1. The effectiveness of digital health and telehealth interventions for gynecologic cancer survivors: A systematic review.
PMID:日期:2026-08-26With the rising burden of gynecologic cancers, survivors face complex long-term challenges, making digital health and telehealth interventions increasingly relevant for supportive survivorship care. A systematic review was conducted, using data sourced from Web of Science, PubMed, Cochrane Library, CNKI, Ovid MEDLINE, Wiley Online library journals, and Scopus from 2010 to December 31, 2024. Following PRISMA guidelines, empirical intervention studies delivered through digital health or telehealth modalities and focused on improving survivorship-related outcomes in gynecologic cancer survivors were included. As a result, a total of 17 studies were analyzed in this systematic review. Results indicate that digital health and telehealth interventions significantly enhance quality of life, alleviate psychological distress, and promote patient satisfaction and self-care capabilities among gynecologic cancer survivors. This systematic review suggests that digital health and telehealth interventions are promising approaches to supportive survivorship care for gynecologic cancer survivors, while highlighting the need for more rigorous study designs, clearer intervention reporting, and stronger evidence on long-term effectiveness.

2. Principles and applications of cross-sectional, case-control and cohort studies in gynecologic oncology: from classical observational designs to target trial emulation.
PMID:日期:2026-08-19Observational research plays an important role in gynecologic oncology. Purposes of such studies include, but are not limited to, examining disease etiology, quantifying disease burden, identifying risk or protective factors, and evaluating the effectiveness and safety of pharmacotherapies and non-pharmaceutical interventions. Building on fundamental principles of observational study design, this article systematically introduces the basic concepts, key design considerations, and main strengths and limitations of three classic study designs: cross-sectional, case-control, and cohort studies in gynecologic oncology, supported by customized, practical examples. Based on the target trial emulation framework, we further elaborate on approaches to addressing common methodological challenges, such as immortal time bias, selection bias, and time-dependent confounding. Finally, we provide concise guidelines for the selection of study design for different research questions, with illustrative examples. This article aims to equip researchers in gynecologic oncology with the most readily useful and up-to-date set of epidemiological tools, thereby promoting broader and more appropriate use of real-world data to generate scientific and clinical evidence in gynecologic oncology.

3. Prevalence and risk factors of preoperative anxiety in women undergoing cesarean section: A global systematic review and meta-analysis.
PMID:日期:2026-08-19Preoperative anxiety is a significant psychological concern among women undergoing cesarean delivery, with potential implications for maternal and neonatal outcomes. This systematic review and meta-analysis aimed to (1) quantify the global prevalence of preoperative anxiety in women undergoing cesarean sections, (2) identify key risk factors, and (3) assess geographical variations in anxiety patterns. Following PRISMA guidelines, we systematically searched PubMed, Scopus, Web of Science, and Cochrane Library without time restrictions, including studies published in English or Persian, up to April 2025. Observational studies reporting prevalence and/or risk factors of preoperative anxiety in women (≥18 years) undergoing cesarean delivery were included. Study quality was assessed using a modified version of the Newcastle-Ottawa Scale, and heterogeneity was evaluated via the I and Q statistic. Random-effects meta-analyses with Freeman-Tukey transformation were conducted for pooled prevalence estimates, with subgroup analyses by geography, income level, and assessment tools. Meta-regression explored temporal trends and moderators. Of 950 identified records, 24 studies (n = 3928) were included. The pooled prevalence of preoperative anxiety was 66.7% (95% CI: 58.3-74.5%), with high heterogeneity (I = 96.7%). Prevalence varied significantly by country (25.0% in Malaysia to 98.0% in Turkey), WHO region (p < 0.001), and income level (70.8% in upper-middle-income vs. 41.0% in high-income countries; p < 0.001). Risk factors included higher education (OR = 1.67, 95% CI: 1.25-2.22) and primiparity (OR = 1.62, 95% CI: 1.03-2.57). No difference was observed between elective and emergency cesarean deliveries (p = 0.670). Meta-regression revealed a temporal decline in prevalence (β = -0.068 per year, p = 0.003). This meta-analysis suggests that nearly two-thirds of women experience preoperative anxiety before cesarean delivery, with higher burden in upper-middle-income countries and among educated or first-time mothers. The lack of difference by surgical urgency underscores the need for universal anxiety screening and tailored interventions. Geographical disparities and declining temporal trends highlight the importance of context-specific strategies and further research into underlying mechanisms.

