risk stratification and preventive management.

5. Testosterone Undecanoate Significantly Reduces Low Density Lipoprotein Cholesterol and Triglycerides in Men With Adult-Onset Testosterone Deficiency.
PMID:日期:2026-09-25Adult-onset testosterone deficiency, a condition characterised by low serum testosterone and hypogonadal symptoms is associated with a higher incidence of cardiovascular disease (CVD) and mortality. To evaluate associations between change (Δ) in total cholesterol (TC), triglycerides (TG) and low-density lipoprotein (LDL)-cholesterol and testosterone undecanoate (TU) treatment. We analysed a registry of men with adult-onset TD (409 TU-treated and 415 TU-untreated men, median follow-up 153 months), restricting the analysis to men whose statin treatment was not changed during follow-up, thus avoiding statin related effects. The study cohort comprised the following groups: TU-untreated/not on statins (n = 95), TU-untreated/on statins (n = 245), TU-treated/not on statins (n = 194), and TU-treated/on statins (n = 207). Intra-group non-parametric statistics were performed as baseline characteristics varied between the study groups. We used multivariate and logistic regression models to determine predictors of ∆ lipids (LDL-cholesterol and TG) and mortality. Whilst there was a significant decrease in LDL-cholesterol and TG (mostly in the initial 24 months) in the TU-treated men, the reverse was evident in the untreated men, irrespective of concurrent statin therapy. Baseline LDL-cholesterol and TG inversely predicted ∆ values in the TU-treated men, regardless of statin therapy. LDL-cholesterol at final assessment appeared independently associated with mortality. In this long-term, real-world registry, TC, TG, and LDL-cholesterol decreased in the TU-treated men with baseline values predicting the change, whilst a gradual increase was observed in the TU-untreated men. Using published data, the relative risk reduction in CVD due to LDL-cholesterol reduction is estimated to be around 40%.

8. The Influence of Suture Type on Postoperative Outcomes in Minimally Invasive Vasectomy.
PMID:日期:2026-09-24This study compares vasectomy outcomes between Chromic Gut and Monocryl sutures used for fascial interposition in a large cohort of patients undergoing minimally invasive vasectomy. Current guidelines do not recommend a specific suture material for fascial interposition; therefore, this study aims to evaluate two commonly used sutures to help inform surgical practice. This retrospective study evaluated 1389 patients who underwent vasectomy by a single surgeon between March 2011 and October 2022. Patients were stratified based on suture type used for fascial interposition: Chromic Gut or Monocryl. The primary outcome was vasectomy failure, defined as the presence of live sperm or >100,000 non-motile sperm/mL on post-vasectomy semen analysis (PVSA). Secondary outcomes included postoperative pain, symptomatic sperm granuloma formation, repeat procedures, and post-vasectomy pain syndrome. Of the 1389 included patients, Chromic Gut was used in 1145 and Monocryl in 244. There were six confirmed failures (0.61%), with no significant difference in failure rate between suture types (p = 1.0). A higher rate of postoperative pain-related follow-up was observed in the Chromic Gut group compared with the Monocryl group (p = 0.045). Additionally, the rate of sperm granuloma and follow-up procedures were higher in the Chromic group, but without statistical significance. This large single surgeon series found no statistically significant difference in vasectomy success between Chromic Gut and Monocryl sutures for fascial interposition. However, an increase in postoperative pain-related follow-up was associated with the use of Chromic Gut. Both materials were associated with low failure rates, consistent with published data.

