心脏副神经节瘤是罕见的神经内分泌肿瘤（NETs），可通过占位效应、栓塞事件及儿茶酚胺过多导致不良后果。本病例报告描述了一名52岁女性，既往有快速心室率型心房颤动病史，因缺血性脑卒中就诊。偶然的影像学检查发现一个8 cm的血管前间隙肿块，紧邻左心室，伴有中央坏死区域。尿甲氧基肾上腺素检测升高，提示为功能性肿块。Cu-64 DOTATATE PET/CT证实无远处转移，且肿块边缘放射性示踪剂摄取增高，支持NET的诊断。CT引导下经皮同轴空心针活检证实为副神经节瘤。切除术后，术后尿甲氧基肾上腺素水平恢复正常。本报告强调了多学科方法在管理缺血性脑卒中后就诊的心脏副神经节瘤中的重要性。
Abstract
Cardiac paragangliomas are rare neuroendocrine tumors (NETs) that may cause morbidity through mass effect, embolic events, and catecholamine excess. This case report describes a 52-year-old woman with a history of atrial fibrillation with rapid ventricular response presenting with ischemic stroke. Incidental imaging revealed an 8-cm prevascular mass abutting the left ventricle with regions of central necrosis. Urinary metanephrine testing was elevated, indicating a functional mass. Cu-64 DOTATATE PET/CT confirmed absence of distant metastases and increased radiotracer uptake along the rim of the mass, which supported the diagnosis of a NET. CT-guided percutaneous coaxial core needle biopsy confirmed the diagnosis of a paraganglioma. Upon resection, postoperative urinary metanephrine levels normalized. This report highlights the multidisciplinary approach to the management of a cardiac paraganglioma presenting after ischemic stroke.
