    This study aimed to assess the clinical and histopathological effects of maximal available and supramaximal energy exposure in direct selective laser trabeculoplasty (DSLT®) on target and nontarget ocular tissues compared with selective laser trabeculoplasty (SLT) and sham treatments. Thirty eyes of 16 Dutch Belted Rabbits were allocated into seven groups: (A) Maximal energy DSLT exposure (2.6 mJ), (B) supramaximal energy DSLT exposure (3.5 mJ) with double nominal density spot spacing, (C and D) nontarget DSLT on sclera and iris at maximal and supramaximal energy, (E) SLT treatment at maximal energy (2.6 mJ), (F) SLT treatment with two overlapping shots at maximal energy (2.6mJ), and (G) sham control. The eyes were examined using a biomicroscope slit lamp, anterior segment OCT (AS-OCT), and a specular microscope at baseline, Day 1, and Weeks 1, 2, and 4 post-treatments. Histopathological evaluation was conducted on eyes enucleated on Days 1 and 29 posttreatment. The study was conducted at the Harlan laboratory, Ness-Ziona, Israel. All DSLT and SLT laser treatments were uneventful. Minor acute responses were observed 1-day posttreatment at the slit lamp biomicroscopy exam, with similar DSLT and SLT responses. No significant clinical or histopathological findings were observed in any treatment groups at 29 days posttreatment. No notable findings indicative of damage to nontarget tissues were noted, including damage to the limbal stem cells. DSLT treatment at the limbus, iris, and sclera yielded no significant clinical or histopathological changes compared to SLT or sham eyes, even with supramaximal energy and double-density exposures.

    To evaluate the effects of antimicrobial photodynamic therapy (aPDT), systemic pharmacological therapy, and their combination on irradiated mandibular post-extraction bone repair relevant to osteoradionecrosis (ORN) development in a rat model. Twenty-six male Wistar rats were initially randomized into five experimental groups: non-irradiated extraction control, irradiated extraction plus systemic pharmacological therapy, irradiated extraction plus combined systemic pharmacological therapy and aPDT, irradiated extraction plus aPDT alone, and irradiated extraction without post-extraction treatment. The model included a single 20 Gy focal irradiation dose to the left mandible, followed by the first mandibular molar extraction 10 days later. During the experiment, some animals were lost due to procedural causes unrelated to treatment, resulting in final group sizes of 5, 5, 3, 4, and 6, respectively. The results were evaluated by micro-computed tomography (p = 0.02), radiographic examinations, histopathological analysis, and assessment of hepatic steatosis. Irradiated untreated animals showed the most pronounced structural and histopathological alterations. All treatment protocols improved bone repair compared with the irradiated untreated group. However, the combined protocol consistently showed the most favorable structural, radiographic, and histological findings, with better preservation of bone architecture and more organized osteogenesis. aPDT combined with systemic pharmacological therapy may represent a promising multimodal strategy for irradiated mandibular bone injury relevant to ORN development.

    This study aimed to evaluate the efficacy of photobiomodulation therapy (PBMT), therapeutic ultrasound (TUS), boric acid (BA), and their combinations in preventing experimentally induced intra-abdominal adhesions in rats. Ninety-six male Wistar rats (14 weeks old) were randomly assigned to eight groups (Control, Laparotomy, Physiological Saline, BA, PBMT, TUS, BA + PBMT, and BA + TUS; n = 12/group). Adhesions were surgically induced. Treatments were applied once daily for 7 consecutive days as follows: BA (8 mg/kg, intraperitoneal), PBMT (904 nm, 3 J/cm applied to the incision site), and TUS (1 MHz, 0.5 W/cm applied to the incision site). On Day 8, rats were euthanized, and peritoneal fluid and adhesion tissue samples were collected for macroscopic scoring, histopathology, biochemical assays, and gene expression analyses. PBMT alone significantly reduced adhesion scores compared with the control (p < 0.05) and showed the most pronounced antifibrotic effect, while BA and TUS, alone or in combination, had limited impact. BA treatment increased malondialdehyde (MDA) levels (p < 0.05), indicating higher oxidative stress, whereas PBMT enhanced antioxidant enzyme activities, including superoxide dismutase (SOD) and catalase (CAT) (p < 0.05). Gene expression analysis revealed that PBMT modulated cytokines linked to inflammation, particularly tumor necrosis factor-α (TNF-α) and transforming growth factor-β1 (TGF-β1). PBMT demonstrated significant potential in reducing intra-abdominal adhesions by alleviating inflammation, controlling oxidative stress, and promoting tissue repair. BA and TUS showed limited efficacy under the present experimental conditions, and BA in particular was associated with increased oxidative stress and inflammatory responses. Further research with optimized dosing, extended treatment durations, and combination strategies is warranted.