4. Second trimester neurosonography.
PMID:日期:2026-08-14Congenital malformations of the central nervous system (CNS) are among the most common congenital anomalies, with neural tube defects being the most prevalent, comprising approximately 40% of CNS malformations. The estimated prevalence of CNS anomalies varies, but many cases remain undiagnosed at birth, potentially leading to underestimation. These conditions significantly impact neurodevelopmental outcomes and contribute to a high societal burden due to the need for specialized medical care and support. Prenatal imaging plays a crucial role in the early detection of CNS anomalies. Routine mid-trimester ultrasound, performed between 18 and 24 weeks of gestation, is the primary screening method, with detection rates ranging from 68% to 92%. Standardized guidelines by the International Society of Ultrasound in Obstetrics and Gynecology (ISUOG) recommend acquiring three axial planes-transventricular, transthalamic, and transcerebellar-to improve diagnostic accuracy. The addition of mid-sagittal plane evaluation may further enhance detection rates. When abnormalities are suspected, targeted fetal neurosonography (with transvaginal probes), incorporating coronal and sagittal planes, is necessary for detailed assessment and diagnosis. Accurate prenatal assessment of CNS anomalies is critical for diagnosis, prognosis, and management planning. Standardized approaches and advanced imaging techniques contribute to improved detection and better-informed clinical decision-making, ultimately enhancing outcomes for affected fetuses.

5. Preeclampsia in twins: How different from singletons.
PMID:日期:2026-08-06Hypertensive disorders of pregnancy contribute significantly to maternal and perinatal mortality and morbidity. Twin pregnancies which already carry an increased risk to mothers and fetuses, are affected in more significant numbers, earlier gestations and more severely by preeclampsia. This narrative review assesses the current state of research on maternal haemodynamics in twin pregnancy and the specific changes in those affected by preeclampsia. We assessed current literature focusing on maternal haemodynamics, angiogenic markers and screening models in twin versus singleton pregnancies. Relevant literature was identified through a search of electronic databases using keywords such as "twin pregnancy", "preeclampsia" and "maternal haemodynamics". Studies were included based on their relevance to cardiovascular adaptation and clinical outcomes in twin pregnancy. We highlight the unique challenges of diagnosing and managing preeclampsia in twin pregnancies compared to singleton pregnancies.

6. How to measure ophthalmic artery Doppler for pre-eclampsia screening.
[英文摘要] Pre-eclampsia remains a leading cause of maternal and perinatal morbidity, affecting 3-5% of pregnancies globally. Screening in most hospital settings relies on risk factors, and only a few hospitals perform individualised risk screening using maternal factors, mean arterial pressure, uterine artery Doppler, and biochemical markers. Maternal ophthalmic artery (OA) Doppler has emerged as a significant marker of cerebrovascular and systemic hemodynamic adaptation. The peak systolic velocity (PSV) ratio reflects an integrated measure of cardiac output and peripheral vascular resistance. To provide a standardised, practical guide for acquiring high-quality ophthalmic artery Doppler measurements to be incorporated into clinical pre-eclampsia screening protocols. The guide outlines a nine-step acquisition technique using a high-frequency (7-18 MHz) linear-array transducer. Following a 5-min rest period, measurements are taken with the patient supine, and the probe is gently placed on the closed eyelid. The ophthalmic artery is identified superior and medial to the optic nerve, with the sample gate positioned 10-15 mm posterior to the globe. To ensure accuracy and reproducibility, an insonation angle of <20° is required, and measurements should be averaged from both eyes. Ophthalmic artery Doppler provides significant incremental value to screening algorithms. In the second trimester, adding the PSV ratio improves preterm pre-eclampsia detection from 72% to 89% at a 10% false-positive rate. The technique is particularly advantageous in resource-limited settings due to its low equipment requirements and rapid learning curve. Safety is maintained by adhering to the ALARA principle, specifically keeping the Mechanical Index (MI) < 0.23 and Thermal Index (TI) < 1.0. Ophthalmic artery Doppler is an accessible, safe, and reproducible tool for assessing maternal haemodynamics. Adhering to a standardised acquisition technique is essential for its successful integration into multimodal pre-eclampsia screening.

