1. Recent developments in the role of oxidative stress in pathogenesis and treatment of inflammatory bowel diseases.
PMID:日期:2026-10-03Inflammatory bowel disease (IBD) is a chronic and relapsing disorder of the gastrointestinal tract. Its prevalence has risen rapidly, and nowadays it is a global health concern. Currently available medications reduce inflammation and alleviate the symptoms, but do not cure the disease. Recent findings suggest that dysregulated redox homeostasis may contribute to IBD progression, positioning oxidative stress as one of the primary mechanisms driving inflammation. Data for this review were identified through a comprehensive search of PubMed and Google Scholar for the period from February 2013 to September 2026. This review aims to describe recent approaches focused on oxidative stress and IBD, including the use of oxidative stress biomarkers, treatment options employing neutrophils and nanoparticles, the emerging role of the Nrf2 signaling pathway, ferroptosis suppression, and oxidative stress-related changes in the microbiome. Furthermore, inflammaging and dietary interventions are discussed in the context of oxidative stress, as they modulate redox homeostasis and influence inflammatory pathways. A better understanding of the interplay between ROS, inflammation, microbiome, and dietary interventions may provide valuable insights into mechanisms underlying IBD pathology, while targeting these pathways may facilitate the development of novel, adjunctive therapeutic approaches in IBD management.

2. Preoperative biliary drainage in resectable perihilar cholangiocarcinoma: real-world burden.
2. 可切除肝门部胆管癌的术前胆道引流:真实世界负担PMID:日期:2026-09-23Preoperative biliary drainage (PBD) in perihilar cholangiocarcinoma (pCCA) remains challenging, and its burden among patients who ultimately do not undergo curative-intent surgery is uncertain. We characterized the PBD pathway and identify factors associated with failure of curative-intent management. This retrospective, single-center cohort study included 109 patients with initially resectable pCCA requiring PBD at a tertiary hepatobiliary center between 2010 and 2025. Drainage sequences, complications, surgical and oncological outcomes, and factors associated with surgical exclusion or 90-day mortality were assessed. Resection was achieved in 84 patients (77.1%). Repeated drainage increased septic morbidity (83.1% vs 62.0%, = 0.013). Pancreatitis with a CT Severity Index ≥ 4 or severe acute pancreatitis was associated with surgical exclusion (50.0% vs 20.2%, = 0.048). Drainage-related septic complications independently predicted impaired disease-free survival (HR 3.69, 95%CI 1.58-8.59, = 0.002). A 7-point score including age > 70 years, arterial hypertension, post-drainage pancreatitis, and post-drainage septic complications predicted surgical exclusion or 90-day mortality (AUC 0.766). PBD carries substantial morbidity and may compromise the curative pathway for resectable pCCA. Early expert referral and planned first-attempt drainage are essential. Optimal management of the PBD period before surgery may help to reduce the burden of the surgical resection.

3. Screening strategies for colorectal cancer: adjusting for individual risk factors.
PMID:日期:2026-09-21Colorectal cancer (CRC) is a common cause of cancer-related mortality. While overall incidence has declined, certain populations - particularly younger patients - have experienced a rise. This suggests a more complex risk profile than the dichotomous average vs high risk delineated in guidelines. Risk is currently largely characterized by polyp and family history; however, many additional variables contribute to risk. We synthesize the available data (Pubmed database, focus on article updates in the past 10 years) on non-modifiable and modifiable risk factors for CRC. We explore how these factors could affect screening and surveillance decisions in otherwise average-risk individuals. We then suggest strategies for personalized approaches to CRC screening, explore the role of shared decision making with patients, and highlight future directions for CRC screening. Our understanding of CRC risk must become increasingly more nuanced to adequately serve, screen, and surveil our patients. Precision medicine offers a pathway to accomplish this, but will require significant research to accurately measure risk, and the ability to adapt risk models as our data evolves. This undertaking grows evermore relevant as medicine becomes increasingly personalized and shared decision making becomes the mainstay of counseling between physicians and patients.

4. IPMN beyond the cyst: toward a broader framework of pancreatic and systemic cancer susceptibility.
PMID:日期:2026-09-21Intraductal papillary mucinous neoplasms (IPMNs) are established precursor lesions of pancreatic ductal adenocarcinoma (PDAC). Although current guidelines primarily focus on lesion morphology, emerging evidence suggests that IPMN may also be associated with broader pancreatic and extrapancreatic cancer risk. This review examines pancreatic field cancerization, including concomitant and metachronous PDACs that may arise independently of the index IPMN. We evaluate reported associations between IPMN and extrapancreatic malignancies, including colorectal, gastric, and breast cancers, while considering surveillance bias, age, shared environmental factors, and hereditary susceptibility. We further discuss germline pathogenic variants, including , and , and identify clinical features that may support risk-adapted genetic assessment. Emerging approaches, including liquid biopsy, radiomics, artificial intelligence, and multi-omics, are evaluated as potential tools for individualized surveillance. IPMN may represent more than an isolated pancreatic precursor lesion and may identify selected patients with broader cancer susceptibility. However, current evidence is predominantly observational and does not establish causality. Future management should complement established guidelines with risk-adapted, patient-centered assessment. Emerging technologies may refine individualized risk stratification but require prospective validation before routine clinical implementation.