9. Trimethylamine N-oxide, a Gut Microbiota Metabolite, Promotes Diabetes-Mediated Erectile Dysfunction via a PANoptotic-Like Process Involving Pyroptosis, Apoptosis, and Necroptosis in CCSMCs.
PMID:日期:2026-09-21Trimethylamine N-oxide (TMAO), a key metabolite derived from gut microbiota dysbiosis, is known to influence various diabetic vascular complications. However, its specific role and the programmed cell death mechanisms underlying the development of diabetes-mediated erectile dysfunction (DMED) remain to be fully elucidated. To investigate the mechanism by which TMAO mediates corpus cavernosum smooth muscle cell (CCSMC) dysfunction and promotes DMED in type 1 diabetic rats, with a focus on the involvement of the PI3K/Akt-regulated PANoptotic-like process. A type 1 diabetes-mediated erectile dysfunction (T1DMED) rat model was established. Fecal samples were collected for gut microbiota analysis, and serum TMAO levels were measured. Primary CCSMCs were isolated, and cell purity was confirmed via immunofluorescence and flow cytometry. An in vitro TMAO-induced CCSMC injury model was constructed, followed by transcriptome sequencing to identify differentially expressed genes (DEGs) and enriched KEGG signaling pathways. Western blotting and immunohistochemistry (IHC) were performed to detect markers of a PANoptotic-like process (encompassing pyroptosis, apoptosis, and necroptosis) and pathway proteins in penile tissues. Human single-cell RNA sequencing (scRNA-seq) data from penile cavernous tissues were analyzed to compare the PI3K/Akt axis and the PANoptotic-like process-related genes in CCSMCs between DMED patients and healthy controls. TMAO-injured CCSMCs were treated with the ROS scavenger N-acetylcysteine (NAC) and the PI3K/Akt inhibitor LY294002. Flow cytometry and Western blotting were used to quantify markers of this death program and pathway activity. T1DMED rats exhibited reduced gut microbiota diversity, with Dorea as the dominant genus. Serum TMAO levels were significantly elevated in the ED group compared with the normal and diabetic non-ED groups. Parabacteroides was positively correlated with TMAO levels, whereas Oscillospira showed a negative correlation. Transcriptome sequencing of TMAO-treated CCSMCs revealed significant enrichment of the PI3K/Akt signaling pathway. IHC and Western blotting confirmed the marked activation of the PANoptotic-like process in penile tissues of T1DMED rats. Analysis of human scRNA-seq data revealed significant upregulation of PI3K/Akt signaling and genes associated with the PANoptotic-like program (e.g., CASP1, CASP3, CASP8, and MLKL) in CCSMCs from DMED patients. Flow cytometry demonstrated increased Caspase-1 and Caspase-3 positivity in TMAO-treated CCSMCs. NAC significantly reduced both readouts, whereas LY294002 significantly reduced Caspase-3 positivity but not Caspase-1 positivity. Western blotting confirmed that PI3K and PANoptotic-like markers were upregulated in TMAO-treated CCSMCs, while α-SMA and eNOS were downregulated. NAC treatment attenuated the TMAO-induced oxidative stress cascade, leading to a reduction in these death markers and restoration of α-SMA/eNOS expression. Similarly, LY294002 attenuated several TMAO-induced molecular changes associated with the PANoptotic-like process and partially restored the expression of CCSMC-associated functional markers. Gut microbiota dysbiosis in T1DMED rats was associated with elevated serum TMAO, and TMAO triggered a PANoptotic-like process in CCSMCs via PI3K/Akt activation, thereby contributing to the progression of DMED.

10. Artificial Intelligence in Semen Analysis and Sperm Selection: Opportunities and Challenges.
PMID:日期:2026-09-21Male factors contribute to approximately half of all cases of couple infertility, and semen analysis remains the cornerstone of the diagnostic evaluation. In recent years, artificial intelligence (AI) has emerged as a promising tool to improve the objectivity and reproducibility of semen analysis and sperm selection in assisted reproductive technologies (ART). This narrative review provides an overview of the current applications of AI in seminology, focusing on sperm motility, morphology, DNA fragmentation, sperm selection for intracytoplasmic sperm injection (ICSI), and sperm retrieval in men with nonobstructive azoospermia. Beyond laboratory applications, the review also highlights the emerging role of machine learning in predicting reproductive outcomes and supporting clinical decision-making in andrology. Although the available evidence is encouraging, most studies remain limited by small and highly selected datasets, a lack of external validation, and scarce evidence on clinically meaningful outcomes. Future progress in the field will likely depend not only on improvements in algorithm performance, but also on the availability of high-quality, representative datasets to enable robust validation and reliable clinical translation. While issues related to data quality, algorithmic bias, and regulatory and ethical aspects still need to be addressed, AI has the potential to become a valuable decision-support tool in reproductive medicine. Nonetheless, further prospective multicenter studies are needed before these technologies can be routinely integrated into clinical practice.