7. Impact of implementation of twin-specific growth charts on adverse perinatal outcomes in twin pregnancies: an interrupted time series analysis.
PMID:日期:2026-07-25Twin pregnancies are associated with higher perinatal mortality and morbidity. The assessment of fetal growth in these pregnancies using singleton or twin charts remains controversial. The aim of this study was to evaluate the impact of the implementation of twin-specific growth charts on adverse perinatal outcomes in twin pregnancies. This is a retrospective cohort study of twin pregnancies delivered at St George's University Hospitals (2000-2023). Twin-specific growth charts were implemented in July 2017; 2020 was excluded due to COVID-19-related confounding. Pregnancies with both fetuses alive at ≥23 weeks were included, excluding cases with anomalies, monoamniotic twins, twin-to-twin transfusion syndrome, termination, or incomplete data. Pre- (2000-June 2017) and post-intervention (July 2017-2023) periods were compared. Outcomes included small-for-gestational age (SGA), fetal growth restriction (FGR), preterm birth (<37 weeks), late preterm birth (34-36 weeks), iatrogenic preterm birth, stillbirth, and neonatal unit admission. Interrupted time series (ITS) analysis with segmented regression of semiannual data assessed immediate and trend changes following implementation. 2008 twin pregnancies were analysed, including 1533 in the pre-intervention and 475 in the post-intervention period. Following implementation, the rates of SGA and FGR diagnoses per fetus decreased substantially (SGA: 53.2% vs. 11.0%; FGR: 47.1% vs. 6.8%; both p < 0.001). ITS analysis demonstrated immediate reductions in FGR (-40.0%, p < 0.001) and SGA (-42.8%, p < 0.001) diagnoses following the policy change, with no significant change in trend. Iatrogenic preterm birth showed a significant immediate reduction (-13.6%, p = 0.016), although without a sustained trend. No intervention effects were identified for stillbirth or neonatal unit admission rates (p > 0.05 for both). Use of twin-specific charts for antenatal growth surveillance was associated with fewer diagnoses of SGA and FGR and an immediate, but not sustained, reduction in iatrogenic preterm birth. No worsening was observed in the adverse perinatal outcomes assessed, including stillbirth and neonatal unit admission.

8. Role of three-dimensional ultrasound in the diagnosis and staging of gynecological malignancies: Focus on endometrial and cervical cancer.
PMID:日期:2026-07-24Three-dimensional (3D) ultrasound has emerged as a valuable non-invasive imaging modality in gynecologic oncology, particularly in the evaluation of endometrial and cervical cancers. This technique offers enhanced spatial visualization of pelvic anatomy compared to conventional two-dimensional ultrasound, improving diagnostic accuracy and preoperative staging. In endometrial cancer, 3D ultrasound provides detailed assessment of endometrial thickness, volume, and vascular patterns through power Doppler angiography, aiding in the detection of malignancy. Furthermore, it allows for accurate measurement of myometrial invasion depth, which is a critical factor in staging and treatment planning. In cervical cancer, 3D ultrasound facilitates delineation of tumor margins, stromal infiltration, and parametrial involvement, contributing to more precise local staging. The integration of multiplanar reconstruction and volume contrast imaging improves the sensitivity and specificity of lesion characterization. Several studies have demonstrated that 3D ultrasound approaches the diagnostic performance of magnetic resonance imaging (MRI) for local staging, while being more accessible, cost-effective, and tolerable for patients. Despite these advantages, limitations include operator dependency and reduced image quality in certain patient populations. Nevertheless, ongoing advances in ultrasound technology and standardized protocols are enhancing its clinical utility. In this narrative review we summarize current evidence regarding the role of three-dimensional ultrasound in the diagnosis and staging of endometrial and cervical cancer.