    Clinicians frequently use diode laser-assisted oral frenectomy to correct anatomical restrictions in the oral cavity as a minimally invasive alternative. However, evidence for its effect on wound healing and on reducing common postoperative complications remains unclear. This systematic review and meta-analysis aimed to evaluate the clinical efficacy of diode laser-assisted frenectomy compared with the conventional scalpel technique. This review adheres to the PRISMA statement, and the protocol is registered in PROSPERO (CRD420251050170). A comprehensive electronic and manual search identified comparative studies of diode laser vs. scalpel frenectomy from January 2015 to December 2025. The analysis pooled the primary outcomes (postoperative discomfort, intraoperative hemorrhage, and wound healing) with 95% confidence intervals (CI) using a random-effects model. Heterogeneity assessment used the I statistic. Visual inspection of funnel-plot symmetry assessed publication bias. Fourteen studies met the inclusion criteria. The meta-analysis showed that the diode laser reduced postoperative pain on day 1 (SMD = -1.73; 95% CI: -2.70 to -0.77) and day 7 (SMD = -1.84; 95% CI: -2.78 to -0.90), compared with the scalpel. The diode laser also reduced intraoperative hemorrhage (SMD = -1.97; 95% CI: -2.89 to -1.05). Wound healing on day 7 did not differ significantly between groups. This meta-analysis indicates that diode laser-assisted frenectomy reduces postoperative pain, significantly. In addition, the laser group experienced less intraoperative hemorrhage and better wound healing at 7 day. Future clinical trials should standardize wound-healing indices, define uniform follow-up intervals, and incorporate cost-effectiveness analyses.

    6. Laparoscopic Methylene Blue Photodynamic Therapy for Intraabdominal Infection: A Preclinical Feasibility Study in a Rabbit Model of Perforated Appendicitis.

    Perforated appendicitis commonly results in intra-abdominal infection requiring prolonged antibiotic therapy. Intraoperative antimicrobial adjuncts are limited, and photodynamic therapy (PDT) may provide a targeted, resistance-independent approach. This study evaluated the feasibility, safety, and preliminary antimicrobial effects of laparoscopic methylene blue photodynamic therapy (MB-PDT) for intra-abdominal disinfection in a rabbit model of perforated appendicitis. New Zealand White rabbits (n = 19) underwent surgically induced perforated appendicitis via appendiceal ligation and electrocautery perforation. Animals subsequently received laparoscopic MB-PDT (n = 9) or control treatment (methylene blue lavage without laser illumination; n = 10) 24-40 h after perforation. MB-PDT consisted of peritoneal lavage with methylene blue (300 µg/mL) followed by 665-nm laser illumination at 20 mW/cm to a fluence of 25 J/cm. Peritoneal aspirates were obtained before and 24 h after intervention to quantify bacterial burden. In vitro PDT was evaluated in isolated organisms. Intraperitoneal organs were assessed histologically for off-target effects. Primary outcomes were feasibility, safety, and in vivo change in bacterial burden. Secondary outcomes included clinical parameter changes and in vitro efficacy. Fourteen rabbits developed peritonitis, and 10 animals completed follow-up (PDT n = 7, control n = 3). Laparoscopic MB-PDT delivery was technically feasible and no histologic evidence of off-target photodynamic injury to abdominal organs was identified. Changes in bacterial burden were not significantly different between MB-PDT and control animals (p = 0.18). Body temperature showed a non-significant trend toward reduction following MB-PDT (-0.37°C ± 1.32°C) compared with controls (0.26°C ± 0.76°C; p = 0.27). In vitro MB-PDT produced significant reductions in bacterial burden across all isolated species (p < 0.001), with the most resistant Gram-positive isolate, Enterococcus faecalis, demonstrating greater susceptibility than Gram-negative species. Laparoscopic MB-PDT was technically feasible and safe in a rabbit model of perforated appendicitis. Although a reduction in bacterial burden was not observed in vivo, MB-PDT produced strong antimicrobial activity in vitro against all bacterial species isolated from infected animals. These findings establish a preclinical technical platform for intra-abdominal MB-PDT and highlight the importance of optimized light-delivery strategies for future translational studies.

    8. Evaluating the Use of High-Intensity Non-Focused Ultrasound Treatment to Improve Skin Laxity and Facial Wrinkles in Asian Patients.

