1. Efficacy and Safety of Polyacrylamide Hydrogel for Stress Urinary Incontinence: A Retrospective Longitudinal US Study with and without OnabotulinumtoxinA.
PMID:日期:2026-09-22Polyacrylamide hydrogel (PAHG) is a urethral bulking agent for stress urinary incontinence (SUI) that received FDA approval in 2020. In mixed urinary incontinence (MUI), PAHG may be used concomitantly with onabotulinumtoxinA to address both urinary stress and urgency components. We sought to evaluate symptom outcomes and safety after PAHG treatment for SUI and to assess outcomes after concomitant onabotulinumtoxinA. We retrospectively reviewed women treated with PAHG for SUI as an outpatient procedure at a single institution between December 2021 and September 2025. Outcomes were compared between baseline and postoperative follow-up intervals of ≤12, 12-24, and 24-45 months. Primary outcomes were changes in the International Consultation on Incontinence Questionnaire-Short Form (ICIQ-SF) and SUI visual analog scale (VAS). Statistical analyses were conducted with < 0.05 considered significant. Among 173 women, ICIQ-SF and SUI-VAS scores significantly improved from baseline across all follow-up intervals ( < 0.001). Pad use decreased at follow-up, and nocturia improved at the first and second follow-up intervals. In the overall cohort, urinary frequency and cough-test positivity numerically improved but did not reach statistical significance. Patients treated with concomitant onabotulinumtoxinA also improved from baseline; however, repeated-measures analysis of variance (ANOVA) did not demonstrate a significant between-group difference compared with PAHG alone. In this study, PAHG was associated with clinically meaningful improvement in SUI symptoms and few complications. Concomitant onabotulinumtoxinA appeared feasible in selected patients with MUI, although superiority over PAHG alone was not demonstrated. Extended follow-up findings should be interpreted cautiously because of attrition at later time points.

2. {"_":"Thyroid Autoimmunity and Reproductive Outcomes: Antibody-Specific Associations in a National Cohort from the Research Program.","i":["All of Us"]}
2. {"_":"甲状腺自身免疫与生殖结局:来自研究计划全国队列中的抗体特异性关联。","i":["All of Us"]}PMID:日期:2026-09-22Thyroid autoimmunity (TAI) is the most common autoimmune condition affecting women of reproductive age. Prior studies have linked TAI to adverse reproductive outcomes; however, antibody-specific effects and their relative clinical significance remain incompletely defined. To evaluate antibody-specific associations between TAI and adverse reproductive outcomes in reproductive-age women using a large national biobank. This retrospective cohort study utilized data from the NIH Research Program (Controlled Data Repository version 8). Reproductive-age women were categorized as antibody-negative or antibody-positive for any thyroid autoantibody (AnyAb), thyroid peroxidase antibodies (TPOAbs), thyroglobulin antibodies (TgAbs), or thyrotropin receptor antibodies (TRAbs). Outcomes included disorders of menstruation, female infertility, endometriosis, and recurrent miscarriage (evaluated in a pregnancy-restricted subset). Associations were assessed using pre-specified pairwise comparisons with Holm-Bonferroni adjustment. TAI was associated with adverse reproductive outcomes in an antibody-specific manner. Disorders of menstruation were more prevalent among women with TPOAb and TgAb positivity compared with controls. Female infertility and recurrent miscarriage were the most robust findings, with strong associations with the AnyAb, TPOAb, and TgAb groups. Endometriosis demonstrated moderate associations driven by TPOAb positivity and overall antibody (AnyAb) positivity. TRAb-specific associations were not consistently significant. Findings varied by antibody type, highlighting heterogeneity within TAI. In this large population-based cohort, TAI was associated with adverse reproductive outcomes, particularly infertility and recurrent miscarriage. These findings refine prior literature by distinguishing confirmatory from novel associations and emphasize the importance of antibody-specific interpretation when evaluating reproductive risk in women with TAI.

3. Building Computable Guidelines and Clinical Decision Support Tools for Cervical, Breast and Colorectal Cancer Screening and Risk Management: A CDC Initiative Update.
PMID:日期:2026-09-22Clinical practice guidelines for cancer screening and management are developed by multiple guideline organizations and published in narrative form. However, guideline concordance can be challenging due to several factors: rapid advances in cancer care, increasing complexity of narrative clinical guidelines, and the unstructured nature of information needed for decisions in electronic health records (EHRs). Clinical decision support (CDS) tools, or computer-interpretable versions of clinical practice guidelines, may help address these challenges when provided to clinicians at the point of care. The Centers for Disease Control and Prevention collaborated with MITRE Corporation and subject matter experts to translate current narrative guidelines for cervical, breast, and colorectal cancer screening and management into standards-based computable guidelines and interoperable CDS tools. The tools apply guideline logic in real-time to patient EHR data to assess screening history and risk and generate recommended actions. Computable guidelines and CDS tools were developed for all three cancers. The cervical cancer CDS was integrated into EHRs and underwent a 6-month clinical pilot evaluation. Breast and colorectal cancer CDS tools progressed to executable code tested with synthetic patient data but have not undergone clinical pilot testing. All three CDS tools are open source, with code available for download. Standards-based computable guidelines can support implementation of complex cancer screening and management recommendations within EHRs. Further development and implementation will require improved structured data capture, interoperable workflows, and sustained support. All three CDS tools require longer-term evaluation to assess usability, acceptability, and clinical outcomes.