9. Occupational exposures and maternal mental health across the perinatal period: a systematic review.
PMID:日期:2026-07-06A growing proportion of women work during pregnancy and return to employment within the first postpartum year. Their working conditions may shape maternal mental health, yet the evidence is uneven. We reviewed how occupational exposures relate to the full range of maternal psychological outcomes, from distress to wellbeing, across pregnancy and the return to work. Ten studies met the criteria. All addressed psychosocial exposures, only two also physical and ergonomic demands, and none chemical or biological hazards. Adverse psychosocial conditions, including job strain, pregnancy discrimination, and poor job quality, predicted higher depression and anxiety in both periods, with perceived stress as a pathway. Protective resources, such as schedule flexibility and social support, helped only under specific conditions. The mental health impact of non-psychosocial exposures is therefore a structural gap, relevant to antenatal care and occupational medicine.

10. Induction of labor in twin pregnancies: systematic review and meta-analysis.
PMID:日期:2026-07-03To evaluate the maternal and perinatal outcomes of twin pregnancies undergoing induction of labor (IOL). MEDLINE, EMBASE, CINAHL, Cochrane database and the Scopus databases were searched electronically up to May 2024 (PROSPERO CRD42024520118). All studies regarding maternal or neonatal outcomes of induction of labor in twins were included. Language was restricted to English. Four different analyses were performed: IOL was compared to spontaneous onset of labor, elective Caesarean delivery, IOL in singletons and at last different methods of IOL in twin pregnancies were compared. Maternal and neonatal outcomes were analysed. Random-effect head-to-head meta-analyses were used to analyze the data. Quality assessment was performed using ROBINS-I for non-randomised and RoB2 for randomised studies. The comparison of IOL and spontaneous onset of labor (8 studies) showed that the incidence of unplanned Caesarean delivery (CD) was similar between the two groups (pooled OR 0.94, 95% CI 0.36-2.45; p = 0.89). Neonatal deaths were significantly lower in the IOL group compared to the spontaneous onset of labor group (pooled OR 0.22, 95% CI 0.10-0.51, p = 0.0003 for Twin 1 and 0.29, 95% CI 0.14-0.60, p = 0.008 for Twin 2). On the other hand, the comparison of IOL with planned CD (7 studies) demonstrated that the rate of composite adverse maternal outcome was significantly higher in women undergoing IOL compared to women who had planned CD (pooled OR 2.48, 95% CI 1.16-5.31; p = 0.02). However, no significant differences were observed among these two groups regarding neonatal complications (p > 0.05). The comparison of IOL outcomes in twin and singleton pregnancies (3 studies) showed no significant difference in the rate of unplanned CD (pooled OR 1.42, 95% CI 0.89-2.28; p = 0.14). At last, the comparison of different methods of IOL (5 studies) showed similar rates of Caesarean deliveries were observed in the different groups (pooled OR 1.01, 95% CI 0.56-1.85; p = 0.97). The risk of bias assessment showed overall low risk of bias for observational studies (ROBINS-I) and some concerns for the randomized studies (RoB2). According to the GRADE system, the quality of evidence was overall low to moderate. IOL may be considered a safe option in twin pregnancies as it is significantly associated with a lower risk of neonatal mortality and does not seem to increase the risk of unplanned CD when compared to the spontaneous onset of labor. However, IOL is associated with higher maternal morbidity when compared to elective Caesarean delivery. Larger studies are needed to further evaluate predictive factors of unplanned CD after IOL among these pregnancies.