    This study aims to investigate the safety and efficacy of high-intensity, non-focused, parallel ultrasound treatments to lift lax tissue in the submental and neck zones, lift the eyebrow, and improve facial wrinkles in an Asian population. This prospective, open-label, non-randomized single-center, self-controlled clinical study recruited 42 participants aged 35 to 67. Participants received a single treatment, covering the full face, neck, and submental zones. Standardized images were taken, and assessments were performed using the Physician Global Aesthetic Improvement Scale (PGAIS) and Fitzpatrick Classification of Wrinkling and Degree of Elastosis. 3D images were taken to calculate the volume change after treatment. Among the 42 patients, 83.3% showed improvement in submental and neck skin laxity at the 3-month follow-up. Improvements were also observed in eyebrow elevation (28.6%) and facial wrinkles (38.1%) at 3 months. The Fitzpatrick Wrinkle and Elastosis Scale showed improvement in 26.2% of patients at 3 months, with sustained effects up to 9 months in 12.8% of patients. A trend of volumetric reduction of 1.04 ± 3.49 ml (p = 0.06) was demonstrated in the jawline and submental area. Transient edema and erythema were noted in 7.2% and 50% of patents initially but resolved on subsequent follow-up visits, and no other adverse events were documented. High-intensity, non-focused, parallel ultrasound is effective in improving skin laxity and facial wrinkles in Asian patients, with minimal side effects and good tolerability.

    This retrospective study evaluated the clinical outcomes associated with a photobiomodulation therapy (PBMT) protocol in patients with inferior alveolar nerve (IAN) injury following oral and maxillofacial surgery. The study specifically investigated whether the etiology of paresthesia, the time elapsed before treatment initiation, and the number of PBMT sessions were associated with variations in clinical outcomes. A retrospective analysis was conducted on 66 affected sides in 55 patients (39 female and 16 males; mean age 35.69 ± 14.49, range 15-80). The study sample consisted of patients with IAN injury due to maxillofacial surgery who were treated with PBMT and followed for at least 6 months. Patients were grouped based on the number of sessions (< 15 vs. ≥ 15), timing of treatment initiation (within vs. after 1 month), and surgical etiology (orthognathic vs. other procedures). Clinical neurosensory tests (two-point discrimination, light touch, pinprick) and Visual Analog Scale (VAS) assessments were performed. Improvements in VAS scores and clinical neurosensory test results were observed during the follow-up period (p < 0.05). No statistically significant differences were found between groups regarding overall outcomes. Although earlier treatment initiation and a higher number of sessions were associated with higher neurosensory scores, these factors did not reach statistical significance in multivariable analysis. Patients with orthognathic surgery-related injuries demonstrated significantly lower neurosensory scores compared to other surgical etiologies (p < 0.05). Follow-up duration was not significantly associated with clinical outcomes. Within the limitations of this retrospective observational study, PBMT was associated with improvements in neurosensory outcomes in patients with IAN injury. However, due to the absence of a control group and the heterogeneity of surgical etiologies, these findings should be interpreted with caution, and a causal relationship cannot be established. Further well-designed controlled studies are needed to clarify the potential role of PBMT.

    To investigate the antifungal effects of berberine-mediated photodynamic therapy (BBR-PDT) against Trichophyton rubrum (T. rubrum) both in vitro and in vivo, and to elucidate its underlying mechanisms of action. The minimum inhibitory concentration (MIC) of BBR-PDT against T. rubrum was determined according to the CLSI M38-A2 guidelines. The MTT assay was used to evaluate the effects of various BBR concentrations and light doses, with results subsequently validated by colony diameter measurements and spore germination assays. Intracellular reactive oxygen species (ROS) levels were measured using DCFH-DA, and transmission electron microscopy (TEM) was used to assess cellular damage in T. rubrum. A guinea pig skin infection model was established, and therapeutic efficacy was evaluated using clinical skin scoring, fungal culture, and histopathological analysis (H&E staining). In vitro experiments showed that at a light dose of 96 J/cm, the MIC of BBR was 12 μg/mL. Neither light irradiation alone nor BBR monotherapy exhibited antifungal activity. Combined treatment with 12 μg/mL BBR and 72 J/cm blue light significantly inhibited radial hyphal growth and spore germination of T. rubrum. Mechanistic investigations revealed that BBR-PDT significantly increased ROS levels within spores, and T. rubrum exhibited extensive cellular damage, including membrane rupture, cell wall loosening, cytoplasmic vacuolization, and organelle degeneration. In the guinea pig model, BBR-PDT significantly reduced scaling, promoted hair regrowth, and improved clinical skin scores. Fungal colony counts from skin cultures were significantly reduced, and H&E staining demonstrated restoration of normal skin structure. BBR-PDT exerts antifungal effects via ROS-mediated oxidative stress. This oxidative stress disrupts T. rubrum ultrastructure through multiple mechanisms. The therapy is effective against T. rubrum in vitro and in vivo. Therefore, BBR-PDT holds promise as a treatment for dermatophyte infections.
