8. Return to Sport and Exercise After Direct Anterior Approach Total Hip Arthroplasty: Minimum Five-Year Outcomes.
PMID:日期:2026-10-01Younger, more active patients increasingly undergo total hip arthroplasty (THA) and commonly inquire about expectations and timelines regarding return to sport (RTS) and exercise. The direct anterior approach (DAA) may facilitate faster recovery and decreased instability risk, but long-term RTS data are limited. A retrospective registry review identified all primary DAA THAs from 2010 to 2020. Patients who had less than five-year follow-up were excluded. Eligible patients completed a survey on preoperative and postoperative sport participation, time to RTS, and potential factors limiting RTS. A total of 886 patients (466 women and 428 men) responded to the survey. Their mean age was 62 years (range, 28 to 93), and their mean body mass index was 29.0 (range, 17 to 49). In the year prior to THA, 84% walked, 31% hiked, 27% cycled, and 11% ran. Postoperatively, 74% returned fully to prior activities, and 16% returned with modifications, with 90% of these patients achieving RTS within one year. High-impact sport participation decreased from 24% preoperatively to 10% postoperatively, and running decreased from 11 to 5% (P < 0.001). Those who discontinued running most commonly cited surgeon advice (34%), fear of damaging the joint (21%), and pain (20%) as contributing factors for running cessation. Overall rates of revision were 4.5% for the running cohort and 3.2% for the nonrunning cohort (P = 0.651). At a minimum five-year follow-up, most patients who underwent DAA THA successfully returned to sporting activities, typically within six months postoperatively. The majority resumed low-impact sports, while participation in high-impact activities declined. Patient counseling and fear of joint damage were common reasons for avoiding RTS, despite mixed evidence suggesting increased risks of arthroplasty failure with high-level activities. Future prospective studies are warranted to better guide postoperative activity recommendations and clarify the long-term implications of sport participation after THA.

9. Kinematic and Mechanical Alignment Yield Similar Outcomes in Total Knee Arthroplasty: A Systematic Review and a Meta-Analysis of Randomized Controlled Trials.
PMID:日期:2026-10-01Although mechanical alignment (MA) is the current gold standard for total knee arthroplasty (TKA), suboptimal patient satisfaction rates have prompted the exploration of alternative alignment strategies. This review examined whether kinematic alignment (KA) improves outcomes following TKA compared to MA. There were four databases searched through September 23, 2024, to identify randomized controlled trials (RCTs) investigating TKA using KA versus MA. Patient demographics, survivorship, and patient-reported outcome measures (PROMs) were abstracted. Meta-analyses were performed to compare survivorship and PROMs. Risk of bias was assessed using the RoB 2 tool. There were 11 RCTs and 972 patients included (KA: 484, MA: 488). The mean follow-up was 3.9 years (range, one to 13). There was no statistically significant difference in all-cause reoperation rate between groups (relative risk (RR): 1.34, 95% confidence interval (CI): 0.71 to 2.52, I = 0%, P = 0.37). A meta-analysis of two studies with greater than 10-year follow-up found no statistically significant difference in all-cause reoperations (RR: 1.21, 95% CI: 0.6 to 2.47, I = 0%, P = 0.59) and component revisions (RR: 1.26, 95% CI: 0.38 to 4.14, I = 0%, P = 0.71) between groups. There was no statistically significant difference in PROMs. In two studies including patients who underwent bilateral TKA (KA versus MA), KA was more likely to be the preferred knee (RR: 2.15, 95% CI: 1.36 to 3.40, I = 0%, P = 0.00). There is no significant difference in objective outcomes or PROMs when comparing KA with MA. However, within-subject comparison from bilateral TKA studies indicates patients are more than twice as likely to prefer their KA knee. Future longer-term studies are warranted to better understand the application of varying alignment strategies in TKA, including which populations may benefit most from KA. Level I.

10. Rethinking the Sequence of Arthroplasty-Focused Evaluation: Is It Time to Look at the Radiograph First?
PMID:日期:2026-10-01Internal medicine training models intended for wide, symptom-driven differentials are the source of the conventional orthopaedic diagnostic sequence of history, physical examination, and subsequent imaging. However, this approach may cause inefficiencies in modern arthroplasty-focused clinics that are inconsistent with the structural basis of the majority of hip and knee pathologies. In high-volume clinics, patients are frequently roomed, sent for radiography, and then reroomed, which interferes with productivity and results in task switching and cognitive fragmentation. Early access to plain radiographs may improve patient safety and diagnostic efficiency given the likely pretest risk of structural joint disease in arthroplasty groups. This commentary suggests a triage-directed strategy wherein advanced care professionals, intake workers, or decision-support tools apply established intake criteria to gather relevant radiographs before the orthopaedic encounter. When utilized carefully, early imaging enables surgeons to place the physical examination and history in the context of known anatomy, promotes more targeted clinical inquiry, and may minimize needless or potentially uncomfortable examination procedures. Although there are legitimate worries about diagnostic anchoring, skilled orthopaedic doctors frequently modify diagnostic ideas when imaging and clinical results conflict, maintaining solid clinical reasoning. This approach is already used in many orthopaedic contexts, including trauma and postoperative surveillance, where imaging already precedes examination. Importantly, triage-directed early imaging is not meant to encourage indiscriminate testing or take the place of a thorough examination. Instead, it is an advancement in diagnostic sequencing designed for an orthopaedic practice with a large volume and a structural focus. Early radiograph availability may increase productivity, maintain cognitive concentration, and improve patient-centered treatment without sacrificing diagnostic rigor if it is carefully adopted and backed by protections for unusual presentations.