5. Prescription Medication Use Among Women Aged 40 and Older in the United States from 1999 to 2020.
PMID:日期:2026-08-26Prescription medication use in the United States has increased over recent decades, with women consistently reporting higher use than men. However, limited research has examined use patterns among women, particularly those 40 years and older who face elevated risks for chronic disease. To assess prescription medication use among U.S. women 40 years and older. Cross-sectional analysis of National Health and Nutrition Examination Survey (NHANES) data from 10 survey cycles spanning 1999-2020. A total of 18,686 women aged 40 years and older with complete prescription medication information. Calendar year modeled as NHANES cycles. Prescription medication use in the past 30 days, overall and by therapeutic class, was estimated for each NHANES cycle. Joinpoint regression estimated average annual percent change (APC). Logistic regression assessed associations between covariates and medication use and polypharmacy (≥5 medications). Polypharmacy among female participants rose from 17.9% in 1999-2000 to 26.0% in 2017-2020, demonstrating an increasing trend early (1999-2004 APC: 7.4, [95%CI: 1.9, 13.6]) followed by stable use (2004-2020 APC: 0.8 [-2.7, 1.9]). Increases in polypharmacy use occurred across racial and ethnic groups and were greatest among women aged 70-85 years. Cardiovascular (1999-2020 APC: 0.9 [0.4, 1.4]) and metabolic (1999-2006 APC: 7.9 [4.1, 15.8]) agent use increased across survey cycles while hormones/hormone modifier use declined (1999-2008 APC: -6.0 [-14.0, -1.3]). Polypharmacy use increased among women 40 years and older, with notable shifts across therapeutic classes. These trends highlight the need for continued surveillance of medication patterns within this population.

7. Prevalence of Opioid-Related Problems and Their Association with Birth Outcomes Among Commercially Insured Pregnant Women.
PMID:日期:2026-08-24Although previous studies report increasing rates of maternal opioid use disorder documented at delivery, the extent to which pregnant women have a clinically recognized opioid-related problem in the year preceding delivery and the effects on pregnancy outcomes are unknown. We used the 2005-2018 MarketScan commercial claims database to identify women aged 13-49 years with a minimum of 12 months of continuous enrollment in the database. Among them, we further identified women with a delivery-related claim who were continuously enrolled for 12 months preceding delivery. Annual rates of opioid-related problems (ORPs), including misuse, dependence, and outdose, and of adverse birth outcomes (cesarean, preterm, and stillbirth delivery) by opioid-related problem status were compared over time. We identified 2,995,032 births from 2,557,438 women who met inclusion criteria. Rates of ORPs in the year before delivery increased from 0.05% in 2006 to a high of 0.34% in 2016, a 7-fold increase, and then fell slightly to 0.29% in 2018. The multivariable regression with linear probability models showed that ORPs were associated with a higher likelihood of cesarean delivery (coefficient: 0.04; 95% confidence interval [95% CI]: 0.03-0.05) and preterm birth (0.05; 0.04-0.05). The effect was more moderate in association with stillbirth (0.00; 0.00-0.01). Rates of ORPs among pregnant women have increased over time and are associated with adverse birth outcomes. Future research should explore differences in birth outcomes by type of ORPs during pregnancy.

8. Update in Sexual Health.
PMID:日期:2026-08-23The goal in selecting these recent articles was to help identify literature that may change the clinical practice of women's health for practitioners in the primary care and general gynecology setting. Articles were identified by reviewing high-impact medical and women's health journals, national guidelines, ACP JournalWise and NEJM Journal Watch. In this Clinical Update, we selected recent publications relevant to the sexual health of women. Specifically, we reviewed guidelines and consensus statements on hypoactive sexual desire disorder (HSDD) and genitourinary syndrome of menopause (GSM), as well as studies around the role of pelvic floor muscle training for GSM and psychological and behavioral health for sexual health in midlife women.

9. A Cluster Randomized Controlled Trial to Improve Influenza and Tdap Vaccination Rates for Pregnant Women.
PMID:日期:2026-08-22We evaluated the impact of a provider and nurse-based intervention on influenza and Tdap vaccination rates among pregnant women. The VAX-MOM trial was a 2-arm cluster randomized clinical trial conducted across 4 health systems (14 obstetric practices in Rochester, NY, and 13 in Los Angeles, CA), from April 2021 to June 2022. Practices were randomized to receive provider and nurse training via online modules (covering influenza and pertussis infections, vaccination, and workflow changes, such as presumptive vaccine recommendations) or standard care. Co-primary outcomes were influenza and Tdap vaccination rates for women delivering live births during the intervention. Analyses adjusted for clustering within practices. The study included 5992 and 6768 women eligible for Tdap vaccination, and 4609 and 5294 women eligible for influenza vaccination (intervention and control practices, respectively). Influenza vaccination rates declined by 5% in the intervention group (57% to 52%) and by 10% in the control group (57% to 47%). Tdap vaccination rates increased by 3% in both groups (67% to 70% in intervention; 72% to 75% in control). Adjusted analyses showed influenza vaccination rates were 8% higher in intervention practices (aRR 1.08, 95% CI [1.00, 1.15], = 0.038; not significant given the < 0.025 cutoff for 2 outcomes). Tdap rates showed no significant difference. Post-hoc analysis excluding one minimally engaged system revealed influenza vaccination rates were 13% higher in intervention practices (aRR 1.13, 95% CI [1.06, 1.19], < 0.001). Provider and nurse training improved influenza vaccination rates but did not significantly affect Tdap coverage in engaged practices.