6. Food-based enteral formulas in pediatric clinical practice: an expert opinion.
PMID:日期:2026-09-14Standard enteral formulas are widely used in pediatric practice but may be associated with gastrointestinal intolerance, limited food diversity, and caregiver dissatisfaction. Growing interest in whole-food feeding has led to increased use of blenderized tube feeds and food-based enteral formulas (FBEFs), although definitions and implementation strategies remain heterogeneous. This expert opinion review summarizes evidence and practice-based insights from an international, multidisciplinary panel of physicians and dieticians on the use of FBEF in pediatric clinical practice. The article was informed by a narrative literature review and a structured expert discussion. Key domains addressed: definition of FBEF, physiological and psychosocial benefits, clinical indications, administration route, transition to adult care; in comparison with homemade blenderized tube feeding. The current evidence, together with real-life experience in clinical practice, suggests that FBEFs are well tolerated and may improve reflux, vomiting, bowel habits, and, caregiver-reported satisfaction. FBEFs offer standardized nutritional composition, food safety, shelf stability, and a lower preparation burden, while still aligning with families' preferences for food-derived ingredients. In our opinion, FBEFs represent a viable option for children requiring long-term enteral feeding, particularly those with intolerance to standard formulas, provided that product selection, monitoring, and caregiver education are individualized.

7. Biliary drainage in perihilar cholangiocarcinoma: historical rationale and the evolving case for selective use.
PMID:日期:2026-09-14Cholestasis prevents optimal treatment of perihilar cholangiocarcinoma (pCCA). To counteract this, biliary drainage (BD) has long been regarded as mandatory prior to any treatment. However, increasing evidence has highlighted the risks associated with BD, notably cholangitis, indicating that while often necessary, drainage requires careful clinical judgment. This narrative literature review outlines the historical development of BD and its rationales, including the restoration of liver function, a prerequisite for liver hypertrophy-inducing procedures, and improvement of performance status. The review also addresses its drawbacks, particularly the risk of cholangitis due to bacterial colonization of the biliary tree and summarizes the evidence shaping contemporary clinical practice. Once regarded as mandatory, BD can no longer be justified as a routine intervention for every pCCA patient, given the associated risks. Undrained partial hepatectomy should be considered in patients with moderate bilirubin levels, adequate future liver remnant volume, absence of cholangitis, normal performance status, and preserved liver and renal function. Still, this applies to only a minority of patients. Similarly, systemic therapy may be feasible without BD in highly selected patients. Future research should focus on defining selection criteria, investigating new biomarkers of cholestasis, and refining drainage techniques to minimize complications of BD.

8. The role of calf circumference in the measurement of sarcopenia in cirrhosis: a narrative review.
PMID:日期:2026-09-07Cirrhosis is a global health challenge associated with significant morbidity and mortality. Sarcopenia is a clinically relevant prognostic marker in cirrhosis but is frequently overlooked in traditional prognostic scores. Gold standard diagnosis relies on imaging modalities that are expensive, not widely available, and involve radiation exposure, creating a need for a simple, reliable surrogate marker that can be incorporated into prognostic systems. Calf circumference (CC) meets these requirements as a practical screening tool with potential future utility within prognostic assessment, pending further validation. A literature search of PubMed, Scopus, Embase, CINAHL, the Cochrane Library, Informed, and the JBI Database of Systematic Reviews (January 2000-November 2025) identified publications on sarcopenia in cirrhosis and the use of CC as a screening tool. International guidelines, systematic reviews, meta-analyses, large observational cohort studies, and narrative reviews were included. This review summarizes the pathophysiology and prognostic relevance of sarcopenia in cirrhosis and evaluates the utility of CC as a cost-effective screening measure with prognostic potential. CC is a feasible, low-cost surrogate marker of sarcopenia in cirrhosis with potential for integration into prognostic scores. Future studies should focus on validating standardized cutoffs and incorporating CC into prognostic models.

10. Gastrointestinal manifestations of common variable immunodeficiency: what a gastroenterologist needs to be aware of.
PMID:日期:2026-08-17Common variable immunodeficiency disorder (CVID) is the most common symptomatic primary immunodeficiency disorder and, given its predilection to affect the gastrointestinal (GI) tract, will be encountered in the practice of a general gastroenterologist, as well as those with more focused interests. Gastrointestinal and hepatic manifestations of CVID - their clinical presentation, endoscopic, imaging and pathological features. Also reviewed, in detail, diagnostic and therapeutic issues that the clinician may be confronted with in dealing with these disorders. Among the protean GI manifestations of CVID a predisposition to enteric infections, chronic gastritis, enteropathy and a form of inflammatory bowel disease feature prominently. Each poses diagnostic and therapeutic challenges; differentiation from celiac disease, Crohn's disease and ulcerative colitis may be challenging, emphasizing the need for awareness of the possibility of CVID when clinical features are atypical. The liver may also be involved, and the gastroenterologist needs to be especially aware of the higher prevalence of gastrointestinal cancer in this patient population.
